    Philadelphia crash…I have 12000+ hours flight time. Kevin, you’re absolutely right about instrument flying in the clouds. Very easy to get what’s called “spatial disorientation” after takeoff climbing into the clouds if you don’t focus on trusting your instruments. Might take… The Learjet crew took off, entered the clouds and the flying pilot got disoriented and realized he was starting to descend so kept maximum power (take off power) and pitched the nose up trying to stop the descent. The problem is he’s disoriented and doesn’t realize that by pulling up he’s already in a bank and that further makes the bank get worse. It’s called spiral death dive. And ends up coming out of the clouds doing over 11,000 ft./min. descent rate and it’s too late by then. Trusting your instruments in the clouds is critical if you feel disoriented good training emphasizes that.

      Is the Bank meter reliable? or can it be offset by centrifugal force? If it's reliable, why was it not relied upon? In old flight games, I always relied on that because the sky was just plain blue.

      Plane crash Philadelphia, 0 time flight time, but 1 million hours of common sense.
      The clouds ceiling was at 400 to 600 ft at the time of the crash. The plane was above the clouds in the first 10 seconds of the flight.
      It was over 1400ft peaked at 1650ft for the next 28 seconds of its flight. Then in the last 2.7 seconds of its flight it dropped all the way to the ground and never lost air speed throughout the flight.
      Spatial disorientation? He was above the low 600ft clouds for most of the flight . If he was at 1650ft ,he was about 900ft above the clouds for most of the flight
      Don't need instruments for that.
      Try again,it was not spatial disorientation, it was not a fire in the plane, ,because there was none, it was not ice ,it was 48 degrees ,and it was not a stall, thats obvious.
      Knowing the facts I just told you ,do you have a different opinion?

    These are all excellent points that I agree with 💯, Kevin. I took my first flight over 25 years ago, so its wild to see some of the same issues I noticed back then are still a problem today. I also remember being pushed to go into ATC back then due to the staffing shortage...
    Anyway, I hope you continue to use your influence to help push for a change.

    I'm a bottom line kind of guy and the altitude emphasis doesn't matter very much. It does seem the helicopter was too high for the route but the main problem was they misidentified the CRJ700 thinking the further away aircraft was it. I've flown a CRJ700 into DCA on that exact arrival and runway and it is definitely a challenge. In that complex environment I don't believe the tower controller should have passed traffic separation responsibility to the UH60 crew. Instead, they should have been given a maintain present position instruction or at least an instruction to fly a specific heading to de-conflict the traffic.
    In addition, I've trained many military helicopter pilots for their fixed wing ratings and though they are well trained many are used to flying in restricted airspace and on specific routes and they will tell you they are very busy and not looking outside much. I would also add that we don't know the helicopter pilot's recency of experience in general and/or recent night flight experience. This can create a HUGE problem.
    Years ago I was in a military training event where we were flying an outbound route on tower frequency and had to pass over a specific point at a specific altitude. We had an inbound route over the same point on a different approach frequency so you had no situational awareness usually gained by listening to radio communications because you had two different frequencies. There was only 300ft separation built into the routes. I approached the point on altitude and switched to approach only to immediately get a traffic alert "patterns have merged" call from approach. I knew where to look and the inbound aircraft was Not at the proper altitude as it was an inexperienced pilot. When completing the near miss report I estimated the vertical separation was 15-30ft. These things happen more often than we would like and the ATC system is supposed to make the difference and help save lives.
    @Kevin - Try HARD to get some actual IMC time with an instructor as you really don't want to find out how much you have "cheated" flying with foggles or hood. Years ago I was instructing a USAF Lt. Colonel in instrument flight with him wearing foggles. We hit actual IMC in the busy Denver Class B airspace and he lost control of the aircraft within 10 seconds of being in the clouds. I executed the unusual attitude recovery and we went on up into Wyoming in visual conditions. Then on the way back the exact same thing happened shortly after we hit the clouds. We had a serious discussion when we got back to the airport. Be Safe!

    They call it the Swiss Cheese Model... several things went wrong here all contributing to the crash. If we are trying to blame the helicopter for only being off by 100 feet... we have created a system that leaves no margin for error.
    1. Too complex of airspace
    2. Misidentified aircraft (maybe requiring adsb in/out in every aircraft)
    3. Military vs Civilian systems/training/frequencies

      @@sanahaskuranage8071 The ATC calls makes it more then plausible that the blackhawk identified the other CRJ 3 miles away instead of the CRJ they hit.

    Kevin, great video. I’m at a legacy airline in the states, only saying that to give credence to my opinion. Your analysis of the DCA event was excellent. I really appreciate you pulling out the sectionals and breaking it down for everyone. I do however strongly disagree with your 5 minute autopilot speech. As a pilot you HAVE to be able to manually handle the aircraft in any situation (actual IFR should not be that big of a deal). You’re right about actual is greater than foggles but through your flying career I hope that you hone your handling skills to the point where AP is not needed in any situation, it is just a tool to help reduce workload, not a crutch. In training you should feel comfortable working on these skills because you are sitting next to (hopefully) an experienced pilot that can correct as needed. But then….you will become that experienced pilot and you will be the one needing to correct as necessary for your co-pilot or if you’re single pilot. AP is great SPRM but you HAVE to be confident behind the controls manually flying and the only way you get there is practicing hand flying.

      Yeah, this is a great point. I stick fly almost exclusively except in cruise to practice - I should have clarified. Train no-AP, train to know how to use AP, and AP w/ passengers / non training. Thanks so much for your comment; I totally agree with you!

    Hey man. I know you keep saying it's your opinion and you're a recent pilot... but as a private pilot myself with only a little over 100 hours, I have to say I find your comments about ADS-B requirements very dangerous. It's not possible to "demand" that everything in the air is emitting ADS-B. There's plenty in the air besides airplanes... there are hot air balloons, paragliders, parachuting, etc. In VFR, you have to *see and avoid*. Relying on an ipad too much would be more dangerous than keeping your eyes outside.

    Damn good take. Especially on fogging and icing situations. I got about 500 hours on flight simulator and the first time i flew through clouds it feels like at anytime there could be something in front of you like a mountain

    There’s a big difference between private pilot training and commercial pilot training by the airlines. Airline pilots train hours and hours in simulators for different types of weather conditions and aircraft faults.

    I took my driver's Ed course in winter and took my driver's examination also in winter. I believe if you train during the worst season you are more prepared then others. I feel more comfortable then others during rain, snow or at night driving probably due to this.

    9:00 - The fact that radar data rounds to the nearest 100 feet is honestly shocking. At lower altitudes, that’s a massive margin of error. Shouldn’t precision be tighter, especially near airports?

    I saw the view from the airport, which two guys were arrested for lifting the video off the airport surveillance film. It clearly shows the helicopter tracking down the airplane , they were at equal heights, not under/above like first released. This clearly looked intentional. CNN released that film, so it’s out there

    Be careful being a newer experienced pilot Kevin. Between 200 hours and 1000 hours is the most dangerous time to be a pilot. You know just enough to get yourself hurt out there.
    That military helicopter pilot doing a check ride was a 500 hour pilot.
    Scariest thing I ever put myself in was freezing rain in the clouds with no de icing equipment in a Cessna 172. Found another altitude to get out of that situation .
    Another scary situation thru no fault of my own that’s similar to this was flying in the clouds and ATC telling me another airplane was near me on a head on collision. But ATC wasn’t talking to them because they were suppose to be VFR. So I’m over here wondering if this guy is in the clouds as well but shouldn’t be there. I asked for a higher altitude to resolve the collision alert

      There will most likely be new rules written into the FAR/AIM because of this incident just like every other rule in Aviation has been written in blood from previous incidents and accidents

    Kevin, your opinion that every plane should have an autopilot may be ok for instrument training. For basic VFR, which is 95% of what I do, I like to fly the entire time. The planes I have don’t have autopilots or even IFR rated GPS. I fly VFR 100% of the time and I’m fine with it.

    Kevin, for the Philly crash especially. The total flight time was under 1 minute. The press conferences are just a bunch of thanking everyone for doing their jobs. Is it silly of me to think that the NTSB, who has had in their possession for 5 days now, the CVR and GSPW, maybe they could rule things out without painting themselves into a corner? Also, living in Philly, there have been people on the local news stating that a body and others had body parts removed from the roof of their houses during the initial evacuation. With that impact and explosion, is it plausible that those found were passengers on the plane? I would have assumed that those poor people would have been "evaporated" in an explosion like that? I'm just not sure that the number of 7 fatalities holds water with the amount of devastation witnessed immediately after the crash.

    From what I've read, planes with a T-tail design (i.e., having elevators high-up on top of the horizontal stabilizer) are inherently more susceptible to stalling as soon as the angle of attack gets too large, compared to traditional tail design. No idea whether T-tail was a factor in this particular accident though.

    This Loss of Separation agreement that the FAA uses may work at airports whose airspace is not as crowded. But this is what they use to facilitate VFR traffic movement without having to take responsibility for it. So they’re basically trusting that VFR aircraft to maintain separation and that separation is “ don’t share any paint.” Unfortunately we’re not building any airports anymore and the popular airports are getting more crowded and under staffed. A good recipe for accidents

    Repeating other aircraft’s tail number is ridiculous. Not everyone uses an ipad to fly around with. As an airline pilot, I never know other aircraft’s tail numbers

    It is my understanding the radar does not determine altitude. It is the job of your transponder to transmit your altitude to the radar. Multiple transponder malfunctions across many aircraft, only add to this problem. Also low time pilot. Adsb is not equipped on all aircraft and not required at all airports. The airports that do require offer exemptions. Spot on about the training. Instructor's are not the pros. They are still learning themselves.

    Not every airplane is equipped the same, even the same models. Not every autopilot is the same. Not all traffic is on ads-b. Having your eyes inside too much is a bad thing. Too much automation and reliance on it is a bad thing. Not every student is independently wealthy training in bling-bling Cirrus and DAs. Not every flight school is rich. Good instructors... like retired military and airline pilots are hard to come by.

    It’s not the military that should scare you. It’s the guys that dont make ctaf calls or have any transponder. The military has transponders and we follow faa law based on the class airport

      @ Class D also. The 60 in the dc crash was in a class B so they had to have mode C on by law. ATC can see their altitude

    Kevin, just would like to say the 'areas of error' at this commercial airport, with 2, if not 3, planes coming in for landing and likely would have done so safely if Tower had redirected that TRAINING military helicopter. I think the issue was with tower's conversation with helicopter, and now all those people so close to ending their trip, are all dead. Hopefully, the policy/practices of the military at this public/commercial airport can and soon will be changed. Now, carry on with what you have to say.

    The ATC gave vague instructions. Should’ve given a reference point to where the CRJ was. Now if the chopper was on a mission other than the one stated then there’s nothing tower could’ve done.

      They did. The first call was when the CRJ was south of the Wilson bridge. The heli called the traffic in sight. The second call was right before the impact and the controller was confirming they still had the plane in the sight. Don’t get it confused that the “do you have the CRJ in sight” was the first call. It was the follow up call.

      @ I heard both calls and they were vague. This may be why we had the crash. If the helicopter was advised where the CRJ was or the helicopter was told to hold positions. Then things may be different.

      @@andrewmartz7076 Just a follow up. They may have been looking at the wrong jet. At the Wilson Bridge they weren’t into their turn. I went back to listen again. Also, we know the helio was too high. At the Wilson Bridge the CRJ should’ve been near 2-3 o’clock to PAT25. They were in cockpit windshield. As PAT25 started to right the CRJ was 12 o’clock just after the bridge. Night vision goggles that bad or is this something else?

    Same applies to Drivers licenses. I guess they give out pilot licenses out like they do DL's, the only difference is there's a big money barrier when it comes to pilot licenses. How many people trained in the parkinglot then took the driving test and then they go driving on the open roads with the rest of the idiots on he road and take out a family of 5 with one mistake.

      They did…there seems to have been a misidentification of the plane that it crashed with..there was a 3rd plane that had taken off and they suggest that is the plane the helicopter referenced that they saw

    Plane crash Philadelphia, 0 time flight time, but 1 million hours of common sense.
    The clouds ceiling was at 400 to 600 ft at the time of the crash. The plane was above the clouds in the first 10 seconds of the flight.
    It was over 1400ft peaked at 1650ft for the next 28 seconds of its flight. Then in the last 2.7 seconds of its flight it dropped all the way to the ground and never lost air speed throughout the flight.
    Spatial disorientation? He was above the low 600ft clouds for most of the flight . If he was at 1650ft ,he was about 900ft above the clouds for most of the flight
    Don't need instruments for that.
    Try again,it was not spatial disorientation, it was not a fire in the plane, ,because there was none, it was not ice ,it was 48 degrees ,and it was not a stall, thats obvious, the plane can fly with one engine fully loaded easily it was not a weight issue, he was supposed to make a right turn ,he never did, why would he put on the auto pilot?
    Did i miss something?
    Knowing the facts I just told you ,do you have a different opinion?
