It doesn’t matter usually. They spell it the way in the video but people in certain places with always pronounce with 儿
Stupid question: is there a difference between tong and teng?
Well, they're interchangeable in most cases. If someone feels hurt on the body, he/she can say "body part+ 疼/痛“. But when someone tries to describe feelings, they, sometimes, have different meanings. For example "你让我心痛“ means "you made my heart feel painful", but "你让我心疼” means “you made me feel bad for you (showing sympathy)".
