Getting out of Trouble (captions and interview removed)

Captions and interviews have been removed from this version to use for training and discussion.
This is a reconstruction of an actual incident where a helicopter malfunction forced the pilot to divert to an unfamiliar airfield. Autopilot mode confusion during the glidescope capture results in loss of control a breakdown in collective situation awareness.
Decision making and review, automation, communication, monitoring and intervention are all identified as contributory factors.
