This is going to sound really dumb, but: at over 40, I always told my friends “someone needs to teach me how to use a pillow”…

Dr Jon, you are a Godsend. I've been struggling with BPPV for years, recently severely. I tried Epley & with better success, half somersault, but never had complete recovery. Last night I switched my pillow position to lengthwise down my back. Game changer and answer to my literal prayers!!! Today was the first time waking without the room spinning. Thank you for your amazing help and dedication. Your videos make sense and are transformational!! You've manage to solve what my doctor and chiropractor could not. TY!!!!!

I started getting cricks in my neck when I was 18. Get off your pillow was the Chiropractors suggestion. This was 54 years ago. I still normally only use on a pillow to sit up with and when I cant breathe from a cold or allergy. It was the best suggestion I received. I wish I could go back and thank him.

Forget _Feng Shui,_ I am going to place my big mirror facing the side of my bed, so I can see myself in order to ensure I am properly aligning. The rolled towel isn't most comfy for me (touch-wise), but is best for me re: alignment. Of course I can always add a soft pillowcase to it. Fortunately in the meantime, I've discovered through T&E that using my great memory foam _travel_ pillow has actually been really helpful for me, seemingly working similarly!
Thank you much for this, Doc!

Thank you for the great advice, I so appreciate the doctors who come to UA-cam and give information for free. I’ve learned so much without being able to visit a physical therapist. I do have one question, I am a side sleeper with a neck hump and I exaggeratingly bring my chin to chest when I sleep. Very shrimp like. Fetal position. I also don’t sleep well on my back, and weirdly get nightmares every time I do? I’m also noticing that on my back with the pillow vertical and in alignment according to this video, I bring my showers up to my ears.
I’m wondering if over time, if I can sleep as long as I can stand it on my back with optimal pillow position, should I notice my body bringing my shoulders up less? I am also do neck and shoulder exercises daily, like IYTL and wall angels.
    Thank you

It’s been 4-5 days that I’m sleeping putting my pillow vertically. I wouldn’t know about my posture yet (which I have forward posture as everybody who stare at their phones and computers long time) but I sleep much matter this way, wake up feeling rested more and with less upper back pain. It is really amazing as if I’ve been doing it wrong all these years 😂. This way the pillow really supports my all body. I would always find hard to sleep on my back and turn around but like this feel that need much less.(Plus as I’m not squashing my half face up against the pillow that’s good for the skin too 😌). Thanks a lot, there is nothing like a good sleep 🙏🏼

I really appreciate this video because after sleeping on the towel I felt a huge relief of pain, and transitioning to the pillow was easier than just sleeping in it and it only thru the night. My issue with sleep has been the pain (which I was aware of) but I also thought it was a combination of the pillow and pain. Best night sleep I’ve had in years!! Thank you.

And I accidentally figured out that if my neck was in good alignment, I slept soundly - which feels like nothing short of a miracle. But, I hadn’t figured out the pillows issue - as for most of my life I slept with my head elevated to help with sinus issues. I was looking for something really thin but not really finding much. And in your videos I see many ideas to try - most of which I had never heard of or thought about. So, I really appreciate you pulling this together.

I also experience vertigo if my head lays completely flat and I look directly up at the ceiling. I have noticed that memory foam pillows are superior to regular pillows but now that mine are worn down they feel no different than a regular pillow and with me entering my 30s this year, I really want to start taking better care of my body. Hopefully I can choose a better pillow for my needs.

Only thing I'd comment on, whilst you don't recommend people sleep face-down, those with covid or similar respiratory issues do in hospital actually get moved into a prone position to help their breathing and recovery. Whilst it's not particularly comfortable, in some circumstances it's advantageous.

Mine is 11.5 inches high, is it too much, what is optimal. I had pillow on it, which I know now from your video I should not, last night I did remove it..and what an improvement. Thank you so much again, it is so hard to figure out sleep. All the best! Jana

I have cervical spine problems. I use a feather pillow and I typically squish it up and put it between my shoulders and my head, so it supports my neck. Regular pillows I do use for my knees. Typically I sleep on my back, sometimes when my back is sore, I will sleep on my side. That's only for a short time. Rheumatoid arthritis makes any position painful after awhile. Thank you for the info.

To avoid chin tuck, I used the contour pillow with a folded pillow sham or small towel on top. The extra fabric gives me something to push up under my cheek to prop up head in alignment with spine.
Occasionally I have such pain that I have to sleep face fown. Years ago I found a pillow that is actually like the wedge but it has a hole for your face and for airflow.

I have had so much trouble with pillows! For years and years... I have a really bad neck and need something that gives not just cervical support but a little pressure for curve correction... can't stand those cervical pillows as the rounded bit at the top slightly pushes head forward or pressures top of head. I love the roll though, where did you get that!!!
I ended up sourcing a pillow from the EU its called Sotvacmit Sleeping Neck Pillow... really amazing. Firm enough to provide some curve correction during sleeping, allows the base of the skull to tilt back so ears are in line with shoulders, good for both back and side sleeping, can use at any angle for different effects... highly recommend it. I have also used the Harley original contour neck support orthopaedic pillow for neck... (whew that was a mouthful), not available in Australia, but really good. The neck support bit is narrow so it provides support and curve correction of the cervical spine without pushing on the occipital area. Most neck pillows I find the roll is too broad making contact with the occipital area and ever so slightly pushing my head forward and I just can't tolerate that, they are made for people with healthy necks. Anyway, thank you for demonstrating these, really helpful.

I happened across this clip right before going to bed after my 3rd day of terrible neck tension and headache from bad sleep. I adjusted my pillow to try to align my neck the way you demonstrated, and I'm writing this after having about 80% less pain than the previous days. 🎉 Thank you. Dr. Saunders. Now I just have to work on the lower/middle back pain, as the pillow under/between knees didn't really work, but I'll take what relief I can get.
I'm sorry if you have already made one and I missed it, but I would love if you did a video for optimum types of pillows for different sleeping styles (Soft, extra firm, etc, Which work best for side/stomach/flip-flop sleepers? Also, what was the large pillow with the dip in the centre at 4:47? That looked helpful.). You touch on it a bit here. but I feel like there's much more to talk about. In all cases, I'm so grateful for a better night's sleep and thank you again. 🙏⭐😴

Oh my goodness, Dr. Saunders. All my favorite positions are bad (stomach and side with head tucked in fetal) and my least favorites are the best; I feel exposed on my back. I ordered a temper-pedic pillow like yours at 5:30 because I feel like I need to still have a pillow-shaped pillow, I'll just turn it as I turn during the night. The tubular pillow also looked nice but I think it'd feel like my head is having to balance to stay on. My self-dx'd cervical radiculopathy seems to be healing little by little each day but then I test it out with jumping jacks or jogging and then it hurts again. Hopefully the pillow will help it heal for good quickly! Thank you for your help! Subscribed! my sleeping position, after a LOT of trial and error is round pillow (which I call bolster) on my neck.
My torso and bed is making about 30 degree angle.
Front body is supported by a really long bolster in which it covers below your neck, and you put it between your leg.
Since i have wider upper torso, I add another pillow under my armpit.
There you go... no stress on my spine at all.
oh... other arm is under the head bolster.
head almost face down between head bolster and torso

I don’t know about the wedge pillow at the end. I’ve never used one. But everything he says I can back up because the pinched nerve in my neck reports to me immediately when I do something wrong - when I get out of line - when I do the fetal position neck tuck when I am side sleeping. . This guy knows what he’s talking about.

From what I u veterans from this info, I have 3 pillow options. After Spinal Neck Surgery 1 1/2 yrs. ago, I am in chronic brutal pain. I had the surgery because I had Cervical Stenosis. Did NOT rush into surgery. Nothing was prescribed except the surgery. C 3,4,5 were involved and a fusion there with a cage around put in. Was in a hard collar for 3 months, soft collar for sleeping only, last month. When hard collar came off, I asked if I needed P.T. I was told no, just given a booklet of home exercises and stretches. Pain never left, went back 2 months later, was sent to a Spinal Pain Dr. 2 mos. of P.T. did nothing. Neck was also very stiff w/ next to no ability to turn. Then sent to Pain Clinic that treated w/ procedures. Had Trigger point shots (8) nothing a pain fusion relief only for the day, 2nd pain fusion, same, 2 nerve blocks, then a nerve burn. All this took 1 1/2 yrs. because appts. could not be gotten sooner. Sometimes had to wait 2,3 mos! Had the nerve burn 9 days ago now. On day 7 after nerve burn, I was rear-ended hard, while stopped at a red light. I only get about 3 hrs. of sleep per night. Little more if I take muscle relaxer. The pain does NOT allow me to sleep and I can barely function during day. My life changed drastically and miserably since this surgery. I do, sleep fetal position, head curved down. I try hard not to, but toss and turn and wake up that way. I sleep very light. I am 73.yrs. old, female, have Fibromyalgia ( made worse since surgery) have 3 other auto-immune illnesses one being SLE, type 1 diabetes for 63+ yrs. Should I use my pillow length wise, the cylinder pillow or ??. Thank you, sorry it took so long to explain, wanted to be thorough.

I think the common advice is that stomach sleeping is bad for you, so unfortunately no one seems to want to give advice for it. I've been a stomach sleeper almost all my life. It's the only way I can actually comfortably fall asleep, despite how much I've tried over the years. Side sleeping kills my hips after awhile, and back sleeping doesn't work for various reasons. But the minute I swap to my stomach, I feel my hips release and it's the best feeling. I only wish there was a way to do it without turning my head to the side!

Thanks a lot, @drjonsaunders! I am struggling with stiff neck & shoulders and found these videos super helpful & well explained. I have 2 questions:
    1. In pillows, we also have various fillings like fiber, memory foam, latex foam, buckwheat, etc. How to select that?
    2. The only pillow that could work for someone who switches in night between back sleep to side sleep, would be the rolled one, right? As the thin normal pillow placed below scapula, would have to be adjusted to below neck when swapping to side, which may not be always possible during deep sleep. Right?

    • @drjonsaunders
      @drjonsaunders  8 місяців тому +1

      1. Choose based on your comfort level, 2. The rolled pillow is good as you suggested, but any pillow can work as long as you get in the habit of correcting your position when you wake during night.
      Best wishes!

I've slept reclined unfortunately due to vertigo issues. The problem with sleeping like this (using a wedge) can cause your head to droop to one side or the other causing some awful pain and stress on your neck muscles. You need some kind of support to keep your head from rotating left or right. I've used a C shaped cervical pillow but I think it's causing problems with my neck. I seem to have best luck with a cervical buckwheat pillow that looks like the roll. It's quite hard though and I'm unable to sleep on the side with it. Thank you for sharing this as I think I need a new pillow that fits me. better than the larger one I have.

I am from Austria and I find it very interesting, that nor any therapist, nor any physician I ever met in my 50 decades was interested in any questions about bed, pillows, sleeping positions or giving any advice therefore. The only answers I got, was "I don´t know, but go to the shop and try it out, where you are laying comfortable".
It is rather shocking, what lack of competence there is. In the shops by the way, there is mostly sales stuff, which has run through obvious strange courses, where they "learn" to "see" when a customer is laying good on a mattress or pillow - I don´t know how that is possible, but they can see from 3 meters and when you are wearing your street clothes, if your spine is in the correct position. In most cases though they always recommend to me a mattress that is too hard and is bringing my spine out of it´s natural curve.
Softer mattresses though have often the problem, not to support the spine, where it should be, as they are soft all over.
The 7 zones that are used to sell the mattresses more expensive, are obvious made after a wired pattern for alien spines, but the sales stuff is telling on and on, that this zones would fit to nearly everybody on this planet, doesn´t matter if they are 1,60 or 2 meters tall, so this bed thing is really a mystical science it seems....

So I stay in bed to avoid falling down when I pass out. I do not get nauseous. I tried the Eppley Maneuver but it did not do anything. I have had this for, it will be, 2 years in February. MRI showed I have Cervical Spinal Stenosis, bone spurs that are starting to press on my spinal chord. But the Neurosurgeon said is not the cause of my dizziness.
