Thank you for explaining proper method for culture collection. As a clinical pharmacist in hospital with a special emphasis on infectious disease this was a constant battle. Especially with decubitis ulcers and culturing of drainage. It's easier to pull your own wisdom teeth than to get a surgeon to do a simple punch biopsy of the wound margin.
I have watched doctors grab a swab for a sample to be tested as soon as they get an opening. When I starting watching your videos I was perplexed by your methods but now understand why you wait to get a sample. Makes very good sense. Happy New Year 😊❤

I see so many (experienced and newcomers) doctors cut into an abscess then immediately shove a q-tip in to get a culture 🤦♀️ Its great to see one do it the correct way! I also like that you explain the reasoning behind the how-to; not alot of people know about the acid/base shift and its effect on numbing or why you don't culture the initial pus. I see so many doctors and PAs collecting pus as soon as a wound is opened and sending it off for testing. On occasion they get lucky and there is microbial growth but oftentimes as you noted, nothing grows or just normal flora from the skin grows. I don't understand why this isn't being included in the BASICS and why CE doesn't seem to get them properly trained. Although, they could be choosing CE courses that don't include microbial growth and proper collection techniques. It's extremely frustrating to watch and know the samples taken and sent to the lab were taken incorrectly.
I was shown proper sampling technique quite late in my residency, by a fairly angry pathologist, lol, but the lesson stuck. I'm not sure why it is taught so poorly?

Really enjoyed watching this. Took me back to my days as a Medical Social Worker, in teaching hospitals, sitting in rounds or being able to overhear teaching moments like this, between the attending physcians and the medical students, interns and residents. It was precious seeing the looks on their faces, when the teaching physician would pose the questions, knowing that the student really knows the answer, rationale, the student responding correctly and the teaching physician confirms. Yhe dmile, the puffing out of the chest...PREVIOUS. Then the teaching physician asks another, more specific question, the answer to which the student might not be that familiar, and the answer and rationale is the same and you see that lightbulb go on, above their already swollen heads... PRECIOUS.
Great procedure!! You are one of the few doctors that flush out the area after the cyst removal. I think this is good for proper treatment and healing. Wonder why so many doctors don’t do this? Your videos are very well done and certainly educational. Thank you so much.
Sorry, I disagree. I have seen similar and bigger cysts treated in a fraction of the time with much betterr results. For 7:48 the first 5 minutes she was jušt playing.

It’s amazing to watch these videos, comparing the right way of doing this and…The Wrong Way (At home, no gloves, no lubrication, no sterile field…yikes!) I really like how this Dr speaks to his trainee. No irritation or aggravation, and explaining it as he went. I’ve worked with a bunch of drs who were bored with the world and just “over it” - I’d never train under one of them!

Thanks for another lesson. This gentleman is going to be feeling much better. That looked pretty inflamed. If i have a tiny bump on my face it's annoying. A massive cyst with all that inflamed tissue and pressure would be pretty unbearable.
I love the assistants that dont seem afraid! They just go right in! Also the drs dont have to tell them what to do or when, they just go for it... I guess it comes more with experience.... However I love a good assistant...
The comment you made about 1% of these can be cured through antibiotic cream resonated with me. I had a big one like this in a very unfortunate area (i'm a woman) and could barely walk because the pain was so intense... I went to the ER to have the doctor tell me it could not be drained and gave me an antibiotic cream... I literally cried walking out there, felt so helpless... The next morning i was starting the first day at my job, the pain was so unbearable that i had to drain it myself, fortunately i watch these kind of videos to know to use sterile instruments and gloves, but it was very hard with no local anesthesia... The relief i felt was the best feeling ever. All this to say, thanks for taking your patients' pain in consideration, and helping them have that relief they surely need.
