Mannitol, a sugar alcohol is an osmotic diuretic that increases plasma osmolarity resulting in the flow of water down its concentration gradient from the intracellular space to the plasma. so, Mannitol induced expanded extracellular volume can exacerbate heart failure.

i believe because mannitol is an osmotic diuretic it is drawing water out of the tissues such as brain and eyeballs to reduce tissue swelling. This water is put into the bloodstream which is then circulated throughout the body. Because this fluid dump can be significant, in some cases it can actually cause heart failure due to the volume of fluid. Even if it doesn’t cause heart failure this fluid can be dumped into the peripheral limbs and the bases of the lungs. Hope this helped, I am in my senior year of nursing school so correct me if i’m wrong!
