    He’s not saying not to soak. He’s saying the mechanical position of using an on-toilet sitz can make the condition worse.

    I had a huge hem for a year, right in the middle, finally got serious about sitz baths three times a day. To my suprise a week later it was gone... i mean... gone gone. It fell off. 😅 One of the happiest days of my life.

    Doc! I’m a pharmacist and thank you for your insight! You are spot on and this is great information! I’m glad you described all of this material clearly and slowly because many elderly people will find this very helpful. I admit I talk way to fast and I have to remind myself to slow down. As you know, if they don’t comprehend the concept they will be lost and forget everything thereby wasting our time! Don’t listen to the jack wagons saying to speed up. It’s perfect for understanding this material.

      I hope if you are distributing ciprofloxacin to people that you are properly warning them of the potential side effects! And that alternative antibiotics should be considered if available! Its horrible!

    I'm so glad I came across this. It validated what I've been experiencing during my hemorrhoidectomy recovery. I'm in week 2 and am still sitting in the bathtub because I couldn't tolerate sitting in the toilet-mounted sitz bath for any amount of time.

      @@pitdawg6418 sending my best to you! I was soaking 15-30 mins after each BM and also anytime the pain was just not manageable with meds. Walking hurt so much but definitely do it even if you're in pain. Of course listen to your body - sometimes I started feeling better after 5-10 mins of walking, sometimes I felt worse and would stop.

    With all due respect, I have to disagree that these toilet sitz baths have no benefits. I had extremely bad external hemorrhoids last week and bought one of these from Amazon out of desperation because I can't fill my tub. Out of everything I tried, from Prep H, Lidocaine, and oral analgesics like Tylenol and Advil, sitting in one of these for 15 minutes reduced my pain the most and for the longest. My pain went from a 9/10 to a 4/10 for several hours afterwards, the sitz bath was worth it. Anatomically I understand what you're saying, and that's why I wouldn't recommend a prolonged session, but the amount of pain reduction just from using a toilet sitz bath should not be taken for granted.

      @Anthony_fu6cI - With all due respect, your personal experience aside, what the doctor is describing is the physical effects of those apparatuses and how they’re a net negative for the _majority_ of hemorrhoid situations.
      It’s nice you had a good experience, but in situations, as he described above, they’re doing more harm than good. The idea is to reduce the hemorrhoids, thus the pain will be alleviated. That device isn’t going to reduce anything physical. It’s the same with a massage… one can massage a sore area and make it feel good, but massaging the same area too long and it’ll make the muscle hurt worse.
      It’s wonderful it worked for you, but perhaps your situation wasn’t quite as severe as others. _(And that’s awesome!)_ However, as a physician, his understanding of how these things impact the overall situation will be more informed, and it’s best to leave it to the experts. Anecdotal evidence is helpful in some situations, but it’s still anecdotal evidence. 😉

      @@sisterspooky you may not have meant it that way, but your message comes off as very passive aggressive and patronizing. I already said I understood the physical/anatomical reasons why he doesn't recommend the bath. I watched the video and understood it very clearly. I was simply sharing my anecdotal evidence which relieved pain for me so others can also weigh in if it worked. No one has to take a random strangers evidence as fact, but if you read reviews for anything at all, then you probably do AT LEAST consider them. You'd also be surprised at how often doctor's can be wrong, that is why it always recommended to ask several doctors. Of course anything of a good thing isn't a good thing, I never recommended sitting on it for hours on end. Pain management and relief should not be underestimated. That is why hospitals have many sorts of distraction therapy and other non-pharmacological methods for pain management. There is evidence based (aka studies based off of many patient's ANECDOTAL stories gathered through surveys) that reports relief from different therapies. The reason sitz baths were even recommended in the first place was because of this evidence based proof that it works for pain relief.

      I agree with you. I was in a lot of pain. The kind of pain that could make a full grown man cry, yes! I almost cried of pain and the pain woke me up in the middle of the night.
      I have a feasure and I don't wish this on nobody. I am still dealing with it and it is a very slow healing process VERY SLOW. I purchased a Sitz bath and the very first try it seemed to have helped. Just don't go over 5 minutes because what the doctor here is saying will happen and is true.
      But I am willing to bet ANY money that when a person has a feasure and has tried preparation H, and any other cream but still has not helped, that person WILL be buying a Sitz bath real fast😂

    For my post hemorrhoid surgery I was recommended by my proctologist to do sitz bath with Epsom salts using warm water. The product I use is the first one you mentioned. For me, it really calmed out mild itching. My proctologist said a key factor to less post operative pain is cleanliness. I was uhmm pretty anal about my sitz bath routine after surgery. I can report zero post operative pain. None. I know this is absolutely not the norm. Feeling fortunate. I'll attribute my excellent recovery in part to the sitz bath routine. I'll look at your other videos regarding options for sitz bath.

    I had a plastic bowl that fit into my toilet seat. Sitz baths worked for me, giving me great relief. I had an allergic reaction to the hemorrhoid cream the doctor prescribed. An antihistamine stopped that.

    My home remedy is to relax in a hot bath with 2 handfuls of rough pure salt and 2 capfulls of antiseptic.

    Totally agree with this and ties in with my findings. Trying to get rid of an external hemorrhoid, using a recently purchased Sitz bath for 10 mins and my hemorrhoid had blown up. Not using this again. I wish I’d seen this sooner. Thank you for the video.

      I'm regretting for purchasing a sitz bath basin now. I'm on my 3rd day and it's not too much pain except for when i poo but i bought is as per my doctor advised me.

      @@cibiecibie I found it’s much better if you run a proper bath about 6-8”, mix in some Epsom salts, and then sit/relax in that for 20 mins or so. There’s not so much pressure as you get with the sitz bath (toilet mounted ones) and on the whole (pun intended 😁) it was much better.

      It's a miserable feeling very painful & the band will come off.

    I so agree! I’ve questioned the difference between sitting on a toilet and sitting in a sitz bath. Makes no sense to sit in a sitz bath for 10-15 minutes!

    If I’m honest, my mini sitz bath was a LIFESAVER when I had my anal fistula seton placement. I would frequently have the problem of my handheld bidet not getting everything off that area when I went to the bathroom, but the sitz would leave me completely clean. It’s really nice for when you’re too sore to bend down to get into a bathtub, which was my main problem after my seton placement. If it’s not good for hemorrhoids then it’s at least good for fistulas!

    Thats why after I clean things up in the nether area with witch hazel cotton pad, I fill up my bathtub with about 4 inches of water and add about a half cup of epsom salt and lower my whole body into a tepid temperatured bath. Thats how I do it and it has worked wonders for me. Nooo sitz baths for me. Thank you for sharing the info Doctor.

    Chung is spot on here. These contraptions are counterproductive. Someone who goes into intricate details can understand. These tubs exacerbate the hemorrhoids by letting them hang into without support. I’ve experienced the same & was always left thinking about their efficacy. I think a bigger solo tub in which one can sit, lay down would be a better bet.

    I have a rice filled heating pad called the bed buddy and I put it in the microwave for two minutes to warm it up. I then lay down and place it under me and across the painful area. It works amazing and stays warm easily for an hour. You could probably make something similar with a cloth bag (a sock maybe?) filled with rice.

      Hemorrhoids flare up / crisis?
      What you need:
      -daflon 1g tabs
      -macrogol 3350 laxative powder
      -water base lube
      ‐clean gloves
      Take daflon tab as directed by the doctor for hemorrhoid crisis dose. Take macrogol 3350 laxative as well 1-2x a day or more until you maintain a soft stool with no straining needed. Maintain the same macrogol frequency.
      Pooping: everytime you are about to poop, where a gloves and apply the lube on your anal area and you gloves prior pooping. Hold you hemorrhoids with your gloved hands from getting fully out of your anus to prevent worsening of the hemorrhoids and/or fissure.
      Do it everytime for 2 months without anal sex. Do regular mild jogging to promote healing.

    Very interesting. I switched to one of these from the bath as it seemed a) more convenient and b) largely recommended. At a certain point I just realized I MUST be doing it wrong because it seemed to be making things worse (especially since I felt better one night not doing anything and then worse in the morning after using it). The whole mechanics did seem odd because the hemmoroids are just sitting out there the same cause of the problem but I figured the water must be making things better somehow I just have it too hot or shouldn't use epsom salts or am not relaxing the right way? I think this verifies my experience and I will switch back to baths or just skip and use the shower when I don't have enough time. BUT why is this recommended in pretty much every how to fix hemmoroids article out there???

    My mom is 100 yrs old, doesn't have hemorrhoids. Tub isn't an option. There's a bidet attachment. A sitz is covered by Medicare. Peri cleanliness is paramount. Her parts are worn out, and approaching stool on the left is visible on her lower pelvic area. Depends are irritating and hold dampness. She has a weak bladder and uses the toilet 2x/hr; it's a huge issue. Even a washcloth is irritating after awhile. After seeing your explanations, I'll compromise and allow a bubbling Sitz bath from 1/2 - 3 minutes max (timed) and not every day. Maybe there is an inflatable she can use while horizontal in the bed, and I must research, as the aging process will require new strategies. I appreciate your soothing and clear explanations and plan to look at your other topics.

    Wont it be incredibly painful to enter and exit a typical tub?! I keep grimacing thinking about lifting one leg or squatting down to the water without intense pain post-op hemorrhoidectomy surgery! It seems these baths would be such an easier option that minimizes tension after an already painful BM.

    I've been using a sitz bath for about 2 weeks for a perianal abscess surgery I had and I can tell it's causing hemmhroids that I've otherwise never noticed so this makes sense. Solve one issue and cause another!

      Sitz baths can definitely help. But if you don't find these remedies to help enough, it may be time to see a specialist.

    Similar to a Sitz bath, I'm hearing from many who had their Hs vanish in a matter of 2-3 days by sitting on a heavy bucket filled with boiling water and minced garlic cloves. In fact, they found relief minutes after one usage when other doctor-recommended creams or ointments didn't really do much. I'm not saying you're wrong (I'm not a medical professional), but it does make me question the medical theory. Ie. Before gravity makes its negative impact, as you stated, could garlic + steam affect the H faster (by shrinking it) whereby counteracting those negative effects?

    Sitz bath was my miracle. Just don’t use them right after you have a bowel movement. That’s when they make the pain worst. The sitz bath is the best thing if you have a problem with swelling.

    Hi doctor,I had my surgery on friday and i couldn't pee as of today . I think urine retention is one of the effects of surgery but for so many days . My doctor hasn't had this experience with his patients before. What do you say about this? I am on cathedral at the moment until tuesday. i did several TOV while i was in hospital. The pain is excessive but i am controlling. Please suggest

    My issue w the toilet insert sitz tubs is that they INSERT INTO THE TOILET 😂 I can’t. I’ll clean my bathtub once a day before I get myself into a cycle of cleaning my toilet after every use in order to not get toilet rim germs on a plastic tub.

      I was like this too, I'm a huge germaphobe and I live with slobs who don't clean up after themselves. But the payoff in pain reduction after using it was worth it, so I just clean everything twice. No regrets!

    Surgeon recommended sitz bath with epsom salt to treat hemorrhoids prior to surgery. To make sure I understand, you recommend against the over the toilet sitz bath tubs, but you do recommend whole bathtub sitz baths (e.g. without involving a toilet)? While waiting for your next video, do you have any other recommendations?

      @@yourfriendlyproctologist everyone is not rich to own bathtubs and soak themselves in hot water. I don't own a bathtub - all i have is a toilet and this sitz bath tub. I sit there for five mins and it gives me relaxation post a bowel movement. I have to squat to expel out stool post the first bowel movement - i have like two streams cos i suffer from anismus.
      But sitz bath have always helped

    I don't have a bathtub, so would you recommend like a basin bucket sort of thing to sit on the shower floor?
    EDIT: I don't think this advice is that fully accurate. I think something is better than nothing, for example because I have no bath tub to lay in to do a regular sitz bath.: instead I tried for about 2 weeks of going to go lay down in bed and relax right after having a BM, instead of using the toilet cover sitz bath, and very quickly I started feeling more discomfort and bleeding increased.

    These are great for quick cleaning with water instead of wipes etc. Depends on the use. And who has time to have a sitz bath in the bath in between bathing or showering!?

    I listened to your logic here and I can't say I agree. While I don't suffer from an issue, my relative does and I am helping to find solutions. My logic is that while some of what you suggest here may be true, there appears to be more benefit from the application the warm water AND mineral than harm gravity might cause. For instance, having to spend more time on the toilet because your condition may also involve constipation does more harm to the hemorrhoids according to your logic. If the device has been proven to relax the area and calm the hemorrhoids, then you stand a better chance of having to spend less time sitting on the toilet when its pooping time, despite the con you've outlined. To me its a matter of weighing the pros and cons.

    Only warm water in sits bath help to relieve in hemorrhoids and fissures pain because doctor don’t give any meds to ease the pain , so how to do warm water remedy to ease pain ? Please let me know , is there any way I can talk to you

      I'm sorry to hear your doctor won't give you pain medication to help. I suggest using a water temperature that is comfortable for you. It does not need to be hot. I will be making a video on sitz baths soon, please stay tuned.

    Anyone know about right levator Ani muscles. They say to sit in this to loosen. Pain in rectum over year. Dr say pelvic floor dys, tight puborectalus, could I have pudendal nerve issues or rectal prolapse. Pain, swelling or feeling of rock or poop. Done pt for year, Botox in rectum, allograft in pelvic floor. One GI doc said have inflamed internal hem. He banded one and I was in severe pain all day. Last GI doc said no problem with internal hem. Going back to urologist who is pelvic floor specialist. What if all this suffering is from internal hemroids. Any thought or answers appreciated....God Bless

    Question. I was recommended using donut pillows for hemorrhoids. Watching this video now, it seems the pillows do the same thing the sitz tubs do, which is have you sitting on basically a little hole. What’s your opinion of these pillows?

      Yes, I agree with you completely! a flat supportive and soft cushion is far better in my opinion.

    I was told years ago never to use the rings to sit on either, they contricted the muscles in your hips and did not relieve the hemorrhoids
