When you return to Kuldahar at a certain part in the story, you have to liberate it from Orogs. This song plays in the Enhanced Edition, never did for me in the original.

Ohh EE plays it. That would explain it. I haven't done that far in the EE yet.

It played in the original IWD too, but it only ever plays once in the game, as Tyler Durden says, when you return to Kuldahar after retrieving the Heartstone Gem. It MAY possibly also be coded as the "combat music" for the Kuldahar map, so attacking NPCs in Kuldahar might trigger it to start playing (I've never tested that out though.)
