As soon as the lights started turning on, the brightness was rapidly reduced over a period of about 1 minute and 20 seconds

It has a rotary switch with a reliable click feeling, not a push switch, so mis-lighting is very unlikely to occur.
If you keep the PD40R in a backpack with the PD40R in the holster that comes with it, mis-lighting will be 100% avoided.
Yes, there is a spot on the surface illuminated by this flashlight.
And there is not much dark area on the border between the spot and the spill, so it seems brighter.

I don't own any measuring equipment so I can't measure it, but I think it's generally accurate.

Turning the switch vigorously to the right end instantly switches to strobe mode.
