My headcanon: maybe when Orange said "he is not the only one who will suffer", he actually meant.....himself! After his Apple died, he actually fall in misery and sadness for losing a "best friend", guilting himself for being "too annoying to him", which is actualky the reason why, in Orange's opinion, Apple's life was so bad. We can even combine this with his line about "was it all worth pretending?" from that FNF "Rotten Smoothie" mod-it can mean that even after his friend's death, Orange tried to hide his feelings, by literally "pretending" that everything is okay, and he is happy.....while in reality he is not
