There are septal perforators from both LAD supplying the anterior 2 /3rd and Posterior descending artery from RCA supplying the posterior one third of inter ventricular septum or IVS. The most important of these septal branches is the 1st septal artery. On cigarette or biri smoking or thaing gutkha, pan masala, khaini or zarda over a long time or developing diabetes by taking lots of rice or wheat products or sweets, atherosclerotic constrictions develop in these arteries causing ischemia.
