A lot of those 'perfect' reaction shots are spliced in from different jokes to the one we're hearing, so the people sitting around the woman wetting herself with laughter won't have just heard the same thing we have. It's never the same atmosphere as being there live, in the room. The camera can lie. Micky's a top comic, so everybody there will have been having a good time - but not necessarily when the joke we're hearing was told. ;-)
