The patch that you bought online is called cool fever.. But you have to put it on the fridge before you use it so you can feel the coolness of it.. It usually use when you have fever.. :)
No, the coolness does not depend whether or not you put it on the fridge before use, you can actually use it as is and it will still be as effective. Since you’ve bought the japanese one, you can actually store it in room temperature, according to package instructions. Storing on the fridge only promotes extra coolness. FYI these items are displayed and placed in the racks and shelves of drugstores in japan, not in the fridge.
