Physically and mentally. This may even point to little nightmares 1 being a sequel. Six has been INSIDE the tower, accelerating the decay of her mind much faster than what is normal. She is far more aggressive than any of the children her age, and I think that could be directly because of how much the signal damaged her brain. The tower may cause feelings of revenge to fester as well. After mono gets trapped there for, presumably, many years, his only goal in life seems to be stopping six. She is the one that the thin man targets, and she is the one he directly harms. When we look at the end of the first game, six basically goes on a power trip, killing those who wanted to cause her harm without hesitation. The tower's power may be setting in even before they go inside. When six warms her hands on the body of the doctor, she feels no sympathy to those who hurt her. Breaking the fingers of the prosthetic, and murdering the porcelain child (who, once again, tried to kill her), are some of our first hints that six is loosing control over her emotional reasoning and anger. Just some rambling, but I think this makes sense.
This is an incredible theory, but sadly Six doesn't have an eating disorder like the guests, it's actually just a curse from the signal tower, which we find out in the secret ending which envolves a glitched remain of six appearing in front of her, and her stomach starting to rumble (the glitched remain also appears every time she eats)

It’s possible because the signal tower warps and distorts there forms and the tower also has the power to bend space and time wich is why their younger self can live at the same time as there older selves.. and it also makes sense why six took over the maw because she needed a vessel to constantly feed her hunger wich is why she assumes the role of the next lady even taking her future selfs powers.

@@s.k.a.tmania6201 The kids from the comics are the glitched children you find scattered around for example the little girl who was digging the hole to escape is the glitched remains we see hovering over the hole with the rats in it. I'm not sure where you find the other glitched children though need to rewatch and reread the comics.
It was so creepy and difficult that I wasn't even able to play it through. I asked a friend to play that part for me, more specifically the part with moving mannequins in the dark corridors. We finished playing the game together and I was actually the one who dealt with the last boss fight, but those mannequins were just too much. Also, in the first game, the lady singing while she was brushing her hair sounded so creepy that I had to turn the sound of the game off. Those three notes she sings, they terrify me and I don't even know why.
I would still like to know why The Doctor crawls on the ceiling and never once touches the ground. Is it some sort of ability brought about by the corruption that allows him to attach himself to any surface? I imagine that he would prefer it to standing on the ground because he could examine his patients more thoroughly with a bird's eye view. The only times he ever crawls normally is when he is crushing those piles of corpses onto you and when he crawls inside the incinerator. A theory I have about that patient on life support is that it was The Doctor's breakthrough. Something that was as close to his vision of a perfect human that he had ever created and was kept alive so that he could further modify it at a later date. However, when we shut off it's life support and The Doctor fails to save it, his breakthrough has been ruined and he is extremely angry, hence why he was so blindly persistent in chasing Mono all the way into an incinerator.
In the artwork it shows the hospital upside down and some of the staircases that we see in the signal tower. In my timeline video I speculated that the tower was warping time and space, so he’s essentially crawling on the floor.
Maybe in the past before he got corrupted by the tower he was like full of himself and he always put himself on top aside from the others and maybe he likes to look down on people like he is the best doctor in the world. So that is what the transmission uses to make him a fat monster and can crawl up to the ceiling because it's like a metaphor to his personality in the past. Or maybe the tower uses his fear on the floor or getting dirt from the floor.
Maybe its because theres holes in the ceiling placed specifically, there are some levels in the game where you have to crawl on the celing and there was a bunch of holes.
Side note: Thank you for the content warning, genuinely cool of you. Idk how normalized it is to do this yet, but it makes me happy when I see it in videos where it wasn't already clear that it tampers with subjects very able to provoke some trauma.
I think at the time of the game, the Signal Tower has been affecting people for years. Therefore the citizens are almost completely gone, explaining why they just mindlessly fall off of the roofs of buildings. They are trying to reach the transmissions. Before the game however, I think the citizens did still have some form of sentience or humanity to them. That's when they went to the doctor for help. And he may have been fixing them, but eventually the tower corrupted him too. This means that the mannequins have been there for a long time. Sad. I really hoped Mono and Six would be able to bring a sense of normality to this world again by the time the credits roll.

Little nightmares imo has 2 major themes. 1) Growing up (being "corrupted" by the world/losing your innocence 2) The Negative Effects Media has on our lives. Protagonists, Antagonists, aswell as the world in general represent different aspects and/or consequences of this. A few examples: The Granny representing how old people end up in loneliness and are basically ignored and forgotten by society, in case of the granny specifically there is probably also a theme of women being less valued in old age for losing their beauty. Given the theory that the granny was the governness of the maw in the past that would also explain 1 of multiple factors contributing to the Ladys obsession with beauty and hiding her aging face. Of course adding to that the the obsession of the LAdy can also represent the general pressure on women through our increased exposure to media. The power to alter our fotos or select only the best has put insane pressure on all people (but more so women) to look their best and get the feeling that with fading looks their worth fades (personal note: men defenitely suffer alot under such thigns aswell and that is in my opinion a horribly overlooked thing but it is udnerstandable that developers would focus on something that is more clear to society) Some more example that I have the best fits for so far. The pretender: Two options here 1) The pretender is actually a child, she represents the naive view of most children that becoming an adult is something to strive for something sophisticated that is in all ways better than being a stupid kid. (Tea Parties are a classic thing of children acting like adults) 2) The pretender is not a child but a small woman that is so pained by the truth of adulthood that she hid herself from the world and now tries to "go back" to that happy innocence of being a kid. The School as a whole: I think the school has 2 pretty clear themes 1) th often discussed theme of modern education breaking children and turning them into little perfect well behaved things. 2) The hidden little monsters such early pressure to perform creates and at the same time the willingness of educators to look away from these "creations" of the system. Hospital and Doctor: I quite like the theory of the fotos showing how the signal basically damaged peoples ability to contain themselves in multiple ways and I would like to add to it. Again the signal is the effect of modern society especially modern media and our exposure to it and thus the pressure that comes with that. we see this represented in all kinds of different forms some of whcih I already talked about others I will in this comment. So the patients could have come to the doctor to have the doctor fix their need for MORE by doing somethign with their brains. Or maybe thy could even want him to damage them because media showed them that people who are simple, who do not care, who simply consume are the HAPPY ones. However what I find also to be a quite fitting interpretation is that the doctor is indeed specialized in (plastic) surgery and that people came to him to obtain the standards that they perceived they have to meet through watching what the signal sends.' However we know that the doctor was driven mad because he just could not achieve what the patients wanted, this could be a representation how no matter how perfect we make ourselves out to be it is NEVER ENOUGH. WE ARE NEVER ENOUGH. The Guests and the General people: The Guests represent multiple things of modern society in my eyes: Overindulgence and Materialism, and competitiveness and replacability. The guests very clearly show how they represent overindulgence and materialism, but they also show the brutality of adulthood by consuming those who are weaker (the children) and at the same time that no matter how "strong" and succesfull you are you can be gobbled up by society at any moment (the chefs killing the sleeping guests) The people represent among possible other things 2 things clearly for me. 1) The increase in depression and suicide through the constant confrontation with the SUPPOSED perfection of the world and others and by that the perceived INADEQUACY of ourselves that we cannot fix how ever much we try. Ultimately leading to only one way out of the sadness..... 2) The distorted faces: Faces are what is easily most unique (on the outside) to us as humans. We recognize each other and differences between us the easiest in our faces. The signal/modern society(the media) puts us all under the same brutal unattainable set of standards. In trying to reach this one supposed goal we all become the same we all want the perfect, faces, bodies, money, career, cars, status etc we become one faceless mass. The Hunter: MY interpretation of the Hunter is still pretty weak but one Idea I have is the representation of people who have given up but try to disengage instead of ending it. People who try to create a world liek th "good olden days" were everythign was nice and happy. But at the same time the hunter capturing kids and sellign them could represent the impossiility to escape modern society how we are forced , in some way or another to participate in this modern horror in order to survive. The Janitor: He feels similar to the Hunter though slightly different in some aspects. His hobby of creating nice little dolls to me feels like a wish for the return to child like innocence and also still trying to keep up the idea from childhood that adulthood and society are something nice and shiny. His (voluntary) blindness also ties in with that representing how we do not want to see the real horror of adult life and to a degree also not the horros that we ourselves commit (what he does with the children) Six and Mono: I think Six and Mono as one big thing clearly represent the corruption of growing up. Though in different ways. Mono hopeful, helping and trusting becomes a bitter man that has learned that noone can be trusted and has thus become passive and simply compliant in what is going on. Just sitting there passing time. Six on the other hand represents the other half the half that are turned into monsters themselves, using others before they themselves can be used (before anyone asks yes I see Six cleary as a "bad" selfish) person) Six continued: I think when people talk about sixxes hunger they think to often of literal hunger for food. However I think it represents something more general that I kidn of already talked about. The general idea we get corrupted with while growing up tht it is NEVER ENOUGH. We always need more and because the standard so high/will always go higher once we obtain it the hunger will never go away only be temporarily alleviated. Lastly the SIGNAL: As much as I liekd the TV idea and the unknown watchers. I think it is different. The people in the world do not only receive the signal and are influenced by it they(we) ARE the signal. We all are being watched and the watchers at the same time. The judges and the judged. they eyes whetehr on doors or in flesh are not some old god types of entities they are US. Meaning how we all constantly watch others in order to know what the goal is, in order to stay "with it", but also we watch to judge, to make ourselves feel better not by rising up but by pulling others down. And at the same time how we are ALWAYS watched nowadays no matter where we are and what we do. The signal is not a real signal nor some weird creature or deity or whatever. IT'S US!
Exactly! They often left tvs and radios on around mental health facilities. And maybe their changes were ignored as part of their psychosis....ugh. So many things to think about
Maybe all people are just affected by the television in different ways... Like, some of the residents if pale city are getting bored and, out if their boredom, they're finding imperfections in themselves. Others are just starting to spend their whole time infront of the television, while some are getting tired if their pale lives and commit sui¢ide...

Its becuase he believes the world hates him. Little nightmares is about suffering from issues such as trauma, obsession, disorders etc and how those issues often result in damaging behaviors that either make them worse or perpetuate said issues. Example: victims of trauma who cope poorly often inflict said trauma upon others. This is why mono becomes the thin man after he is left alone. He had defeated his personal nightmare becuase of his friend. But then he was abandoned by said friend and so succumbed to it, becoming the thin man. He then goes on perpetuating the cycle.
12:27 the ghost kid covers his face just like mono because mono dosent want people to know his identity when he grows up, i think the ghost kid is doing the same. we know the ghost kid is missing his right arm but if you look closely the hunter is also missing his right arm, if you look the hunter has a fake right arm he put on himself. i think ghost kid is covering his identity because he dosent want people to know he will end up as the hunter (just like mono covers his face so people dont know he is the thin man)
i personally think that one idea is that six is the sixth iteration of a line of girls who become the lady and mono is a constant, always there always the same person who becomes the thin man as he is and always will be him. this can be possible with the porcelain lady doll you can find in chapter 4 for the unladylike trophy with the description 'hello, have we met?', which i believe to be her asking mono as every lady to be has met him and made him the thin man to repeat the process
Also I have a theory that Six had been in the hospital before. She seemed to know where she was going and took the lead way more during that section of the game. I think that when you return to her and see her breaking the fingers of that mannequin it’s because she already knows about the hands and is immobilising it in case of it coming to life. Not to mention she seemed to enjoy watching the doctor burn as if she had some sort of history/vendetta with him

Like, he just chases after Mono, I understand that its possible just for the sake of game play he would only focus on Mono, but when its this game that has in world explanation for literally everything even if we haven't figured them out yet, its a little weird she gets ignored
It’s likely the Doctor has tunnel vision on Mono as might be enraged that Mono took off his Patients life support, either that or he thought because Mono was trapped in the furnace that he was easier to go after.
No, the long arms and the short legs was an effect of the distortion of body parts monsters of the Little Nightmares, caused by the frequencies of the signal tower. Monster Six is an example of how "Roger"/ The Janitor gets his main iconic features.

So the time spent inside the signal tower might have corrupted Six's brain, which would explain her hunger problem. Or even why she is so aggressive towards Mono after he destroys her music box.
My theory is that mono was in a loop and then, after six let him go she went back to the maw to find out more about that place then kills the lady and when she grows up, years later, she becomes the woman living in the maw and staying away from the main land away from mono or the tall man
Good theory but if she is the lady why does she go up the light and at the end we hear horns of a boat. This shows that someone has left obviously six.
Roger was likely trying to escape the Pale City, because he didn't want to be absorbed by the Eye through the tvs. That may be why he was blind, simply trying to tear out his own eyes in order to avoid becoming a Viewer. This could also be why he was so infatuated with the tv when you turn it on--he simply cant help himself, there's still part of him that is attracted to the TV's like all the viewers. We know that Roger went to the Maw to escape some greater threat, and I think he could have been trying to escape the corruption and mutation of the Eye. That clearly didn't work however, seeing as he's just as monstrous as the rest of them. Also, does anyone else wish they put more of the Viewers with cords attached to TVs or TVs for heads like they put in the sketches? I think it would be so cool to see a Viewer melded with a broken TV that tries to attack you but can't due to being anchored down. Or when you hop through a TV it turns out to be one that a Viewer is attached to, so you have to run away quickly.
2:06 - I wonder if perhaps the missing limbs is meant to be a twisted reference to that old phrase "costing an arm and a leg" when one buys something extremely expensive? That, if going with the plastic surgery idea, perhaps those people were so desperate for that physical perfection, that they were willing to LITERALLY pay an arm and a leg for the doctor's services.

Had no idea they existed until I finished the game and started watching playthroughs of it

Something you might find interesting is Nikko has managed to break the game to play it in first person, meaning you can get some close-ups of the theories you are making here. For example, they go in super close to the boy on the chess piece in the school, and even all around him so you can see whether he is a real, or porcelain boy. hope this helps with your theories!

i would honestly love to see a psychologist's point of view of little nightmares. i would sit through a three hour video if it meant i could watch you deduce the neuroscience parts from a psychological point of view. i want to be a criminal psychologist/criminal profiler when i leave school if i dont manage my teaching degree (life long dream, very fortunate that my family can afford it), so this is all very interesting to me!!

It could be Roger In the first game, we chop his arms off. He would then go to the doctor to get prosthetic arms but in the process, was put on life support. This explains why the doctor has his mask
I think that little thing the patient on life support has on its head is actually a blindfold, which I also think that points to that patient being Roger (AKA the Janitor)

I really love how you point out things no one else seems to even slightly think of. I did notice the ghost-sheet child actually. But I have a question: Do you think the creature Mono seems to be walking on before getting to the chair in the end could be a giant brain-thing? Brain is what takes in all the eyes see - even things we don't look at mindfully - and twists it into nightmares. That's the connection I made when I saw it for the first time.
12:42 in my opinion, that tentacle thing in the bathtub could be either a gigantic stomach or a really huge bowel movement 😂. I mean it has flies surrounding it, and if there’s one thing we know about flies, it’s that they’re attracted to decaying or smelly rotten things. 😖
The doctor hangs on the roof so that maybe its harder to see if he's coming for the patients and its like a "SURPRISE youre next to be fixed" and/or is using it to see the patients mire clearly from a birds eye view
I actually thought that the dangling thing on the life support person is just a piece of the bandage on its face, that’s why it looks like a something a bit thick then becomes thinner, it’s the bandage swaying behind their head while the doctor lifts them up

To my completely untrained eyes the brain damage looked like it could've been from a trans-orbital lobotomy (basically stabbing someone in the brain through their eye), which if I remember correctly were used to "treat" the mentally ill because it effected their sense of self and generally made them docile
Here's a bigger nightmare: The medical world could never have found out what part of the brain controlled which part of the body from a dead person. The person would have to have been alive.

This is an incredible theory, but sadly Six doesn't have an eating disorder like the guests, it's actually just a curse from the signal tower, which we find out in the secret ending which envolves a glitched remain of six appearing in front of her, and her stomach starting to rumble (the glitched remain also appears every time she eats)

It’s possible because the signal tower warps and distorts there forms and the tower also has the power to bend space and time wich is why their younger self can live at the same time as there older selves.. and it also makes sense why six took over the maw because she needed a vessel to constantly feed her hunger wich is why she assumes the role of the next lady even taking her future selfs powers.

The hospital was the creepy one for sure
It was so creepy and difficult that I wasn't even able to play it through. I asked a friend to play that part for me, more specifically the part with moving mannequins in the dark corridors. We finished playing the game together and I was actually the one who dealt with the last boss fight, but those mannequins were just too much. Also, in the first game, the lady singing while she was brushing her hair sounded so creepy that I had to turn the sound of the game off. Those three notes she sings, they terrify me and I don't even know why.

I would still like to know why The Doctor crawls on the ceiling and never once touches the ground. Is it some sort of ability brought about by the corruption that allows him to attach himself to any surface? I imagine that he would prefer it to standing on the ground because he could examine his patients more thoroughly with a bird's eye view. The only times he ever crawls normally is when he is crushing those piles of corpses onto you and when he crawls inside the incinerator.
A theory I have about that patient on life support is that it was The Doctor's breakthrough. Something that was as close to his vision of a perfect human that he had ever created and was kept alive so that he could further modify it at a later date. However, when we shut off it's life support and The Doctor fails to save it, his breakthrough has been ruined and he is extremely angry, hence why he was so blindly persistent in chasing Mono all the way into an incinerator.
In the artwork it shows the hospital upside down and some of the staircases that we see in the signal tower.
In my timeline video I speculated that the tower was warping time and space, so he’s essentially crawling on the floor.
Maybe in the past before he got corrupted by the tower he was like full of himself and he always put himself on top aside from the others and maybe he likes to look down on people like he is the best doctor in the world. So that is what the transmission uses to make him a fat monster and can crawl up to the ceiling because it's like a metaphor to his personality in the past.
Or maybe the tower uses his fear on the floor or getting dirt from the floor.
Maybe its because theres holes in the ceiling placed specifically, there are some levels in the game where you have to crawl on the celing and there was a bunch of holes.
He must have a connection with: a Caterpillar

I think at the time of the game, the Signal Tower has been affecting people for years. Therefore the citizens are almost completely gone, explaining why they just mindlessly fall off of the roofs of buildings. They are trying to reach the transmissions. Before the game however, I think the citizens did still have some form of sentience or humanity to them. That's when they went to the doctor for help. And he may have been fixing them, but eventually the tower corrupted him too. This means that the mannequins have been there for a long time. Sad. I really hoped Mono and Six would be able to bring a sense of normality to this world again by the time the credits roll.

Little nightmares imo has 2 major themes.
1) Growing up (being "corrupted" by the world/losing your innocence
2) The Negative Effects Media has on our lives.
Protagonists, Antagonists, aswell as the world in general represent different aspects and/or consequences of this.
A few examples: The Granny representing how old people end up in loneliness and are basically ignored and forgotten by society, in case of the granny specifically there is probably also a theme of women being less valued in old age for losing their beauty. Given the theory that the granny was the governness of the maw in the past that would also explain 1 of multiple factors contributing to the Ladys obsession with beauty and hiding her aging face.
Of course adding to that the the obsession of the LAdy can also represent the general pressure on women through our increased exposure to media. The power to alter our fotos or select only the best has put insane pressure on all people (but more so women) to look their best and get the feeling that with fading looks their worth fades (personal note: men defenitely suffer alot under such thigns aswell and that is in my opinion a horribly overlooked thing but it is udnerstandable that developers would focus on something that is more clear to society)
Some more example that I have the best fits for so far.
The pretender: Two options here 1) The pretender is actually a child, she represents the naive view of most children that becoming an adult is something to strive for something sophisticated that is in all ways better than being a stupid kid. (Tea Parties are a classic thing of children acting like adults)
2) The pretender is not a child but a small woman that is so pained by the truth of adulthood that she hid herself from the world and now tries to "go back" to that happy innocence of being a kid.
The School as a whole: I think the school has 2 pretty clear themes 1) th often discussed theme of modern education breaking children and turning them into little perfect well behaved things. 2) The hidden little monsters such early pressure to perform creates and at the same time the willingness of educators to look away from these "creations" of the system.
Hospital and Doctor: I quite like the theory of the fotos showing how the signal basically damaged peoples ability to contain themselves in multiple ways and I would like to add to it. Again the signal is the effect of modern society especially modern media and our exposure to it and thus the pressure that comes with that. we see this represented in all kinds of different forms some of whcih I already talked about others I will in this comment.
So the patients could have come to the doctor to have the doctor fix their need for MORE by doing somethign with their brains. Or maybe thy could even want him to damage them because media showed them that people who are simple, who do not care, who simply consume are the HAPPY ones.
However what I find also to be a quite fitting interpretation is that the doctor is indeed specialized in (plastic) surgery and that people came to him to obtain the standards that they perceived they have to meet through watching what the signal sends.'
However we know that the doctor was driven mad because he just could not achieve what the patients wanted, this could be a representation how no matter how perfect we make ourselves out to be it is NEVER ENOUGH. WE ARE NEVER ENOUGH.
The Guests and the General people:
The Guests represent multiple things of modern society in my eyes: Overindulgence and Materialism, and competitiveness and replacability.
The guests very clearly show how they represent overindulgence and materialism, but they also show the brutality of adulthood by consuming those who are weaker (the children) and at the same time that no matter how "strong" and succesfull you are you can be gobbled up by society at any moment (the chefs killing the sleeping guests)
The people represent among possible other things 2 things clearly for me.
1) The increase in depression and suicide through the constant confrontation with the SUPPOSED perfection of the world and others and by that the perceived INADEQUACY of ourselves that we cannot fix how ever much we try. Ultimately leading to only one way out of the sadness.....
2) The distorted faces: Faces are what is easily most unique (on the outside) to us as humans. We recognize each other and differences between us the easiest in our faces. The signal/modern society(the media) puts us all under the same brutal unattainable set of standards. In trying to reach this one supposed goal we all become the same we all want the perfect, faces, bodies, money, career, cars, status etc we become one faceless mass.
The Hunter: MY interpretation of the Hunter is still pretty weak but one Idea I have is the representation of people who have given up but try to disengage instead of ending it. People who try to create a world liek th "good olden days" were everythign was nice and happy. But at the same time the hunter capturing kids and sellign them could represent the impossiility to escape modern society how we are forced , in some way or another to participate in this modern horror in order to survive.
The Janitor: He feels similar to the Hunter though slightly different in some aspects. His hobby of creating nice little dolls to me feels like a wish for the return to child like innocence and also still trying to keep up the idea from childhood that adulthood and society are something nice and shiny. His (voluntary) blindness also ties in with that representing how we do not want to see the real horror of adult life and to a degree also not the horros that we ourselves commit (what he does with the children)
Six and Mono: I think Six and Mono as one big thing clearly represent the corruption of growing up. Though in different ways. Mono hopeful, helping and trusting becomes a bitter man that has learned that noone can be trusted and has thus become passive and simply compliant in what is going on. Just sitting there passing time.
Six on the other hand represents the other half the half that are turned into monsters themselves, using others before they themselves can be used (before anyone asks yes I see Six cleary as a "bad" selfish) person)
Six continued: I think when people talk about sixxes hunger they think to often of literal hunger for food. However I think it represents something more general that I kidn of already talked about. The general idea we get corrupted with while growing up tht it is NEVER ENOUGH. We always need more and because the standard so high/will always go higher once we obtain it the hunger will never go away only be temporarily alleviated.
Lastly the SIGNAL: As much as I liekd the TV idea and the unknown watchers. I think it is different.
The people in the world do not only receive the signal and are influenced by it they(we) ARE the signal.
We all are being watched and the watchers at the same time. The judges and the judged.
they eyes whetehr on doors or in flesh are not some old god types of entities they are US.
Meaning how we all constantly watch others in order to know what the goal is, in order to stay "with it", but also we watch to judge, to make ourselves feel better not by rising up but by pulling others down. And at the same time how we are ALWAYS watched nowadays no matter where we are and what we do.
The signal is not a real signal nor some weird creature or deity or whatever. IT'S US!

Its becuase he believes the world hates him. Little nightmares is about suffering from issues such as trauma, obsession, disorders etc and how those issues often result in damaging behaviors that either make them worse or perpetuate said issues.
Example: victims of trauma who cope poorly often inflict said trauma upon others.
This is why mono becomes the thin man after he is left alone. He had defeated his personal nightmare becuase of his friend. But then he was abandoned by said friend and so succumbed to it, becoming the thin man. He then goes on perpetuating the cycle.
12:27 the ghost kid covers his face just like mono because mono dosent want people to know his identity when he grows up, i think the ghost kid is doing the same. we know the ghost kid is missing his right arm but if you look closely the hunter is also missing his right arm, if you look the hunter has a fake right arm he put on himself. i think ghost kid is covering his identity because he dosent want people to know he will end up as the hunter (just like mono covers his face so people dont know he is the thin man)

i personally think that one idea is that six is the sixth iteration of a line of girls who become the lady and mono is a constant, always there always the same person who becomes the thin man as he is and always will be him. this can be possible with the porcelain lady doll you can find in chapter 4 for the unladylike trophy with the description 'hello, have we met?', which i believe to be her asking mono as every lady to be has met him and made him the thin man to repeat the process
Also I have a theory that Six had been in the hospital before. She seemed to know where she was going and took the lead way more during that section of the game. I think that when you return to her and see her breaking the fingers of that mannequin it’s because she already knows about the hands and is immobilising it in case of it coming to life. Not to mention she seemed to enjoy watching the doctor burn as if she had some sort of history/vendetta with him

It’s likely the Doctor has tunnel vision on Mono as might be enraged that Mono took off his Patients life support, either that or he thought because Mono was trapped in the furnace that he was easier to go after.
Maybe the one on life support was Roger but he just combined a lot of different arms together to give him the long arms.

No, the long arms and the short legs was an effect of the distortion of body parts monsters of the Little Nightmares, caused by the frequencies of the signal tower. Monster Six is an example of how "Roger"/ The Janitor gets his main iconic features.

So the time spent inside the signal tower might have corrupted Six's brain, which would explain her hunger problem. Or even why she is so aggressive towards Mono after he destroys her music box.
My theory is that mono was in a loop and then, after six let him go she went back to the maw to find out more about that place then kills the lady and when she grows up, years later, she becomes the woman living in the maw and staying away from the main land away from mono or the tall man

This has raised my Anxiety Levels to a whole new Level but It was worth it for The Lore!-
Roger was likely trying to escape the Pale City, because he didn't want to be absorbed by the Eye through the tvs. That may be why he was blind, simply trying to tear out his own eyes in order to avoid becoming a Viewer. This could also be why he was so infatuated with the tv when you turn it on--he simply cant help himself, there's still part of him that is attracted to the TV's like all the viewers. We know that Roger went to the Maw to escape some greater threat, and I think he could have been trying to escape the corruption and mutation of the Eye. That clearly didn't work however, seeing as he's just as monstrous as the rest of them.
Also, does anyone else wish they put more of the Viewers with cords attached to TVs or TVs for heads like they put in the sketches? I think it would be so cool to see a Viewer melded with a broken TV that tries to attack you but can't due to being anchored down. Or when you hop through a TV it turns out to be one that a Viewer is attached to, so you have to run away quickly.
2:06 - I wonder if perhaps the missing limbs is meant to be a twisted reference to that old phrase "costing an arm and a leg" when one buys something extremely expensive? That, if going with the plastic surgery idea, perhaps those people were so desperate for that physical perfection, that they were willing to LITERALLY pay an arm and a leg for the doctor's services.

Something you might find interesting is Nikko has managed to break the game to play it in first person, meaning you can get some close-ups of the theories you are making here. For example, they go in super close to the boy on the chess piece in the school, and even all around him so you can see whether he is a real, or porcelain boy. hope this helps with your theories!

But I have a question: Do you think the creature Mono seems to be walking on before getting to the chair in the end could be a giant brain-thing? Brain is what takes in all the eyes see - even things we don't look at mindfully - and twists it into nightmares. That's the connection I made when I saw it for the first time.
At this point I am fairly certain it's all the residents mashed up together, so there would certainly be brains in there at least!

To my completely untrained eyes the brain damage looked like it could've been from a trans-orbital lobotomy (basically stabbing someone in the brain through their eye), which if I remember correctly were used to "treat" the mentally ill because it effected their sense of self and generally made them docile
