By the time this game came out, Rikidozan had been dead for 37 years (murdered by the Yakuza). This entrance really shows what a great legend he is and how he's still regarded as such, ever after so many years. Epic!
He didn't die due to not listening to doctor's orders regarding the stabbing? I read that the wounds weren't serious and later on he ordered a ton of sushi and sake which was said to be disobeying doctor's orders (the latter I imagine would be detrimental at the time).

He was a glorified thug. Just like most gangsters. He should have never committed the shoot on Kimura. Thank god at least Inoki is a little better than that.
Amaru i don't know the exact story but it went something like this: The puroresu community organized a megafight with rikidozan vs masahiko kimura, probably greatest judoka ever. It was planned that there was gonna be a series of matches(i am not sure about this). So, the match was going as planned and they were performing a worked shoot which was extremely popular in japan at that time. However 7 or 8 min into the match, kimura kicked rikidozan in the groin which completely infuriated riki. He then decided to take the matter in his own hands and immediately unleashed a series of open hand slaps and vicious kicks to which the referee stopped the match and gave rikidozan the win. However, this angered the "yakuza" to such a degree that they decided to avenge kimura's humiliation. (And i have read somewhere that kimura had some influence in the yakuza). 9 years later some guy stabbed rikidozan in the back while he was partying and then he died a week later. This is the story as far i know. However, if there is any misinformation in it i would love to get corrected.
