    Hello, Product Designer here.
Good idea I agree but I've seen it in use and it behaves like a teeter totter. I had the pleasure of seeing it in action at Timberline Lodge during the mountain bike season. Several times the wheel would get stopped short and the injured person would be thrust forward. You know that feeling you experience on the upside of the teeter totter?
Two people held the sides from tipping, and one at each end to prevent it from teeter tottering. All out of synch and catching the Terra Tamer from spilling the injured person constantly creating a kind of erratic roly poly (doll that tips and centers back up again.)
If I may, I'd like to offer a better solution.
The solution would be, I think, to have the wheel all the way at one end. So that the leverage is the whole stretcher.
Let me explain the concept. With a long and strong enough lever resting on a fulcrum (the center of gravity, in this case that would be the wheel), a single person can lift a whole house.
Look into the Law of the Lever by Archimedes.
At one end mountain biking handle bars (to keep from tilting side to side) and at the farthest end the wheel (to hold more weight, the wheel could be 1/4 of the way from the end. (the golden ration)
Also maybe the handle bars can actually turn the wheel to guide the stretcher through the trail to prevent back and forth motions when choosing a line over terrain that doesnt have a trail.
