I would say the issue of wearing a helmet for over a year, selling a helmet that can kill you, and the fact that 90 percent of the people in the game didn't die in the first week when they starved because nobody found them in real life and because someone pulled the helmet off in the first day. Also let's not forget power outages, internet issues, and the fact that moving these people to a hospital would be insanely difficult to do. All of this on top of literally everything about the game and how it makes no sense.

It's a small thing but not something I noticed on my first watch (or through like my 30th rewatch if I'm being honest haha) and I just thought that interesting. Hundreds of rewatches later you could notice the smallest thing that never stood out before.
