In terms of hospice care, it's for end of life treatment. It's not like a nursing home at all. I work in the UK and here, its run by palliative care consultants and specialist teams. In very bad cases where symptoms are not manageable at home such as nonstop vomiting/pain/unfixable blockages/impending catastrophic bleeding, this is where the hospice takes over. Think of it as the ICU for end of life. Once their symptoms are more manageable, they are discharged back to the comforts of their home. In most cases, if not all, especially for the middle/upper class, patients would have had a decision regarding their end of life care planned out in advance... so it's not actually up to their next relative. It's a high service and I imagine even in private companies, alot of goodwill is involved. Also not to be confused with euthanasia 'pulling the plug'
