Today is 2024/08/09 and my birthday, this is last session of my internship in TMS and TDCS , I'm psychology student and this opportunity is priceless for me to be aware of such instruments
I participated in a clinical trial for rTMS at the black dog institute is Australia about 10 years ago, for treatment resistant anxiety and depression. During the treatment regime I described to the neurologist, at length and unprompted, changes to my metacognition that I was perceiving in almost real-time. This description correlates with my experience remarkably well.
I bought a fisher Wallace home device. I've had it for less than two weeks. Last night was my first night I slept without any assistance medically. Which is ground breaking to me. I have a history that stretches as far back as 13 years. It's a miracle!! I do it twice a day at home.
I've been subject of a test with TMS. I never quite knew exactly what was wrong with me. All I know is that right after the first session with TMS I felt I could understand how people manage to stay motivated and going through their daily routines. I could almost derive pleasure from activities. But the truth is, that at least for me, I could only feel it for the first two weeks (the regimen they were testing was a 50hrtz, 30 pulses 10 min every day for 4 weeks). It really improved my focus and helped me feel things very positively. But it passed. I would really love to have that fortitude of mind.
So the DLPFC is usually the area that is stimulated in TMS for depression because in many ways it's the highest part of the brain. It's the part that decides how to respond to information coming in. It has the cells that respond to what just happened, and that trigger what action to take. It's hypothesized that psychiatric patients have a lack of DLPFC activity; a lack of conginitive control. To much rumination, negative thinking etc.
Had anyone completed TMS? Does sleep improve overall? I am on week 3. Any gap in treatment I struggle with sleep (Such as over the weekend in between treatments).
What's confusing is that they don't know why it works. That's scary to me. I want to know why, or at least a general idea of why it works. Also I want to know, if it works on helping depression and can create such a big difference, then what "other" big differences is it making?
Like anything related to the phenomenon of consciousness, there is no exact way of knowing. Only experimenting. I have been through one of the possible regimens and I can say to you that it really something in me (it unfortunately lasted very shortly) but it helped me understand that I'm not as simple as I would like to be. We are all complex organisms.
Why and how it works are explained in these videos IMHO TMS->increased Left DLpfc Activity, that improves Executive function and cognitive control. This network is the Attention/Executive network, one of the major large scale cortical networks. Standard opinion is that DLPFC Inhibits Subgenual Cortex (Brodmann area 25) which is active during sadness. (See Drevets et al)
a lot of psychiatry is like this, someone did something in an experiment at one point, maybe expecting some result, said oh it does this as well? We can use it to treat that, (viagra for example). So not knowing why it works or did what it did actually makes scientists want to research and suddenly they have break throughs on how the brain works even more.
TMS is not usually used for seizures. But tVNS (transcutaneous Vagus Nerve Stimulation) works well and is very safe for seizures (and anxiety, headaches and insomnia)
Dr Rustin Burlow Can TMS cure depression caused by electrocution. ? I have had debilitating insomnia triggered by sleep onset myoclonus for the last 11 years since 2010 and have been on different meds . This treatment does not work well, so in desperation to rid my insomnia I used a device (Fisher Walace stimulator) at home that passes a 2 amperes current for 20minutes (did 10 in 5 days sessions total 200 minutes of stimulation) from electrodes placed on both sides of the head to improve /erase insomnia , but there was no improvement (insomnia). Instead it triggered adverse symptoms of depression , tinnitus, nerve pains, nerves transmitting wrong messages (e.g today if i listen to white Noice (e.g rain sounds) to calm my mind trying to sleep, instead i experience pains at the bottom of both my feet , AND a strong nervousness feeling in my head and along my spin. Also when i eat something or take anything including medications i experience SRANGELY enough pains under both of my feet (which lasts for 15 minutes then slowly disappears) .Back on eating food normally after you swallow food you feel it down in the gut .In my case i don't feel(filling up feeling) in my gut but instead i get this feeling (filling up of food)in my spine (as if the food is going to my spine instead of stomach) a feeling that starts slowly from the bottom of the spine(tail bone) and proceeds upwards my spine. Question? On depression can TMS be used to treat depression induced by electrocution to the head as in my case? What do you think the device(mybe it was faulty or i used it wrongly) did to my brain? Do you think the 2 ampere low voltage current stimulated all parts of the brain? Actually my memory is not that good, My speech has been impaired i now sometimes stutter when i speak, i feel a tight band around my head . How can all this symptoms be reversed? In my case would TMS be better than ECT.? What should i do? Plz respond to me.I liked you presentation so much ,i would like to talk or work with you. Thanking you in advance
I believe early versions of the equipment were based on diagnostic tools for stimulating nerve responses and mapping connectivity in the brain. Mind you, I'm possibly half remembering that info.
Correct. Its original intention was diagnostic in nature. It was studying nerve responses in the body and state changes in the brain after being stimulated via the magnetic field.

There are two different machines one for depression and one for OCD, anxiety, and PTSD. The different machines target different places in brain and different current level.You have to find the TMS for OCD/anxiety. My son is getting it for OCD! Tourette's, and anxiety. It is interesting because the do testing and illicit a response first and do a map so they are targeting the right spot.
@Moataz Ahmed we don’t really use “permanent” or “cure” in mental health. Depression is a relapsing remitting disorder. Oncologists (cancer doctors) do the same. They would tell you “you are in remission” they would never say it’s permanent.
Nope, this is entirely different from electroshock therapy, as this is using a magnetic field to stimulate the brain, causing the neurones to fire chemicals, these chemicals produce a signal or current that is transferred from one nerve cell to another.

mike gramm Really??? Seems like a light bulb is off. When did you realize that something was off? I realized that my mind became foggy one morning in October 2018, I couldn’t fully awaken. Although, I couldn’t shake this fog I reported for work. I thought my problem was from two knee surgeries I had during the prior summer months, specifically, from the anesthesia.
I wanted to say, electro shock is now called ECT, and has been developed from a torturous guessing game into a real medical treatment. Nowadays, ECT is given to people under anesthesia with very specific guidelines. It is similarly given to people with difficult-to-treat mental illnesses, etc. I hope you feel better soon!
@@altheaunertl ECT can also bring about side effects like memory loss. TMS looks like the way to go. I'm seriously considering it for anxiety, as I've seen a few videos on youtube where people have related their experiences gaining success over anxiety with TMS.

I don't think they know why any psych meds work
You'll be waiting hundreds of years for the "why" in mental health. Most treatments for mental health are not really understood.

1hz right DLPFC is most common for GAD.
