    Every year more workers die attempting rescues in confined spaces than victims of confined space accidents. Enclosed or partially enclosed areas can be deadly. Employers and workers must be aware of the hazards of confined spaces.
    Incidents in confined spaces are not common, but they can happen suddenly and when they do occur the consequences can be devastating. Employers must take the necessary steps to ensure worker safety around confined spaces.

    This made me want to go hug my dad and tell him I love him. I can't imagine the pain Cody went through. I'm glad he's not eating himself up with blame over not being there sooner. No way he could've known.

      I watched this one today in my Maritime Class at RCM Maritime and I felt the same way. That one hurt me to watch. Most of the time stuff like this doesn't bother me, but this did. Rest in Eternal Peace Mr. Dan!!

    The safety director at my job told us that we should watch this, but also said that she couldn't show us due to language. (my work place had a no swearing policy). I'm glad I looked this up and wish that we were able to show this at work. It really nails what NOT to do.

    Great video, shows the importance of a pre-entry check with a multi-gas detector and attached to any personnel whilst working in any confined space

      It also shows the importance of being trained in confined space emergency response and it also shows the importance of not entering or rushing into a confined space to preform a rescue. Cody and his colleagues were lucky that they survived entering the confined space as in most cases when people or would be rescuers enter confined spaces and are overcome they usually end up dying as a result of their exposure to poisonous gases or a lack of oxygen.

    An excellent depiction of how seemingly innocent locations, places people have been working in many times before without a problem, can become deadly. Simple oxidisation in enclosed and partially enclosed spaces is something easily overlooked.

    I had to find out more information on this story bc the video left a lot of gaps and questions that weren't answersed. I think it's misleading and deceptive to attribute this to confined spaces alone without also explaining why the men collapsed. So I read up on the story and there wasn't any chemicals or gasses down in the compartment that immobilized the workers. According to their analysis, the compartment which had been sealed for 3 years was rusting and since there was no ventilation or no proper ventilation, the rusting process used up the oxygen inside the compartment bringing the concentration down to less than 16%. The other compartments were ventilated and when tested they showed an O2 concentration of 20.9-21% which would not have impaired the workers had they been in any other confined space on the barge. If something had happened to father in another compartment, the son would have been adequately able to go for help but because there's was little oxygen and he stayed with his father so long, each breath he took brought him close to passing out. So that is the reason his father died and the workers passed out. It wasn't chemicals or gasses, it was lack of oxygen due to the poor or lack of ventilation of a rusting compartment that consumed the available oxygen while it was sealed.

      It's not misleading and deceptive to attribute this to confined spaces because it IS a confined space. A confined space by definition has the following attributes: 1. Is either enclosed or partly enclosed; 2. Is not designed for continuous human occupancy; 3. Has limited means of entry which may complicate the provision of first aid or rescue; 4. Is large enough for a worker to enter. Clearly, the work area in the video fits the definition. Note that there is no mention of a potential atmosphere, either real or developing. Once a space is defined as a confined space, however, it sets off a series of protocols which must be followed before entry. One of those is ensuring the atmosphere is fit for use, which means testing. Approach every work area with a critical eye and always ask questions. Hope this helps.

      I think he means its misleading because he THINKS they never mentioned why they collapsed in the video. He says he did some research to figure that out. I believe he just missed the part in the video where it mentions it was a lack of oxygen (9:01) and thinks they didn't give that information, which I agree, is kinda important to understanding why they collapsed, ya know? You don't just fall out from being in confined spaces in general.

      Anything below 19.5% oxygen in atmosphere is too low and you'll fall unconscious. The reason there is no panting & choking, is because one is still happily breathing nitrogen, which is roughly 70% of air in atmosphere. For the record, the amount of oxygen in air should be 20.9%, that's normal air. By definition, they're not suffocating, they're suffering from hypoxia.

    From what I understand a lot of these shipboard confined spaces are *extra* nasty because the motherfucking hatches are often difficult or sometimes even impossible to fit through with breathing gear on. So sometimes even people who know they need the gear will go in without it, underestimating how fast the problem will affect them, or figuring they can work fast enough.
    So, in addition to worker training and equipment provision, I think there also needs to be a hatch retrofit program. If something is being scrapped, cut a bigger hole on principle.

      The exact situation you describe occurred in a US Coast guard vessel when a crew member entered the chain locker which could only be entered from a small and narrow hatch which was usually bolted shut and an access ladder consisting of small indents in the bulkhead. When the man reached the bottom of the chain locker he was rendered unconscious/unresponsive due to the low oxygen levels in the compartment, and when he failed to emerge and respond to spoken calls other men entered and were also overcome and the rescuers found that their closed respirators were too bulky to let them pass through the hatch whilst wearing them so they had to lower the respirators in and don them whilst partially inside the chain locker.

      I agree. I work for a company cleaning out tanks in barges and it drives me nuts that they don’t do drills or have someone trained with a oxygen mask. They’re should always be someone trained with an oxygen mask, a tripod and pulley than can quickly be set up over the man way and and a ropes or tether that can be brought down by the trained individual and attached to a stretcher if someone passes out down there. I honestly don’t get paid enough for my job

      inch big one There was very little oxygen in the sealed area. They didn't check for oxygen amounts before going in after they unsealed the compartment. The oxygen was used up because the steel inside rusted. Rust is oxygen combining with a metal.

    It would have been good to tell the viewer what gas was there and why it accumulated down there and further led to the tragic loss of the elderly gentleman.

      I'm guessing there was no toxic substance down there. It was a sealed compartment. Sure, it was filled with oxygen when it was sealed. But as the bulkheads began to oxidize the oxygen levels would have dropped as part of that reaction. Could have been as low as a few percent. What I don't understand is how these people didn't know this was a very real danger. I mean, people knew about the dangers of enclosed spaces long before WWI and took precautions.

      Little oxygen huge killer in confined space reading on a meter supposed to be 19.5 minimum or 23.5 maximum at my work every morning we have to have it checked before we go in for hot work all the time we need ventilation so the oxygen doesn’t go below what it supposed to be for the reading even for gas lines and torch leads one fan goes in one tube blows in one blows out it’s a huge safety deal

      the hatch had been sealed for 3 years (at the point of the accident) and inside things had been rusting. the rusting process takes up oxygen and because the space wasn't ventilated (all the oxygen was used up by the rusting and had not been replenished) the level of oxygen was at too low a level to sustain human life. they passed out because they had suffocated, no oxygen was getting to their brains or anything really. the dad died because he had been without oxygen for too long. cells everywhere start dying at around 2 minutes maximum, but if you can get oxygen soon you can survive cells dying, usually it leaves permanent damage however, especially in the brain. if too many cells die then obviously you die.

    The video was filmed in an apparently new space. Brand new metal, grease pencil markings on the bulkheads. The video filmed above deck is dirty and greasy. The problem would be H2S or Hydrogen Sulfide gas. The compartment would have contaminated water in it. H2S dissolves readily in cold water. It is heavy, and sits on top of the water. Simply walking through it would release the gas out of the cold water. Warming the water releases the gas as well. The father may have slipped or tripped and fallen into the water, overcome by the gas. First man down. The son entered the compartment, stirring up more water releasing more H2S. At the level of his father's chest was over come by the gas. Second man down. Each would walk through the water stirring up and releasing more H2S, thus having a shorter walk. Third and fourth man down. The ones who survived are lucky to be alive! A four-gas gas detector would be the answer, along with proper training.

      I read up on the incident. They don't believe it was H2S. According to reports, this particular compartment on the barge had been sealed for 3 years prior to opening and was not equipped with ventilation like the other compartments. Over time, the compartment had rusted using up the oxygen bringing the concentration down to below 16% compared to the 20.9-21% in the other compartments. So the men basically walked into a death trap where there wasn't enough oxygen and maybe for his father, his age, weight, health, along with the time he was in the space contributed to the fact that he died and the others had survived. From what I read the "ladder" they used was only 2 m (6.75 ft) high which is 3/4 ft taller than I am. I understand it was dark but what I don't understand is, since it doesn't say if it was a ladder or rungs or whether it extended to the bottom or whether there was additional length beyond the last step, even if it's dark there should have been a little bit of light to where you could see to the bottom below the hole or even slightly further beyond the hole especially with flashlights. I know it's a reenactment but somebody had to have saw at least that the last man called for help but then collapsed. If Cody and his father were further down, I can see not being able to see them but if the 3rd guy who entered came back towards the hole to yell for help then collapsed, I don't believe that the 4th guy went into the hole not knowing that something was seriously wrong. I think he saw him and thought he could quickly get to him and get out. And if he collapsed right away, I don't buy that the last guy above ground couldn't see him. Again, I know it's a reenactment but I can see Cody staying with his father further in beyond the light (although saying I'm going to help and then not moving is not the definition of "going to get help") but had he gone towards the hole I still think the other guy may have tried to go in for him but he could have alerted the others on deck that he was passed out and they could have assumed or predicted his father was in there too but nonetheless,I think the 3rd guy couldn't see Cody and ran back to yell for help alerting those on desk, but then he passed out and I think the 4th guy saw him but didn't see Cody and was just trying to be a hero and ended up putting himself and the others at further risk. Although I'm still torn as to how they couldn't see down just under 7 feet with 2 flashlights when you can lie down on a bed and shine a light ahead of you and see it on the wall in front of you over 10 ft away. If the other two had just radioed, called or went for help, help could have gotten their sooner.

      @@mjallen13081. I think I can guess how there was difficulty seeing people in there. For one thing, their lights would have been pointing away from them (and somebody might have collapsed on top of one. For another... The only view in from outside was the hatch, and looking into a dark space from out in the sun can cause difficulty with seeing light that's inside it, especially when looking through a relatively small hole.
      2. I had figured it was oxygen depletion from rust, since I knew about that particular bit of shipboard nastiness from a video about two unfortunate men dying while trying to work in a ship's chain lockerthank you for the extra confirmation and for pointing it out.

    Before entering any confined space use a sniffer to check for O2 , carbon dioxide ,and any other gases ,chemicals,.before not after you have gone inside.take percausions .

    This is a very hard-hitting video. It is the best I have ever seen to drive home the importance of safety in confined spaces. I am not offended by foul language, but if you plan to expand your audience you will have to drop the four letter expletives. I like your approach because it is real, but some people would be so focused on the bad language that they would fail to get the message you are trying to get across.

      That's just plain silly, EVERYONE knows all the words, were not 2 year old children, and almost everyone uses, fuck, shit damn and a bunch of others every day anyway, especially teens and 20 somethings. The most annoying damn thing is seeing crap like F****K it's like this isnt FOOLING anyone!! we all know what the word IS, just USE IT were not 2 years old!

      Mystery is why after the first 2 guys were obviously down there and not responding, did the 3rd and then 4th guy climb down TOO! the one guy is arguing with the 4th guy about not going down when he should have been running to the phone for help!

      From 2011 to 2018, 1,030 workers died from occupational injuries involving a confined space. The annual figures range from a low of 88 in 2012 to a high of 166 in 2017. This continues to happen every year and the root causes are: not being aware of the hidden dangers associated with confined space work, like hazardous atmospheres - oxygen deficient etc. You can't smell or taste or sense these types of hazards, you need a multi gas meter. the second leading cause is lack of training and or not implementing the steps required for confined space work as per the OSHA standards USA or OHS in Canada. It's so sad that the numbers are what they are because confined space deaths are preventable. If your not sure if a space that you have to enter is safe, consult the Confined Space Flow Chart on the OSHA website, ask questions and don't take chances. Be safe :)

      only if you want the maximum amount of people to die. Jumping in an oxygen deficient space with no breathing equipment to save someone just means you die as well
