  • Buprenorphine is an increasingly popular alternative to long term opioid use, with a lower risk profile. But how to safely manage acute pain episodes in patients who are receiving buprenorphine therapy? Clinical pharmacy specialists Foy and Uritsky discuss…

Its sickening and tragic how the Healthcare community now treats every chronic pain sufferer as a drug addict. They see your history of narcotics and instantly red flag you and tell every other hospital and doctor NOT to give you anything. Even dentists now won't give you any pain killer. Lol now they call motrin and tylenol "Painkillers" to try to confuse you... My dentist even said "even if you have surgery, we wouldn't give you any narcotic... Thats our policy now due to the war on drugs."

Actually I was forced to have a spinal surgery before given any opioids, only paracetamol afterwards. Less than a year later my pain came back to stay. Prescription grade NSAIDs don't even touch it now but just 30mg codeine would have saved my life if I could just get it without having to visit the doctor almost every week for refills (there is no paracetamol free codeine and they aren't allowed to have more than 300mg per package while it's still prescription only and my doctor won't prescribe more than 20 pills). The weekly lecture I get on how I should use Gabapentin instead (I will never put this shit in me) just adds to the suffering and my rage towards the medical community ripping people off while they are still suffering!

@@Sketch1994 I am so sorry you're being gaslit and punished for having pain!! It's awful what you've gone through 😥 I had 2 dentists tell me just 2 weeks ago, to my face that they value their license over my pain and suffering, and that it's their policy to take care of themselves first before patients needs. It’s absolutely criminal how insurance company’s are treating chronic pain patients ! they are dictating how much pain medication they will fill I am filing lawsuit I’m done being treated like a fking criminal §, have been on pain meds for 26 years or more and I have never had a issue ever, our chronic pain support group has lost over 20 people to suicide because doctors have stopped prescribing them and they were bed ridden and in severe pain!! Somethings got to give or I will lose everything my jobs very physical and without proper pain meds I will lose the job.

Couldn’t agree more after being on high dose of opioids for over 15 years because of the “opioid crisis’” I’m now on Suboxone and in constant pain
On top of that now I’ve sustained another injury and going to the dr is a complete waste of time as they won’t do anything for you at all
A few months ago I had a kidney stone and went to the emergency and I happen to be in a room with someone in for the same reason
Right in front of me they were offered and given oxycodone for the pain and I was given nothing
It’s absolutely disgusting what they have done to us they might as well dropped us all off on a desert island

  • @eBrigid

Amazing. In story after story, patients tell of having their chronic pain medications withheld by hospitals, flat out refused even if they are the patients own prescriptions. Then also refused post-surgery opiate analgesics. Slamming the already hurting patient into withdrawal on top of the pain that put them into the hospital. So then you have a patient in complete deterioration and also panic and rage, having to fight for their life and fight the staff of the hospital, having to make threats and demand patient advocates, trying to leave the hospital so they can at least resume their regular chronic pain medication. If these are your recommendations, do not delay in getting them out there, because people in pain are still suffering, medically deteriorating, and suiciding every day.

Good point; I would argue it’s worse. Last much longer, and because of the enormous half-life, substituting with any other exogenous opioid for maintenance is impossible until you’ve gone through at least five days of full-on withdrawal. I have b,een prescribed bupre for years for arthritis pain. However when the pain pointed to hip surgery the bupre had little effect on the pain. However I could not take stronger meds because I needed to work. When it cam time to induce pain meds just prior to surgery the Dilaudid had no effect. When they found out I had been taking bupre for years (but not the week prior to surgery) they immediately went to fentanyl....that worked. However post surgery, the Dilaudid again had preactically no effect once again. I had to to triple the dose of Dilaudid (with surgeon permission) to get post-op pain relief. I was so happy to get back to bupre.
Recent mental health studies show bupre patients score higher on mental health indices than geberal population. In my case this is true. My outlook and expectations are greatly improved with bupre. I just feel happier. Diluadid and fentanyl definitely did not have this effect.

It's a civil rights violation to not have your pain managed. If they take your meds contact an attorney. Then get a second opinion from out of network pain doctor. Take that to your pcp. When that doesn't work. Take them to court

Been on subutex since 2008 from being on a trial at Columbia/Presypeterian and it still works on my pain. Yes I take a very large amount 32mg a day but so what? over these years I have developed tolarence as all opiates can do but the point is it is working. The analgesic ceiling effect is BS I can attest to that. Yes low dose even a patch for opiate naive patients but long term? No you will need more plus when I had my hip replaced they used hydromorphone along with my buprenorphine and surprise! it did over take the buprenorphine and provided relief then-back to just buprenorphine. The take away I hope people understand is two fold-buprenorphine is a pain killer and if one needs post op pain relief, an additional strong opiate will work in conjunction-one does not need to stop their bupe.

OK. But, what do we do when we need surgery and have acute pain that suboxone simply isn't going to work on. Yes, I'm sure it works well for opiate naive people. But, all you're doing for a suboxone patient is taking them back to baseline.I don't think the medical community has answered this question sufficiently.

When I had hip surgery I took bupe and hydromorphone but a very high dose of hydromorphone and they worked just fine together, This is a myth that you cannot take regular opiates while on bupe-you can but need a higher dose. I'm living proof and when the acute post surgery pain was done, I stayed on the bupe.

@@glenbearh9109 you are very lucky. Most people who have addict on their medical record don't get much. I just had surgery a month ago and was given 10mg oxy every 8 hours. So, 30mg a day. My bupe dose is 24mg a day. I told them it didn't work at all for the pain I was having and was more or less told too bad. They didn't do anything for me. So, I went through post op with nothing but Tylenol and my normal bupe dose. It was miserable. I wasn't even allowed to take motrin because it can make you bleed more.

What about the withdrawal that is nearly as bad, if not as bad, as opioid withdrawal? I've been on buprenorphine for 7 years now because I can't get through the absolutely debilitating agony of getting off and leaving my receptors raw. There's no buprenorphine for buprenorphine if that makes sense. Kratom has potential. Still so much better than being a slave to opioids.

@Cager Carter (Omg, I'm an idiot. I didn't see the end of your comment. XD Either way, all the rest of what I said still stands)
Look into using Kratom for getting off of it. I'm going to start doing this myself because I want off of this drug and Kratom seems to be the only thing that's allowed people to actually do it with minimal to no withdrawal, and as long as you do it responsibly, I've seen MANY UA-cam videos from people telling their stories of how it saved them from being a slave to the long term effects of either Buprenorphine or Suboxone (which is still Buprenorphine, just has nalaxone added to it).
If you're willing, because I'm looking to start as soon as I order my first batch (I've been on the fence about which kind I want to use), I'd like to talk to you about your own experience with this drug and what it's done to you physically as well as mentally and see if I'm right in believing that this drug has done me more harm than good. Also, should you also choose to do the Kratom taper route like I will be, might make it easier to trade notes on what works and what doesn't.
We don't have to suffer in silence anymore. I've decided I'm finally ready to kick this drug to the curb. I just can't do it anymore.
