Hi Tony ...Medial rotation and Lateral rotation are not restricted in side arm position. Just flex your right elbow to 90 degree and do slight GH extension and take humerus at the back and try to touch your contralateral pelvis ASIS. You can figure out there is enough Internal rotation at GH joint.
The thing of restriction can be felt even at abduction position. If you keep your elbow straight in arm abduction and then do IR and ER you will feel that same restriction as felt during straight side arm position.
    Kindly share your views please..

Hi, good to hear from you.
      1. Side arm position here refers to one particular context and not the one like you mentioned, side arm refers to the one as I demonstrated thats arm held in close approximation to body. Either the arm( refers to region from shoulder to elbow is in contact with trunk. At this context the ROM is restricted due to approximation and contact between soft tissue masses( muscles and other tissues).
2.The eg. stated by you takes arm away from body and cannot be thus referred to as side arm position or at least cannot be consider for particular example above.
      3. Your eg. is so nice , I appreciate ur thoughts but during the movement above reaching out pelvis its not just the GH joint ROM that plays but contribution are made by all 4 joints of shoulder as well as contribution by elbow and distal parts.
4.The eg for shoulder abduction , I cannot agree with I m sorry its abduction at 90 degree were the rotation is maximum more over the coracoacromial arch opens up( for ur understanding simplified) and frees the ROM at abduction. I would suggest you try 90 degree abduction with elbow and wrist all straight or the neutral u can feel the difference, once you move on to lateral rotation at above position you can literally sense the head of humerus getting free to move and greater range of LR over MR.
      5. To understand the difference I would suggest you keep ur arm close to body elbow at 90( max close approximation) and try the MR & LR, Then try at 45 abduction, 90 degree abduction and 180 degree abduction ( without bothering about other joints). I am sure you will feel the difference.
      6. At present I am looking into book which you asked by mail, if I figure out will let you know. Kindly excuse for note replying to mail
      Good luck to u
The next question I will answer tmrw, bit busy wth works.
      Take Care
