Before they got hit we passed out. That's why we're inside limbo and that's also why during our loop we hear about an accident happening. The sister and brother are stuck in this limbo with you. The reason they say "it's not your time yet" is because we aren't dead, we're close to and passed out. So instead we saved the little boy and sister from limbo and gave them peace by dropping them off the train. Allowing their spirits to be free. The entities on the train are likely demons who also reside in Limbo. The poor old man seems to sadly be trapped inside. He likely died of old age on the train and can't find his way out. Whatever the demon was that chased you was likely the thing keeping people inside limbo or stuck in whatever this place was, but you freed them.
