Should a candidate use abbreviations such as DM, if he/ she is writing to a home care nurse?

Abbreviations should not be used too much, but can be used occasionally as long as the reader would understand them, for example, you could use ADLs in a letter from a nurse to a nurse.

Do we still need to write full name in the introduction paragraph as we already mentioned in Re: ,or just surname only? ( l am writing regarding your resident, Mr Smith?)
