Steam quality can be an issue, Faciliy Engineers must notify the OR/CP when they are about to clean the steam line. Have seen this issue too many times. Imagine black black soot coming out of the steam and discoloring the chamber, wrappers and instruments and peep packs including the steam tape.
It's hard to say what it is without seeing pictures and being able to investigate. But if you are seeing black smudge, then I would work on trying to get to the root cause. Is it being found after cleaning? Is it being found after sterilization? Are there any chemistries in the department that are different than the usual cleaning chemistries we use in manual and automated cleaning practices? Are you noticing the residue in decontam after being used in the OR? These questions are just to help guide you in finding the root cause.

I've witnessed black specks inbedded in the tray liners before any process has taken place due to poor quality of liners....
