This may not be totally relevant but I'll post here anyway, and it's all straight from my head, and not totally sorted out, but it is something I have thought about.
Asking Stupid Safety questions in the workplace.
      -----------------------------------------------------------------------------
      - There is no such thing as a Stupid Question
      - The only Stupid Question is the one you didn't ask.
      - The person you are asking is responsible for your safety.
      - If you don't get answers to your satisfaction... then get out!
      - If you don't get ananswer you understand, ask again, till you understand.
      - If the questions get you sacked... Then there is a safety problem.
      - Don't do anything you are uncomfortable with, ask!
      - If you don't understand, Ask!
      - If you see a potential safety problem... fix it or point it out, if you don't and something happens, it is your fault,

This is the REALITY when one is involved in a BUSINESS and not an ENGINEERING one. Businesses prioritize PROFIT above everything else including the professions in medicine, law, accountancy, architecture, food science, human resources, and yes, engineering. The basic rule is, "Toe the line or else, find another place of employment".
It takes very brave and altruistic entrepreneurial people at the very top of a business to submerge the profit motive in place of engineering excellence.

There is some truth to the claim that "you have to break eggs to make an omelette", or in other words, in order to innovate you have to break with tradition. But that should not mean not considering the consequences, especially the consequences of failure. If they are serious, as when involving people's lives or health, then you have to proceed more carefully. Take smaller steps - it's called iterative or incremental development and is well established. But as long as we live in a culture that panders to winners, no matter how they got there, someone will always be in a hurry, consequences be damned.
