    The respiratory examination is one of the three core clinical skills, respiratory, cardiac and abdominal examinations. In this video, we demonstrate how to perform a clinical examination of the RESPIRATORY SYSTEM for your medical school Clinical Skills OSCE. As the resp exam is sure a core skill when it comes to examining patients, students should assume that a respiratory assessment is a high yield station for any clinical exams or clinical assessments.
    For a passing grade in your Clinical Skills OSCE, for resp examination follow the approach of:
    - Inspection
    - Palpation
    - Percussion
    - Auscultation
    HOWEVER, a respiratory examination OSCE station does not just involve auscultating the lungs, this video also demonstrates some of the specialised examination techniques required in examining patients such as tactile vocal resonance, and checking the JVP which may be seen to be elevated in pulmonary hypertension
    Coughs, colds and general concerns about the chest are a common reason for patients to see a doctor, and in any speciality is probably the most commonly performed patient examination

    Please note that there is no ABSOLUTE way to perform a clinical examination. Different institutions and even clinicians will have differing degrees of variations - the aim is the effectively identify medically relevant signs.
    However during OSCE assessments. Different medical schools, nursing colleges and other health professional courses will have their own preferred approach to a clinical assessment - you should concentrate on THEIR marks schemes for your assessments.

      ​@@lars1296 Medical problems occur with little to no obvious symptoms.
      Occasional check-ups are done so as to not let them go undetected and worsen, potentially to the point of being untreatable.
      As an example, one life-threatening illness that should be checked for regularly is cancer, particularly from the 40s onward because that's the age range most cancer occurs in - with a _very_ high prevalence.
      There's an adage about this - "prevention is better than a cure".

    What’s the difference between medical and clinical? And why would Dr Gill have the patient say “one, two” and not “ninety-nine”?

      "Ninety-Nine" is a mistranslation. Vocal fremitus was discovered by a German Doctor. The sounds transmitted through the lung changes if there is a consolidation (eg infection). The best way to check for vocal fremitus is to ask the patient to say a word which is a diphthong, ie has low tone vowels.
      In GERMAN, where the discovery was made, "Neunundneunzig" (ie German for ninety-nine was used) is a diphthong.
      The PROBLEM is the translation, as "ninety-nine" is NOT a diphthong, so technically gives inferior results for the examination, so "one-two", "balloon", or "toy boat" could be used in English
      This is one of my favourite misnomers in medicine
      - and YES I am that much of a geek!

    Thanks for this video and the time and effort that went in. But, when you examined the back, you palpated for chest expansion last and tested for vocal fremitus after auscultation for breath sounds. Perhaps after ausc, you might have instead auscultated for whispering pectoriloquy.

    I enjoy your videos for many reasons,but I have no idea of medicine so I would like to know what are you looking for in each exercise...for example when you look the fingers,what symptoms are you looking for,what answers do you expect...which connetion is there with the respiratory system...Thank you!

      I would guess it has to do with the blood vessels under the nails. Blood must replenish the nails quite quikly after you squeeze it out. But I am no doctor.

    While everything is important, I think new procedures have been put in place to advance the process of the respiratory exam. Perhaps, only using this method for those that exhibit respiratory distress in the non emergency setting.

    I know this is an older video (hehe i always go back to it for the epic unintentional asmr :)) )... but i have a question, Dr. Gill:
    So... i recently did a CT scan and, amongst other things (aka lymphoadenopaties - on my neck, armpit and groin) it said that i ALSO have those bad boys in my lungs (which makes sense with my shortness of breath).
    My question comes from a place of curiousity.

    Would there be any possibility that a non-invasive respiratory exam (just like this one you just performed) could detect lymphoadenopaties in a patient's lungs?

    How would you change the examination for a patient with a bra/breasts? Would you position your hands any differently? And would you need extra consent or to give more clarification of what you're doing before examining that area? Thank you for the video!

I could be hacking up blood and its like I might get a blood test done or something that's it. I also either don't get any results back or they are always fine, But I am not fine. I actually recently had surgery and was on medical leave for 23 days because I am perfectly healthy and fine. Saw 3 specialists 6 Dr's in total (2 of the Dr's I saw twice) nobody could tell me what I had, how to solve it, if it could be solved. What caused it. I was told I didn't need surgery, I did need surgery, it was optional if I had surgery. It wouldn't kill me, could kill me in long run. Last Dr said " I wouldn't want to live like that" or "I wouldn't want to live with that" Feb will be 2 years since the beginning of dealing with it all. Oh also I was in so many words told surgery may or may not resolve all my issues lol. I was told after surgery oh you "should" be ok now. SHOULD BE. I guess nothing in life is definite but damn. I am luckily a lot better but as of now its a waiting game to see if I will be good in the long run because I have 0 idea cause they either had 0 idea or didn't care to tell me and they certainly didn't try to figure anything out or do anything.

      Sorry for the rant. All this was needless to say that you and other Dr's online seem to care more and be much more thorough then any Dr I have EVER come in contact with.

    How do you do percussion so that you’re able to get a resonant sound? Are you tapping on the fingernail or DIP? When I practice on the lungs I’m unable to get a good sound

    Is it weird Im currently watching this as a respiratory inpatient in hospital?😂 Is it weirder that im wondering why I've never had this full exam (severe asthmatic) and only parts of it? Never had the neck thing but pretty much had everything else🤷‍♀️😂

      These examinations are for the gold standard, and the education of students.
      A little how you learn to drive one way, but often people will end up taking shortcuts as they become more confident driving.
      I know one doctor - Professor at UHCW - EVERY case is like watching him do formal examinations. He hits every note, every time.
      Interestingly enough, he’s respiratory too!
