    Update. The protagonist has a dream about his future, his dream warns him of what's going to happen to him when he turns old and gray. He doesn't know what this means, but he gets a brief reflection of his future self. He wakes up, It's 1920. Surely this was just a dream, right?

    I think this was a patient, the old man is the protagonist’s body as we see him deny the fact his memories are slipping. The ending is when terminal lucidity happens. The protagonist rests easy with some of his memories intact. When he disappears, that means he is now dead.

      Dementia* also you can only die of dementia by having internal suppressed immune system problems, the dementia just adds to it making it worse. But you can’t die from it. (No hate btw!)

    It seems like he's coming back from war based on what he's wearing if so then I would like to say that after the surviving soldiers came back from the world wars they would come off the train and for some have trouble finding their family because they couldn't recognize them. I watched a movie made in Germany about WW2 (or one, it's been a year) and the aftermath it followed a few different groups of characters but there was this soldier who came back home and ran to his family and embraced his daughter calling her his wife's name.
    I found the movie, it's called "Das Wunder von Bern" (The Wonder from Bern or The Miracle of Bern). I 100% percent suggest it but warning, the father hits his son and he also cooks the boy's rabbit.

    From your portrayal it seems the protagonist is the patient's soul/mind trapped in the deteriorating train. That's why he look shocked after the portrait is blurry(unrecognizable) I do like it.

    My great grandfather was a railway driver during the late 1930's around London, he saw the blitz with his own eyes, retired in the 1960's, i never got to know him, my mum always said about how hard his dementia was for her, apparently the only thing he remembers are trains, and didn't know why, just a little story I'd like to share.
