Funnily enough they still do this today i had it done as a child due to a bleed on the brain it's called trepanning or bore hole surgery. They drilled two holes in my skull to release the pressure buildup on my brain. Funnily enough though one of the symptoms prior to finding out about the bleed was actually excruciating headaches that occurred every night for about a week before i started showing stroke like symptoms and went to the hospital to get it checked out. It actually worked wonders i was essentially a cabbage before the surgery because of the bleed and couldn't move and could barely talk but when i woke up i felt perfectly fine so yeah it might be a little wild but it works.

Moreover, were the patient truly were suffering an intracranial hemorrhage, the doctor would need to be able to find the bleed to maximize the treatment's effect, something that's a lot more difficult without modern medical imaging tech. Lastly, in the oldest variants of the surgery (when it was tied more closely to religion/ritual than to medicine, the two being a bit blurred in our prehistory), there was no attempt to close the hole in the skull, so only the skin would regrow (after a considerable time), leaving the brain far more vulnerable to trauma. P.S. any claims that the oldest variants of the surgery were meant to treat one-off headaches are unsubstantiated; in truth it's far more likely there were some medical undertones to what was more or less a religious ritual. it may have been thought to help with headaches at that time but that would not always have been the primary reason for going under the "knife". in a number of areas of the ancient world (on every continent but Antarctica in fact) this procedure was commonplace, with dig sites revealing in some cases 30% of skulls having been "trepanned", despite estimated survival rates of 40-80%. this fits better with a religious or coming-of-age ritual that was also used in medical emergencies, than it does with a purely medical procedure. nonetheless, at least in Europe, by the time of Hippocrates the procedure was already being used strictly for serious neurological issues, not one-off headaches, and by the 10th century had survival rates approaching 90%. so if as a time traveler you made it back to the 15th century, and came down with a little bit of the brain bleed sniffles, you might just want to take your chances under the "knife" (i.e. drill). P.P.S some real geniuses in the modern day still do this surgery outside the medical context for purposes like "opening their third eye", "oxygenating the brain", and "returning to a child-like level of neuroplasticity".
Trepanning would actually release a lot of pressure on the brain, which led them to believing it worked. Even back in medieval times, the surgery was successful, as can be seen by signs of healing on the skulls of those who had underwent the procedure.

Yes. In fact certain "headaches" (brain bleeds) are still treated in exactly the same way, and there's evidence the procedure is at least 10 thousand years old (being the oldest surgery for which we have evidence). The problems with doing this in the 1500s (or earlier) are obviously b) lack of disinfectant and anesthetic leading to complications, but more importantly a) lack of diagnosis. While headaches are certainly a symptom of intracranial hemorrhage, the vast majority of headaches arise for much more mundane reasons, and would not have been improved by relieving pressure in the brain. In fact it's far more likely that migraines or cluster headaches, chronic conditions, would have prompted patients to undergo this (unhelpful) treatment, as opposed to true intracranial hemorrhages, which are an acute condition that rapidly incapacitates sufferers. Moreover, were the patient truly were suffering an intracranial hemorrhage, the doctor would need to be able to find the bleed to maximize the treatment's effect, something that's a lot more difficult without modern medical imaging tech. Lastly, in the oldest variants of the surgery (when it was tied more closely to religion/ritual than to medicine, the two being a bit blurred in our prehistory), there was no attempt to close the hole in the skull, so only the skin would regrow (after a considerable time), leaving the brain far more vulnerable to trauma.
P.S. any claims that the oldest variants of the surgery were meant to treat one-off headaches are unsubstantiated; in truth it's far more likely there were some medical undertones to what was more or less a religious ritual. it may have been thought to help with headaches at that time but that would not always have been the primary reason for going under the "knife". in a number of areas of the ancient world (on every continent but Antarctica in fact) this procedure was commonplace, with dig sites revealing in some cases 30% of skulls having been "trepanned", despite estimated survival rates of 40-80%. this fits better with a religious or coming-of-age ritual that was also used in medical emergencies, than it does with a purely medical procedure. nonetheless, at least in Europe, by the time of Hippocrates the procedure was already being used strictly for serious neurological issues, not one-off headaches, and by the 10th century had survival rates approaching 90%. so if as a time traveler you made it back to the 15th century, and came down with a little bit of the brain bleed sniffles, you might just want to take your chances under the "knife" (i.e. drill).
P.P.S some real geniuses in the modern day still do this surgery outside the medical context for purposes like "opening their third eye", "oxygenating the brain", and "returning to a child-like level of neuroplasticity".

Trepanning would actually release a lot of pressure on the brain, which led them to believing it worked. Even back in medieval times, the surgery was successful, as can be seen by signs of healing on the skulls of those who had underwent the procedure.
