ePub documents are very finicky. The Word docx has to be completely clean and free of residual styles that may have lingered. Only basic formatting listed in my presentation will work. Trying to add extra styles and formatting in the docx file before conversion will lead to problems. I will do the basics in Word, then convert to an ePub, then stylize the rest in Calibre.
What about converting into an epub3 file that contains audio and video? Can you send me to some instructions for doing that in Calibre please? Thank you.
Captions won't work in the initial conversion... only the basics as shown in the video. Captions and special formatting will have to be done in the Calibre editor.

I did this with a word doc but the images became scattered and the word and image styles are gone. What mistakes may create these problems?

At the "output" section, if you tick the box "preserve cover aspect ratio", calibre will not distort images. Hope this can fix your problem
