You're supposed to lock the handcuffs after you get it on the person so that the cuff size stays giving the person enough room. (Obviously for cops, they need it tight on the criminal so it removes any possibility of escape. But keeping it tight for non-criminals would cause pain.) In less serious reasons for handcuffs, it can be loose and locking the cuff in place prevents harm to the skin/body. On the handcuff key, there's a reason why on the opposite side of the key there's a random stick/poker poking out. That's to lock the cuff.
