Did you think about adding your company name to the order form? I would input the shirt sizes; everyone does not know the lingo.

I have it on other forms that one is just for me to know what they want on their shirts.

I have another form for groups that they put their sizes and a place for info like their name or a # or any specific personalization they want on specific sized shirts etc.
