Had this happen on 2 different ps4. First ps4 hard drive was pooched, had to replace it. 2nd ps4 followed these steps and it worked. With "fail" screens same as yours. ***Last resort before taking in to have fixed. This WILL erase all data including user info, games, settings. This will reboot your ps4 to a "new install" state as if you had just taken it out of the box. 1. Unplug ps4, download update file from playstation support onto USB drive. Make sure to make and name folders and file correctly. 2. Plug in ps4, start in safe mode (hold power button for 7 seconds, release when you hear second beeps) In safe mode, update system with USB. (This fails but is necessary) 3. Unplug ps4 for 30 seconds, then plug back in. Start ps4 in safe mode with USB still in, initialize with update (This fails but is necessary) 4. Unplug 30 seconds, plug back in with USB still in. Initialize ps4 without update. This worked for me. Hope it does for others, gl. But again the 1st ps4 after pulling my hair out for 2 days I took it to a repair shop and they installed a new HD.
I had this problem 2 weeks ago. I tried updating with thumbdrive and reformatting to no avail. Went and bought a new hard drive now it works. Remember to always back up your saved games on the cloud or thumbdrive. I didn't have that foresight. But after the new hard drive it works again. All hard drives eventually wear out so be careful.

This is caused by two or three things. 1. Corrupted Update 2. Hard drive is going out. 3. Faulty powersupply causing updates installation to mess up sticking you in a boot loop. The best thing you can do is replace the hard drive and redownload the firmware update and hope it works.

Hi everyone! if your PS4 is doing this it means your hard drive is dead, all you have to do is replace the hard drive with a new one, install the factory reset update and it will work again. I replaced my son's PS4 hard drive with a 250GB one and it came back to life. remember though!! you have to download the factory reset file to reset your ps4 to factory settings, then go so safe again and choose the last option, good luck.
If your PlayStation won’t exit safe mode or says can’t start after multiple attempts of restart or intialization replace hard drive, I replace my ps4 drive with a broken ps3 I pulled the drive out and swapped in, then download the system files off PlayStation.com not the 400mb file go lower and find the 992mb file and download it onto a flash drive, I went 6 months with a broken ps4 that wouldn’t restart prior to doing this
Ghost of Tsuhsima is a glitchy mess for me right now. Either the characters themselves will freeze during the cutscene, or their lips will be moving with no audio and the subtitles will be gone as well. I'm on a PS4 Pro 4K HDR... I tried reinstalling the game, that didn't work. I tried Rebuilding my Database through Safe Mode, that didn't work. I tried prioritizing Performance over Higher Resolution, that didn't work. I tried to Initialize my console, that didn't work. Hopefully a patch is released by next week...doubtful tho. This issue was also very present when I played The Last of Us 2. I've never had this difficult of time with any games until 2020...*sign...what an overall crappy year so far.

Was stuck in safe mode. What worked for was leaving the the ps4 alone when it asks to connect the controller via usb(20-30 mins) then doing a initialization! Without that it would just restart and turn on and off completely. Hope it helps some

So before you guys do option 6 on the safe mode menu and have to reinstall everything on your system try what worked for me: 1. Boot in safe mode 2. Install the update via internet 3. Your ps4 will turn off and not restart as usual if you are having this problem, so when this happens turn it on but boot it in safe mode again 4. Select the option in safe mode to restart your ps4 which will bring up the error screen but this time you should be able to hit ok and then let your ps4 do its thing. It will restart and should work this time :)
MrThrasher00 you need to download an update called the lodge its like 900 gb something like that abd you need to make a file in your USB called PS4 then in the PS4 folder make an UPDATE folder and the update goes in there

Hello to everyone, i found the way to fix it: You need to did a low level format to your disk. You need to connect the HDD of your ps4 to your PC (with a usb/sata adapter). I downloaded the programm "HDD low level format tool". Will take about 3 hours to format all disk (500GB disk); this process will erase all data. After that you need to initialize the disk with "Disk Management" tool in Windows, assing a letter (d:, e:, f: or something) an reformat in exFAT. When you finish with those steps, extract the disk and put in on the PS4. Plug your pendrive with the complete PS4 system ( NOTE: THE SIZE OF THE COMPLETE SYSTEM IS 1 GB, NOT THE 400 MB OF THE UPDATE). And do all the steps shown in this video.

@@belloumirafik6314 it's currently not working. I think I might have to buy a new console. I installed the OS with a USB storage stick, erases everything , and after downloading it all again, it will work until it goes into rest mode. Then it craps out.
No it hasn't. It takes some fiddling but it'll let you factory reset it. Although I had to do the update twice now. For some reason my system said it wasn't updated so I had to do the reinstall process again.

Ok this happened last month with the 5.00 update, and I had to reset the whole thing like you guys are talking about. So I did that and used USB stick with all the updates, got it working and was all good. That is until this new 5.01 or whatever update came out 2 days ago, now it's happening all over again. So for all you others having this problem, this is what I read in the forum's, and what I feared. Apparently, if you did this once, and it happens again not long after you think you fixed it, most likely your harddisk is shot or on its way out. So now I guess I have to A. Replace said hardisk or B. Get a new ps4. So good luck to anyone else having this problem.

My ps4 has been hit with so many power outages, that anytime it happens which it did last night, my ps4 does this and must be factory reset everytime. It also WILL NOT accept a flash drive or online update so yes i have to delete everything....This doesnt make me mad, what makes me mad, is that i didn't get to upload my NEW saved data for my games before this happened to the PSPLUS cloud and now i lost literally HUNDREDS of hours of my life. Theres nothing i can do either cause these power outages happen at random since i live in Florida so its honestly just annoying as hell
Okay guys if you have this problem take these steps - > 1. DON'T DO ANYTHING Stupid leave ur ps4 like this 2. Grab a USB with more then 1gb space on it and ur pc 3. Go to Playstation. Com and download the 1gb version update make sure u got nothing on ur USB 4. Once it's downloaded plug it in to ur ps4 and go to the USB device on ur ps4 and update it 5. U won't lose any data on ur ps4 And ur good to go again

Finally got into my menus. I hope it want crash from ps4 menu (not safe mode). I have version 6.02 1. Try getting your HDD out and putting it back in into PS4 2. Make sure you have USB flash drive in it with the version you need (in my case 6.02) 3. Turn on your TV and put it into HDMI, your PS4 should be off at that time. 4. Now plug HDMI also into PS4, then plug power cable into PS4. 5. Hold power button until second beep 6. Go for option 6 (Initialize PS4) 7. Now your PS4 will boot up into menu, if not try again couple times, if that doesn't work repeat it again until you get into menu again 8. Go into Settings -> System -> Turn off option (Enable HDCP) 9. If PS4 crashes in menu again, you should probably get a new HDD

I have the same issue after upgrading to 3.5" HDD 3TB I found that the disk is just not starting fast enough for the PS4 and it pops up a message of "Can't start ps4" as if there is not disk connected. If I reboot it from this point, the HDD is already spinning and the boot is successful although it requires a physical connection of one ps4 controller. The solution for now is to auto put the ps4 in Rest mode. The disk never stops and the ps4 starts .
My PlayStation hasn't worked in a month now I initialized it and it worked but then an update for a game came along and it just kept stopping at 4% and then saying cannot download and so I turned my PS4 off like completely and turned it back on and it went to this screen, I've tried everything the update it told me, another update everything and nothing is working so i initialized it again and it's still not working it just kept showing the PS4 symbol then going black and that's where it stayed then it just shuts off

nerd i have the same issues i already fix turn off your ps4 and turn on press and hold 7 sec until u heard beep sound twice and press controller u see 7 options and just do the rebuild data base on option 5 do not use option 6 or 7 hope it helps
You need to name the folder on the flash drive like this: PS4 / UPDATE / ... Then put the downloaded file. And everything will work! But not only UPDATE / ... - its error!
Same thing going on for me but i didnt use usb. I plugged direct to the internet and tried doing update. It did update just like this video only to bring me back to the safe mode etc. So then tried all the other options and no luck. Before this it used to say it couldnt read harddrive error. I have the same problem. identical. I also have such a pocket for a larger hard drive. when I disable consoles incorrectly, I have to do PS4 initiation then delete all data but the system starts up. I can still reinstall the software but it also deletes all data from the console. I have it from the 5.00 software

*THIS IS HOW YOU CAN "FIX" IT* If the storage update stuck at 0-25% if u get black screens after re-trying many times to update your ps4 or do anything alse, try to change the HDD the only way to test it out if ur ps4 is dead or u just fucked up the HDD. (happens when HDD is corrupted can even brake and make weird noises (click, click) from corruption) if you cut the power instant or something similar happen. I had this exact fault! I changed the hard drive and worked perfectly afterwards! 18-09-2017 tried everything for months!

I've heard of ppl contacting the certain game to get back their lost purchased items but I never looked into that I just assumed they would only retrieve lost purchased items only for big UA-camrs

Jeremy Woods bro i try to update my software but with my usb stick and the files that they wanted but later on they tell me that they cannot acess system storage and when i try to just initialise my ps4 it loads but then when its done my ps4 stays on a black screen and the ps4 button is blue and never goes off it be like he is breathing u know but never going off.. do u know something i could try?
I like how everyone says opinion 6 deletes everything but when I did opinion 7 nothing was deleted it all went to library and nothing was uninstalled
So before you guys do option 6 on the safe mode menu and have to reinstall everything on your system try what worked for me:
1. Boot in safe mode
2. Install the update via internet
3. Your ps4 will turn off and not restart as usual if you are having this problem, so when this happens turn it on but boot it in safe mode again
4. Select the option in safe mode to restart your ps4 which will bring up the error screen but this time you should be able to hit ok and then let your ps4 do its thing. It will restart and should work this time :)

MrThrasher00 you need to download an update called the lodge its like 900 gb something like that abd you need to make a file in your USB called PS4 then in the PS4 folder make an UPDATE folder and the update goes in there
MrThrasher00 you need the PlayStation on add mode as well and you need to press the bottom one which will reset your PS4 but at least your os4 will be working

Hello to everyone, i found the way to fix it: You need to did a low level format to your disk. You need to connect the HDD of your ps4 to your PC (with a usb/sata adapter). I downloaded the programm "HDD low level format tool". Will take about 3 hours to format all disk (500GB disk); this process will erase all data. After that you need to initialize the disk with "Disk Management" tool in Windows, assing a letter (d:, e:, f: or something) an reformat in exFAT. When you finish with those steps, extract the disk and put in on the PS4. Plug your pendrive with the complete PS4 system ( NOTE: THE SIZE OF THE COMPLETE SYSTEM IS 1 GB, NOT THE 400 MB OF THE UPDATE). And do all the steps shown in this video.

@@belloumirafik6314 it's currently not working. I think I might have to buy a new console. I installed the OS with a USB storage stick, erases everything , and after downloading it all again, it will work until it goes into rest mode. Then it craps out.

No it hasn't. It takes some fiddling but it'll let you factory reset it. Although I had to do the update twice now. For some reason my system said it wasn't updated so I had to do the reinstall process again.
Although from what I am seeing is it is caused by a HDD issue.

Ok this happened last month with the 5.00 update, and I had to reset the whole thing like you guys are talking about. So I did that and used USB stick with all the updates, got it working and was all good. That is until this new 5.01 or whatever update came out 2 days ago, now it's happening all over again. So for all you others having this problem, this is what I read in the forum's, and what I feared. Apparently, if you did this once, and it happens again not long after you think you fixed it, most likely your harddisk is shot or on its way out. So now I guess I have to A. Replace said hardisk or B. Get a new ps4. So good luck to anyone else having this problem.

My ps4 has been hit with so many power outages, that anytime it happens which it did last night, my ps4 does this and must be factory reset everytime. It also WILL NOT accept a flash drive or online update so yes i have to delete everything....This doesnt make me mad, what makes me mad, is that i didn't get to upload my NEW saved data for my games before this happened to the PSPLUS cloud and now i lost literally HUNDREDS of hours of my life. Theres nothing i can do either cause these power outages happen at random since i live in Florida so its honestly just annoying as hell

1. DON'T DO ANYTHING Stupid leave ur ps4 like this
2. Grab a USB with more then 1gb space on it and ur pc
3. Go to Playstation. Com and download the 1gb version update make sure u got nothing on ur USB
4. Once it's downloaded plug it in to ur ps4 and go to the USB device on ur ps4 and update it
5. U won't lose any data on ur ps4
And ur good to go again

Finally got into my menus. I hope it want crash from ps4 menu (not safe mode).
I have version 6.02
1. Try getting your HDD out and putting it back in into PS4
2. Make sure you have USB flash drive in it with the version you need (in my case 6.02)
3. Turn on your TV and put it into HDMI, your PS4 should be off at that time.
4. Now plug HDMI also into PS4, then plug power cable into PS4.
5. Hold power button until second beep
6. Go for option 6 (Initialize PS4)
7. Now your PS4 will boot up into menu, if not try again couple times, if that doesn't work repeat it again until you get into menu again
8. Go into Settings -> System -> Turn off option (Enable HDCP)
9. If PS4 crashes in menu again, you should probably get a new HDD

I have the same issue after upgrading to 3.5" HDD 3TB
I found that the disk is just not starting fast enough for the PS4 and it pops up a message of "Can't start ps4" as if there is not disk connected.
If I reboot it from this point, the HDD is already spinning and the boot is successful although it requires a physical connection of one ps4 controller.
The solution for now is to auto put the ps4 in Rest mode. The disk never stops and the ps4 starts .

My PlayStation hasn't worked in a month now I initialized it and it worked but then an update for a game came along and it just kept stopping at 4% and then saying cannot download and so I turned my PS4 off like completely and turned it back on and it went to this screen, I've tried everything the update it told me, another update everything and nothing is working so i initialized it again and it's still not working it just kept showing the PS4 symbol then going black and that's where it stayed then it just shuts off

You need to name the folder on the flash drive like this: PS4 / UPDATE / ... Then put the downloaded file. And everything will work! But not only UPDATE / ... - its error!

Same thing going on for me but i didnt use usb. I plugged direct to the internet and tried doing update. It did update just like this video only to bring me back to the safe mode etc. So then tried all the other options and no luck. Before this it used to say it couldnt read harddrive error. If the storage update stuck at 0-25% if u get black screens after re-trying many times to update your ps4 or do anything alse, try to change the HDD the only way to test it out if ur ps4 is dead or u just fucked up the HDD. (happens when HDD is corrupted can even brake and make weird noises (click, click) from corruption) if you cut the power instant or something similar happen.
