I’m an EMT. We don’t give mouth to mouth because the dangers of catching a disease is not worth the minuscule benefit of breaths. CPR is leagues more important than breath.
In new CPR guidelines, mouth to mouth is not required as chest compressions are more important than the time it takes to administer that breath. It is still recommended that mouth to mouth happens if there are two people doing CPR, but they specifically sell a mask for this reason, because one downsides of the process is people have a habit of throwing up while being saved. Besides that, there is also the need to avoid diseases. Another downside is that ribs have a habit of breaking during CPR due to the amount of force that is used to keep your heart going. If you're lucky, you'll just get away with bruises.
