You should to complete all four Strike Force Missions. Mason and Karma must stay alive. Menendez doesn't need to be killed at the end. I don't remember exactly what it is, but you need to find some documents or files in the missions. You get the best possible ending by completing all the points.

Black Ops did better at portraying the gray-ness of war than the reboots of MW by making the good guys do bad things, same with Adler who brainwashed and killed Bell in Cold War.
