I vote for the garage. Also you are not alone, we had to put my grandfather into elderly care because he has Alzheimer's. It becomes a serious problem in our modern world because people live longer but they aren't living better if they have these diseases and the system is underfunded so we can't do anything without help and/or money.
Option 4 seems the more logical, for a young guy is not a problem getting up the stairs and let's face it either the garden or the garage regardless how good might be insulated will be cold and damp in winter.

The garage seems safer and more livable. I've seen plenty of the garage conversions locally
My parents died before they reached their 60’s so I don’t have experience with a decision like this but I would probably move in with her as her roommate since it will be temporary versus spending all that money to convert spaces. You’ll still see your wife and kids, it just won’t be 24/7, for now, treat it like you’re sequestered on a special ops assignment.
That’s what I’ve been doing the past year or so. It’s been tough. I’m going to flip the script and bring her into our house
Does it make sense to put her in care locally? I don't know cost or availability. The lack of mobility is tough. If she has to go to your place the garage certainly looks like the best option. When the inevitable happens it also provides a spot for one of your teens to go
