Lung Sounds (Abnormal) Crackles (Rales) Wheezes (Rhonchi) Stridor Pleural Friction Rub Breath Sounds

  • Abnormal lung sounds that include crackles (formerly called rales), stridor, wheezes (formerly called rhonchi), pleural friction rub, and stridor. There are two types of abnormal breath sounds such as discontinuous and continuous. Discontinuous sounds include: coarse crackles, fine crackles, and pleural friction rub. Continuous sounds include: low-pitched, monophonic wheeze & high-pitched, polyphonic wheeze, and stridor. Fine crackles sound like high-pitch, crackling noises mainly on inspiration and can't be cleared when coughing. Coarse crackles are low-pitch, wet bubbling sounds that can occur mainly during inspiration but can extend into expiration. Pleural friction rub is a harsh, grating sound heard during inspiration and expiration. Low-pitched, monophonic wheezes are heard mainly in expiration and have a low-pitched whistling tune or whine with ONE sound quality. High-pitch, polyphonic wheezes are heard mainly in expiration and have a high-pitched musical instrument tune with MORE THAN ONE sound quality. Stridor is heard during inspiration and is a high-pitched whistling or gasping sound with a harsh sound quality.

    Wow just learned something new I have been under the impression that Rhonchi was the upper airway congestion that's heard when there is secretion build up and clears with coughing . The team of nurses im working with have a patient in particular that is trach dependent has lots of mucus but clears up with suctioning/neb txs/cough assist. One nurse always writes she hears crackles so I asked her about it and she was very adamant that it was crackles, I'm always one to learn so that's how I found my way here. I thought crackles did not clear with coughing and the ones I acknowledged were the sounds that were fine. Now I learned there are coarse and fine. I'm glad she stuck with her first thought because I would not have known if she gave in and questioned her own nursing skill.

    Thank you so much for sharing this video. For years I have been waking up to this fine crackling sound. I have gone to the doctor and told them about it. This sound doesn’t happen often. It comes and goes. But I am still very concerned of why it is happening. The last time this happen was about 9 months ago. I just found another doctor. She is running a bunch of tests on me. I will let her of this crackling sound.

    I currently have the "fine crackles" heard near 1m 28s in video. It's very faint and only I can hear it, but it's concerning. Does anyone know why??

    I’m still wheezing it’s like I can’t close my mouth for one minute if I do then it’s going to get worse and worse and ppl can die from not breathing so I’m trying to make sure I can still breathe

    My strange sound is more of a fast beat whistle and doesn't stop when both breathing out or in. In fact, I can breath normally but the whistle sound still goes at its own beat. Only happens when I lie down.
