Hey, I just had a question about treatment. If the patient has cardiomyopathy with crackles, edema and a bp of 90/60, would you request an order for furosemide? I love your videos so much, so informative :))
Great question! I would definitely be cautious about the administration of furosemide with this borderline BP as the med can cause hypotension. Checking the patients' albumin is helpful and sometimes IV albumin given before a diuretic helps to pull the fluid from the extravascular (lungs and LEs) back into the intravascular space by oncotic pressure. This usually helps elevate the BP enough to then be able to administer the furosemide safely and diurese the patient. Keep a close eye on the BP after you give these meds and monitor urine output closely. As always, patient care scenarios vary and individual health conditions need to be taken into consideration. Hope this helps!
Thank you so much. You explain everything so well!
