It 2022, in March, and I just found this video but it is still so helpful for me. Thank you! I have had the worst time trying to get my ADD medication dialed in because I have the absolute most uneducated people in the area of ADD/ADHD medication‘s, prescribing my Vyvanse. ADHD medication do not take weeks or months to dial-in. They’re not Anna depressants or something, they don’t take forever and a day to start seeing if it works for you or not. It takes literally, 5 to 6 days to know if you are on the correct dosage and the right set up. Meaning, you shouldn’t have to keep increasing your medication because it stops working, which is my current problem. Instead, Dr. Parker says in another video, who specializes in us, you need an afternoon dose that’s a short acting to carry you through for the rest of the day. But since I have to be my own advocate and my own educator, these videos are what help me when I have to go in for another prescription (as by law, schedule 2 medications, they can only give you 3 months at a time and they’re 3 separate individual prescriptions, not refills.), and explain it to them with all the information/knowledge I have gathered on my own, in hopes that they won’t argue with me and treat me like I’m a drug seeking addict. It is awful to be treated like a pill head because everyone else ruined it for people like me, who actually do have ADD.

However, Vyvanse is mostly absorbed intact from the small intestine. It is hydrolyzed by esterase enzymes in blood. Snorting Vyvanse probably does not work because it is positively charged and cannot cross the mucus membranes of the nose. Injecting Vyvanse has been shown to get you high, in pioneering research done by kids at your local Emergency Department.

Adderall/l-amphetamine really helps my mood for some reason while vyvanse/d-amp is solely a focusing tool. Adderall fully cured my bipolar problem for ~8 months'ish so far. Amazing time we live in.
Same here. I need the L amphetamine for more full coverage dopamine boost. Just pure amphetamine (Dexedrine), doesn’t have any mood elevation component, and no body stimulation. Not bad, but I def prefer a mix salt treatment

With Vyvanse, release begins from the small intestine, the l-lysine gets cleaved off by stomach ph, and releases the dextroamphetamine. (making it strictly biologically activated.) Levoamphetamine effects the PNS nervous system indirectly effecting nor-epirnephrine.

TLDR; Adderall takes immediate effect and requires multiple doses, Vyvanse takes approximately an hour to kick in and lasts longer; only requires one dose a day. Vyvanse (most likely) does not work properly or at least as effectively if snorted. Vyvanse is approximately $300 (as someone that has been prescribed a daily dose of Vyvanse, I can confirm this. In my experience, the price does not change based on the mg dosage.) The medications are very similar but, again in my experience and own research, have different side effects. From what I have asked two different doctors, Vyvanse is far more natural and safe than Adderall. Adderall is also far cheaper than Vyvanse (probably due to the whole patent thing) and, according to the person in this video can range from $10-$70, depending on where you're from. It is vital in my opinion to note that (at least in Canada) if you visit your psychiatrist, s/he can give you little prescription cards (no idea what they are actually called, sorry!) that you can give to the pharmascist whenever you are due for a prescription refill (they may not allow you to get a refill if it is more than something like 24-48 hours before necessary as they don't want to risk any possible prescription abuse on your part) and it is far cheaper that way. I believe it's something like $50-something for it in that way. (Only been on it a few months and my father is the one that keeps the cards on him and does refills so I don't know anything about it besides that, sorry!)
I think that although you are factually correct in saying that there is technically no difference between the 2 base compounds, the inhibitor has some positive aspects that help make the drug a lot more sustainable for certain patients. Since the drug requires protein and amino acids from food for it to work, it forces you to keep up with your nutrition giving you an incentive to eat as apposed to adderall which can have a greater negative effect on your appetite. This helps to extend it's effectiveness and provide a more consistent, level feeling of focus which is preferable to the distracting peaks and crashes of other versions. It also makes it easier to come off of since it seems to stay active for a while after you stop taking it so the come down is less pronounced and it doesn't give you the same "I need to eat everything" crash that adderall does.
having experience with the adhd-meds, including ritalin/concerta, i prefer vyvanse. i like the slower gradual onset. adderall was good but due to the various salts and metabolism rate i found the onset a bit jerky. dexedrine was fast but it also has a lot more abuse potential (sniffing & injection) which is my understanding as to why it's not as prescribed. ritalin was too short lived whereas concerta wasn't, although with that i was clenching/grinding my teeth. btw - i'm coming at this from a therapeutic perspective, not someone whose used these recreationally. fortunately where i am in canada i don't pay a cent for any of these :D
On Concerta was it effective for you. I’m taking adderall but I feel like my metabolism runs through it quick so it doesn’t feel as effective but so did the XR version
Appreciate the video immensely! Just switched back to adderall from vyvanse. I felt as if the half life of the drug made it harder for me to sleep than adderall, which makes sense now.

My doc put me on vyvanse cause adder all made me so anxious and paranoid and now I know why she did that. I always thought these drugs were all the same. I don't feel no physical side effects like I use too and no wonder I can't use vyvanse in the gym also. Aderall worked like a charm there . Thanks for this info . Helped a lot
Donivan Reis Vyvanse is dextroamphetamine bonded to an amino acid. In the blood this bond is broken leaving dextroamphetamine. It would be like the difference between an orange that is peeled and one that is not. It is one way to repatent a drug, since now it is technically a new compound. It would be as if you had a patent for peeled oranges and that expired. You would then patent the unpeeled oranges, since technically it is different compound, and continue to sell it for the patented price. They are both oranges and the same thing. Dextroamphetamine is peeled, Vyvanse is not.

I've personally taken both but I was only on Vyvanse for ADHD for a couple months in the first semester of my senior year in fall 2010. The doctor took me off of it because my social worker at school and my therapist at the time thought it was causing me to act up. However, Adderall and later concerta also did the same thing, but although not quite as much. I think I was able to sleep a little more easily on Vyvanse though, and didn't perform quite as well academically on it compared to the regular Adderall concerta. However on Vyvanse I had less tics compared to when I was on the other. I also had a dual diagnosis of bipolar and also took risperdal for it from 2003 to 2012. Some years later ASD was officially diagnosed and bipolar was dropped and I no longer take ADHD meds.
Evekeo is racemic amphetamine and it was launched in the US recently at high price. Benzedrine was the ORIGINAL commercial formulation of racemic amphetamine, I believe Benzedrine launched in the 1930s as an inhaler and as a pill in the early 1950s. Benzedrine was then replaced by the enantiopure Dexedrine, made by Smith-Klein French (SKF), which has since changed its name.
To my knowledge, levoamphetamine was never approved or marketed in the US. I could check the FDA website, but making it enantiopure costs money and I have never heard of a motive. In contrast, levomethamphetamine is called levmetamfetamine and is marketed for nasal congestion.
I will never ever understand why Anti-depressive meds did not help me Lexapro, Zoloft I couldn’t sleep at night but Dexidrine makes me feel so alive and I can actually sleep at night!!

I tried vyvanse and it didn’t do much except for help me stop being so much of a scatter brain. But on adderall I was sooo focused, happier, and SUPER confident. Also I wasn’t hungry on both which is a plus because I have a binge eating disorder. But tbh I prefer adderall because I can actually tell a mental difference.

Great video. BTW, interesting pharmacology, but the bigger and more muscular you are (more lean mass), the longer Vyvanse will last. The reason is because more blood volume means the drug disperses into more blood, and your body only makes a certain amount of the enzymes to activate it. I found studies on PubMed about this. Vyvanse is a lot better than Adderall, at least in my case. Frankly, I think the only real downside of Vyvanse is it can last sometime too LONG! I take it with Intuniv (guanfacine) which HUGELY help extend it even more. Great combo. Good video.
Zenzedi is exactly the same as Dexedrine and somehow it looks like it got a patent. I am not a lawyer, it may be a "branded generic" similar to Methylin, which is generic (non-patented) Ritalin, trade name methylphenidate, with a special trademarked name - specifically, Methylin.
Been prescribed both, sometimes at the same time. 1-75mg (orange/blue) capsule of Vyvance in the morning, & 3-30mg tabs of Adderall taken throughout the day. Being said, Shire’s original product Adderall (Amphetamine Salts) is far superior to Shires new gimmick Vyvance.

I don't think you can snort it. from what I know about digestion, protein breakdown takes place in the stomach and then continued in the small intestine by proteases. isn't this the case?
It all comes down to "The Bottom Line". The reason why Vyvanse is around today (just like adderall before it) had nothing to do with "discovering" a new and "better" way to help treat symptoms of ADD or ADHD so as to help people, no no no. it's only around because the pharmaceutical industry, as a whole, is a business, and any For-Profit (Business) Organizations will tell you that they exist primarily to generate a profit, that is, to take in more money than it spends. He explains that very well throughout the video. I put together a breakdown of the video where he (if you read between the lines) talks about the fact that profit is the goal with big pharma rather than than helping people live better lives.(4:43 - 5:20, 6:36 - 8:27, 11:42 - 12:01, 13:21- 13:44, 14:11 - 14:14 /14:23 - 14:48, 15:58 -16:08 ). Make a video on DMAA and how it compares in effects and in molecular structure to Adderall, Vyvanse(dextroamphetamine), and Ritalin. Dmaa is or was a big thing in pre workout supplements. Im curious if its effects on cognition are similar to dextroamphetamine or adderall cuz it can still be gotten online for cheap wit no script as a "reaearch chemical" like $15 for 1 gram

Lauren A Ask your doctor to write you a prescription for Dexedrine extended-release spanules. My insurance stopped covering vyvanse so I switched to cheap generic Dexedrine spanules. If your new med isn’t helping you might need a higher dosage.
Hi! Not sure if you will ever see this, but I'm curious about something and would appreciate your feedback. I took vyvanse for several years and it worked very well for me but recently stopped working. I suddenly had horrible side effects ("physical" anxiety, lack of focus, restlessness, repetitive fidgets/ weird muscle movements, intrusive thoughts, etc). I am now on adderall and I feel less anxious, less restless, and more focused. Considering what you explained about the two drugs, I cant imagine why this would be the case if the l-isomer is the one that increases physiological symptoms of anxiety. Any ideas? Hi, I know this is an old comment but maybe I can contribute. I should preface by saying I am not a psychiatrist or a pharmacist, so take this with a grain of salt (haha pun). If the dosage of adderall and vyvanse are approximately the same then you are actually receiving more dextroamphetamine by taking vyvanse due to only 75% of adderall being that compound. However it is unlikely that your dosages over time were exactly the same, so I’d have to know what they were to comment more on that. The other thing that I personally felt was neglected in this video is that the release time of each compound matters a lot in terms of the effects. The faster the release happens the more of a “high” one gets, for example when someone snorts adderall at a college party. Vyvanse, due to its extra step, has a much slower release time and this means that the therapeutic effect tends to be more stable over time as opposed to adderall, at least in general. Finally, the way each one is metabolized is heavily impacted by an individual’s physiology. My Dr even told me that I should not drink lots of orange juice as it could affect my stomach’s pH and allow less of the dose into my system! Having a relatively “inert” version of the compound prior to it entering the blood could have a large effect depending on an individual’s digestion.
You sound like you work for my insurance company! It's not just about which one is cheaper! You seem to brush off the differences between Dextro and Vyvanse as if the differences are meaningless, when the reality is, Dextro and Vyvanse have much different impacts. Dextro hits like a bus and the come-down is sharp. Making it hard to find the right dosage, and I always get a headache from Dextro due to the sharp come-down. Vyvanse doesn't hit as hard initially, but it gets there eventually, and the come-down is WAY more tolerable.

Do you have any idea about how can Dexadrine cause more physical side effects than Adderall for some people? It doesn´t seem to make sense. Both the little chemistry I know from college and the "research" I have been doing this past year support exactly what your video says, that it is the Levo the one that causes most of the side effects( heart punding, insomnia, etc...) but still I know Dex has worse effect on those areas than Adderall for a bunch of us...Do you have any idea of how or why that may be 0.0? This video was quite informative up to the point until he starts to wear a tin-hat claiming that including a rate-limiting enzymatic cleavage to an L-lysine portion of a molecule was just for a patent, or that including a mixture of the two salts is a bad idea (i.e. Adderall's 3:1 ratio) concluding that drugs containing only Dextroamphetamine is the most useful. In this study, www.ncbi.nlm.nih.gov/pmc/articles/PMC3666194/, it was proven in rats that l-amphetamine significantly enhanced and prolonged the efflux of dopamine in the striatum produced by d-amphetamine, leading to a synergistic effect v.s. tin-foil hat patenting. Both isomers were also capable of increasing striatal dopamine efflux by >5000% of baseline values, though it is agreed that d-amphetamine is more potent. Now Arnold's premise was to that the two isomers have different effects on aggression and hyperkineses. Though the paper does end with yet another hypothesis, it does go over points where l-amphetamine did show benefits toward patients subjected to hyperkinetic syndrome, aggression, and hostility by both isomers via a dopaminergic mechanism, while those with anxiety or over activity could benefit solely from d-amphetamine. This is much more conclusive than the tin-foil hat that you put on about America's Patenting System™.
ok, so now that adderall IR as well as XR is available in generic forms, WHY do doctors still prescribe 'mixed amphetamine salts' aka generic adderall when dextroamphetamine (dexedrine) is really the only useful molecule? Would it be reasonable to bring this up to my doctor and request just generic dextroamphetime? Also, I recently came across another ADHD drug called Evekeo™ which is 50/50 levoamphetamine/dextroamphetamine. Seems like they're reinventing the wheel. What do you think???

With Vyvanse, release begins from the small intestine, the l-lysine gets cleaved off by stomach ph, and releases the dextroamphetamine. (making it strictly biologically activated.) Levoamphetamine effects the PNS nervous system indirectly effecting nor-epirnephrine.

TLDR;
Adderall takes immediate effect and requires multiple doses, Vyvanse takes approximately an hour to kick in and lasts longer; only requires one dose a day. Vyvanse (most likely) does not work properly or at least as effectively if snorted. Vyvanse is approximately $300 (as someone that has been prescribed a daily dose of Vyvanse, I can confirm this. In my experience, the price does not change based on the mg dosage.) The medications are very similar but, again in my experience and own research, have different side effects. From what I have asked two different doctors, Vyvanse is far more natural and safe than Adderall. Adderall is also far cheaper than Vyvanse (probably due to the whole patent thing) and, according to the person in this video can range from $10-$70, depending on where you're from. It is vital in my opinion to note that (at least in Canada) if you visit your psychiatrist, s/he can give you little prescription cards (no idea what they are actually called, sorry!) that you can give to the pharmascist whenever you are due for a prescription refill (they may not allow you to get a refill if it is more than something like 24-48 hours before necessary as they don't want to risk any possible prescription abuse on your part) and it is far cheaper that way. I believe it's something like $50-something for it in that way. (Only been on it a few months and my father is the one that keeps the cards on him and does refills so I don't know anything about it besides that, sorry!)

I think that although you are factually correct in saying that there is technically no difference between the 2 base compounds, the inhibitor has some positive aspects that help make the drug a lot more sustainable for certain patients. Since the drug requires protein and amino acids from food for it to work, it forces you to keep up with your nutrition giving you an incentive to eat as apposed to adderall which can have a greater negative effect on your appetite. This helps to extend it's effectiveness and provide a more consistent, level feeling of focus which is preferable to the distracting peaks and crashes of other versions. It also makes it easier to come off of since it seems to stay active for a while after you stop taking it so the come down is less pronounced and it doesn't give you the same "I need to eat everything" crash that adderall does.
having experience with the adhd-meds, including ritalin/concerta, i prefer vyvanse. i like the slower gradual onset. adderall was good but due to the various salts and metabolism rate i found the onset a bit jerky. dexedrine was fast but it also has a lot more abuse potential (sniffing & injection) which is my understanding as to why it's not as prescribed. ritalin was too short lived whereas concerta wasn't, although with that i was clenching/grinding my teeth. btw - i'm coming at this from a therapeutic perspective, not someone whose used these recreationally. fortunately where i am in canada i don't pay a cent for any of these :D
On Concerta was it effective for you. I’m taking adderall but I feel like my metabolism runs through it quick so it doesn’t feel as effective but so did the XR version

You said Vyvanse is dextroamphetamine. I was under the impression it was lisdexamfetamine.
Donivan Reis Vyvanse is dextroamphetamine bonded to an amino acid. In the blood this bond is broken leaving dextroamphetamine.
It would be like the difference between an orange that is peeled and one that is not. It is one way to repatent a drug, since now it is technically a new compound.
It would be as if you had a patent for peeled oranges and that expired. You would then patent the unpeeled oranges, since technically it is different compound, and continue to sell it for the patented price.
They are both oranges and the same thing. Dextroamphetamine is peeled, Vyvanse is not.
ONEEZE It's a prodrug, correct? as Codeine is a prodrug for morphine and it's other metabolites?
The amino acid lysine is attached to dextro-amphetamine really for the sake of having a new Patent and profitable drug

I've personally taken both but I was only on Vyvanse for ADHD for a couple months in the first semester of my senior year in fall 2010. The doctor took me off of it because my social worker at school and my therapist at the time thought it was causing me to act up. However, Adderall and later concerta also did the same thing, but although not quite as much. I think I was able to sleep a little more easily on Vyvanse though, and didn't perform quite as well academically on it compared to the regular Adderall concerta. However on Vyvanse I had less tics compared to when I was on the other. I also had a dual diagnosis of bipolar and also took risperdal for it from 2003 to 2012. Some years later ASD was officially diagnosed and bipolar was dropped and I no longer take ADHD meds.
Evekeo is racemic amphetamine and it was launched in the US recently at high price. Benzedrine was the ORIGINAL commercial formulation of racemic amphetamine, I believe Benzedrine launched in the 1930s as an inhaler and as a pill in the early 1950s. Benzedrine was then replaced by the enantiopure Dexedrine, made by Smith-Klein French (SKF), which has since changed its name.
To my knowledge, levoamphetamine was never approved or marketed in the US. I could check the FDA website, but making it enantiopure costs money and I have never heard of a motive. In contrast, levomethamphetamine is called levmetamfetamine and is marketed for nasal congestion.

I tried vyvanse and it didn’t do much except for help me stop being so much of a scatter brain. But on adderall I was sooo focused, happier, and SUPER confident. Also I wasn’t hungry on both which is a plus because I have a binge eating disorder. But tbh I prefer adderall because I can actually tell a mental difference.

Great video. BTW, interesting pharmacology, but the bigger and more muscular you are (more lean mass), the longer Vyvanse will last. The reason is because more blood volume means the drug disperses into more blood, and your body only makes a certain amount of the enzymes to activate it. I found studies on PubMed about this. Vyvanse is a lot better than Adderall, at least in my case.
Frankly, I think the only real downside of Vyvanse is it can last sometime too LONG! I take it with Intuniv (guanfacine) which HUGELY help extend it even more. Great combo. Good video.

Zenzedi is exactly the same as Dexedrine and somehow it looks like it got a patent. I am not a lawyer, it may be a "branded generic" similar to Methylin, which is generic (non-patented) Ritalin, trade name methylphenidate, with a special trademarked name - specifically, Methylin.

Been prescribed both, sometimes at the same time. 1-75mg (orange/blue) capsule of Vyvance in the morning, & 3-30mg tabs of Adderall taken throughout the day. Being said, Shire’s original product Adderall (Amphetamine Salts) is far superior to Shires new gimmick Vyvance.

It all comes down to "The Bottom Line". The reason why Vyvanse is around today (just like adderall before it) had nothing to do with "discovering" a new and "better" way to help treat symptoms of ADD or ADHD so as to help people, no no no. it's only around because the pharmaceutical industry, as a whole, is a business, and any For-Profit (Business) Organizations will tell you that they exist primarily to generate a profit, that is, to take in more money than it spends. He explains that very well throughout the video. I put together a breakdown of the video where he (if you read between the lines) talks about the fact that profit is the goal with big pharma rather than than helping people live better lives.(4:43 - 5:20, 6:36 - 8:27, 11:42 - 12:01, 13:21- 13:44, 14:11 - 14:14 /14:23 - 14:48, 15:58 -16:08 ). Just like in politics and private/for-profit prisons... we need to take money out of our (the U.S.) healthcare system.

Lauren A Ask your doctor to write you a prescription for Dexedrine extended-release spanules. My insurance stopped covering vyvanse so I switched to cheap generic Dexedrine spanules. If your new med isn’t helping you might need a higher dosage.

Hi! Not sure if you will ever see this, but I'm curious about something and would appreciate your feedback. I took vyvanse for several years and it worked very well for me but recently stopped working. I suddenly had horrible side effects ("physical" anxiety, lack of focus, restlessness, repetitive fidgets/ weird muscle movements, intrusive thoughts, etc). I am now on adderall and I feel less anxious, less restless, and more focused. Considering what you explained about the two drugs, I cant imagine why this would be the case if the l-isomer is the one that increases physiological symptoms of anxiety. Any ideas?

Hi, I know this is an old comment but maybe I can contribute.
I should preface by saying I am not a psychiatrist or a pharmacist, so take this with a grain of salt (haha pun).
If the dosage of adderall and vyvanse are approximately the same then you are actually receiving more dextroamphetamine by taking vyvanse due to only 75% of adderall being that compound. However it is unlikely that your dosages over time were exactly the same, so I’d have to know what they were to comment more on that.
The other thing that I personally felt was neglected in this video is that the release time of each compound matters a lot in terms of the effects. The faster the release happens the more of a “high” one gets, for example when someone snorts adderall at a college party. Vyvanse, due to its extra step, has a much slower release time and this means that the therapeutic effect tends to be more stable over time as opposed to adderall, at least in general.
Finally, the way each one is metabolized is heavily impacted by an individual’s physiology. My Dr even told me that I should not drink lots of orange juice as it could affect my stomach’s pH and allow less of the dose into my system! Having a relatively “inert” version of the compound prior to it entering the blood could have a large effect depending on an individual’s digestion.

You sound like you work for my insurance company! It's not just about which one is cheaper! You seem to brush off the differences between Dextro and Vyvanse as if the differences are meaningless, when the reality is, Dextro and Vyvanse have much different impacts. Dextro hits like a bus and the come-down is sharp. Making it hard to find the right dosage, and I always get a headache from Dextro due to the sharp come-down. Vyvanse doesn't hit as hard initially, but it gets there eventually, and the come-down is WAY more tolerable.

Do you have any idea about how can Dexadrine cause more physical side effects than Adderall for some people? It doesn´t seem to make sense. Both the little chemistry I know from college and the "research" I have been doing this past year support exactly what your video says, that it is the Levo the one that causes most of the side effects( heart punding, insomnia, etc...) but still I know Dex has worse effect on those areas than Adderall for a bunch of us...Do you have any idea of how or why that may be 0.0?
I haven´t manage to get any straight answer besides "everyone reacts differently", but this sounds like a very weird kind of different, when one isomer has an effect it just shoudn´t have at all... So I´d appreciate your thoughts on it

This video was quite informative up to the point until he starts to wear a tin-hat claiming that including a rate-limiting enzymatic cleavage to an L-lysine portion of a molecule was just for a patent, or that including a mixture of the two salts is a bad idea (i.e. Adderall's 3:1 ratio) concluding that drugs containing only Dextroamphetamine is the most useful.
In this study, www.ncbi.nlm.nih.gov/pmc/articles/PMC3666194/, it was proven in rats that l-amphetamine significantly enhanced and prolonged the efflux of dopamine in the striatum produced by d-amphetamine, leading to a synergistic effect v.s. tin-foil hat patenting. Both isomers were also capable of increasing striatal dopamine efflux by >5000% of baseline values, though it is agreed that d-amphetamine is more potent. However, I also found it funny how this study was "support funded" by Shire themselves, so I myself put on my tin-foil hat and dived through their references (you may have heard of a reference, but it is something you did not list in this video): pdfs.semanticscholar.org/a231/0e705013f26dbe2380525e8e8f475623e6d5.pdf.
Now Arnold's premise was to that the two isomers have different effects on aggression and hyperkineses. Though the paper does end with yet another hypothesis, it does go over points where l-amphetamine did show benefits toward patients subjected to hyperkinetic syndrome, aggression, and hostility by both isomers via a dopaminergic mechanism, while those with anxiety or over activity could benefit solely from d-amphetamine. This is much more conclusive than the tin-foil hat that you put on about America's Patenting System™.

ok, so now that adderall IR as well as XR is available in generic forms, WHY do doctors still prescribe 'mixed amphetamine salts' aka generic adderall when dextroamphetamine (dexedrine) is really the only useful molecule? Would it be reasonable to bring this up to my doctor and request just generic dextroamphetime?
Also, I recently came across another ADHD drug called Evekeo™ which is 50/50 levoamphetamine/dextroamphetamine. Seems like they're reinventing the wheel. What do you think???

Have you ever seen street Amphetamine being made? It's unhygienic,it's explosive environment,it's rotting people's stomach linings and there teeth.
