Estradiol in Men: Myths and Facts

    Testosterone is the precursor hormone for estradiol. Estradiol is a hormone more abundant in women than men that is produced by the aromatization of testosterone in liver, fat and other cells. Nature created it for a reason. It has been shown to be responsible for healthy bone density but its role in men's sex drive, body composition and other variables is source of great debate.
    Many anti-aging or men's health clinics prescribe anastrozole, a blocker of estradiol production, to men who start testosterone replacement (TRT). Higher estradiol blood levels not only can cause breast tissue growth (gynecomastia) but also water retention (edema). Some people speculate that high estradiol can also lead to erectile dysfunction but no scientific papers have been published on this subject. Since higher testosterone blood levels can originate higher estradiol levels, the belief is that using anastrozole will prevent breast tissue growth and erectile dysfunction by lowering any potential increase in estradiol. However, we have no data on how high is too high when it comes to this hormone in men. Some even speculate that low testosterone-to-estradiol ratios may be more closely correlated to gynecomastia and erectile problems than estradiol alone.
    The truth about these speculations is starting to emerge but we still do not have enough data to say what the upper value of the optimal range of estradiol really is. We have a lot of evidence about the lower side of the optimal range since it has been found that estradiol blood levels below 10-20 pg/ml can increase bone loss in men. A recently published study also nicely demonstrated that low estradiol can be associated with higher fat mass and lower sexual function in men. So, be very careful when a clinic wants to put you on this drug without first justifying its use.
    Another concerning fact is that many clinics may be using the wrong estradiol test that may be over-estimating the levels of this hormone in men. An ultrasensitive estradiol test more accurately measures estradiol in men instead of the regular test that costs less.
    Fortunately, most men on TRT do not develop gynecomastia even without using anastrozole (gynecomastia is common in bodybuilders who may use high doses of testosterone, however). Those that have gynecomastia at TRT doses (100-200 mg of injectable testosterone or 5-10 grams of testosterone gel per day) may be genetically predisposed to having more aromatase activity or have liver dysfunction. Treating all men who start TRT with anastrozole from the start may be counterproductive since this may lower estradiol to very low levels. Some physicians monitor estradiol blood levels after 6-8 weeks of having a man start TRT alone using the ultrasensitive estradiol test to determine if anastrozole use is warranted. Doses range from 0.25 mg per week to some clinics using excessive doses of 1 mg three times per week. After 4-6 weeks on anastrozole, its dose can be adjusted to ensure than estradiol is not under 20 pg/ml. Fortunately, many men on TRT do not need anastrozole at all. www.excelmale....

    i felll into the you need an AI on TRT. I kept my E2 very low for to long. low E2 gave me high BP, ED and no sex drive. now i dont take an AI and all is good

      @excelmale doctor im bodybuilder i never been on testo but i had nipple sensitivity that make big issues for my chest estetics so i went to chek my estrogen levels i find that i have 39.70 pg/ml what should i do !!

      dutch dragon chances are he's not going to reply to you but I did had recently check on my estradiol level and I may be able to give you some insight order to remedy to your problem. Most likely you didn't know how to use your anti aromatase inhibitor and that cause the low estrogen levels.If that's the case stop using it immediately so your estradiol levels can go back to normal.Depends on what kind of steroid using and how long you been on your cycle determines your estrogen and your prolactin levels. you need to monitor your estrogen by the frequent blood tests. I would also add proviron to your regimen and a little bit of Clomid say out of 30 days or a month you do one week of 50 milligram a day Clomid.And once your estrogen level goes back to normal you have to keep monitoring it. Also advisable to use a little bit of HCG say 500 International unit twice a week for 4 to 6 weeks that would also help to raise your estrogen a little bit.

      peter csigo bro thanks but u didint understand my case i have high estrogen as i see from the video cause i have 39.70 pg/ml i never been on steroids am natural bodybuilder but i had recently problem in my nipples it becomes sensitive and sometimes its so not shreded as before so i readed that it could be related to high estrogen the doctor said in his video that more than 37.30 pg/ml is high level of estrogen and that the best level is between 25 and 30 pg-ml so i wonder what i do !!!

      dutch dragon hey man I'm sorry I misunderstood what you were asking however I don't think you have high estrogen you have a low estrogen because I'm holding my lab result and the reference range is between 60-190 pg/mL.

    Hi Thanks for this valuable video. I recently started TRT (T was ~200 ng/dl and E was 11 pg/ml). My T dose is 100 mg/week. My doc also advised me to take Anastrozole 1 pill per week. However once I take the pill i felt horrible with headache and more fatigue. Can it be because of my E has gone further down now? My E was anyways low. Or something else going on here .. . any suggestions/thoughts pls.

    Taking aromasin for 3 months made me gain 25 pounds of fat, complete limp dick, complete loss of muscle pump and crushing brain fog. Worst part no doctor or endocrinologist I spoke to had any idea why aromasin would do this to a man.

    Hi Nelson. I love your videos. I am on TRT replacement. I went from .50 a week to 1.00mg a week. I split the injections to keep my test level consistent. I was on .50 for 7 weeks and was retested. My initial estradiol went from 30 to 48. I am not sure if I need to be on an AI just yet. I have no real symptoms as of yet only that my face feels warm and I have had some minor acne come out just on my face. I will be retested in 4 weeks and was wondering if my estradiol increases, should I consider a AI? Thx

      You probably mean .5 to 1 cc. The only reliable estradiol test is the sensitive one done using the LC/MS assay (Labcorp or Quest have it). I would not use an AI at your estradiol level. Most guys are crashing their estradiol with too much anastrozole. We need estradiol for many purposes. Here is the test and some additional info:
