What app/program do you use to edit these noteblock covers?

I use Premiere Pro for editing, and replay mod was used for the intro. The effects (time set commands, beacons, and the disappearing exit) were all made in-game with command blocks and redstone

No, I actually build all my covers myself. I use structure blocks to help speed up the process by pasting an 18×12×2 glass layer with the repeaters, leaving only the note blocks to be placed and tuned.
