  • Identifies self by name. Identifies resident by name.
    Washes hands.
    Explains procedure to resident, speaking clearly, slowly, and directly. Maintains face-to-face contact whenever possible.
    Provides privacy.
    Adjusts the bed to lowest position so that feet are flat on the floor. Locks bed wheels. Puts non-skid footwear on resident and fastens. Places gait belt around resident’s waist over clothing.
    Stands in front of and faces resident with feet shoulder-width apart.
    Grasps belt and bends knees, while assisting resident to stand. Checks for dizziness.
    Places cane in strong hand. Places cane 6 inches in front, steps out with weak side, then strong side. Helps as needed with ambulation with cane, walking slightly behind or on the weak side of resident.
    Watches for obstacles in the resident’s path.
    Lets the resident set the pace, encouraging rest as necessary.
    Returns resident to bed or a chair. Removes gait belt. Positions resident comfortably and checks alignment. Leaves bed in lowest position.
    Places call light within resident’s reach.
    Washes hands.
    Documents procedure.
