I think there might be a few deaths you missed: Any deaths related to The Lizard King Hitting the bottom of the shaft with the circular platform If there are any others, I can't think of them.
i know that the hover joust deaths were spoofed in a cutaway in Family Guy. and if you do choose to take the drink in the "drink me" scene in your last life, the next time you play, the drink will be replaced with a loaf of bread with a sign saying "eat me".
On that note, how many of these deaths actually looked like they would kill Dirk? 1:50 for example. Hurt like hell, yes. Kill? Not likely, unless there's another deathtrap behind that wall.
I had an Amiga 2000 and had this game, could barely get past the first level. Kept pressing keys and moving the joy stick around and it never seemed to work.
