You can have 2 pistons firing at once and time it like a 4 cylinder. It wouldn't be a fast, but it would have a lot of torque. And you can keep out the gear, i think there's just too much power loss there. You can also adjust the timing so it fires both cylinders at the end before working inwards, it might help keep the forces on the crank more even, which i think could be beneficial since it's so long.
