    I regularly train many veterinary ophthalmologist on cataract surgery especially Phaco, as you mentioned the importance on operating room set up is something our young beginners should concentrate. Great sharing Thanks

    Im a vet surgeon and have been doing cataract surgeries on animals since last few years. Your tips on microscope are very useful. and other videos also on phaco are also very useful as we dont have any training centres for us here in india but learned a lot from from videos.

    Thanks for video Sir nicely mentioned practical difficulties n solution before starting the surgery… while sitting on temporal side am not able to put legs freely under table , while focusing table obstructing my legs .. kindly mention how to avoid it

    thank u very much for very nice video..sir what about presbyopic glasses?? does it req to be used while doing surgery...in otherwise emmetropic eye??

      You dont need presbyopic spects to look through microscope, but you may need if you wish to see whats there on the assistant trolley

    Thanks for the video Sir, One doubt- In the patient position section the eye of patient is almost reaching lower end of sternum, shouldn't it be somewhere around umbilicus level for good and comfortable posture?

      You can lower head end of ot table. Can ask patient to fold knees which makes the position more comfortable

    Sir I have - 0.75 cylinder in one eye other eye is emmetrope, i am not comfortable with both with glasses and without glasses, but without glasses depth perception is not good, what best I can do operate comfortably.

      Check your IPD and convergence capacity with synaptophore or RAF ruler. It may be because of convergence insufficiency

      Also if you are stressed during surgery your accommodation may increase and can lead to headache or blurry vision.

    Very useful video sir, tilting lower end microscopes may loose their coaxial component leading to poor depth perception so it's not recommend for bigginers..

    Sir, nice video....I usually operate on IPD 69 with glass on as I am comfortable with that but my IPD on autoref and manually is 65.....then why I need 69 IPD in microscope... thanks for reply

      Sometimes the eyepieces are set wrongly so actual distance may be different than what is noted on the ipd scale of microscope. Thats very common with older microscopes or if the eye piece assembly was opened at some point of time
      Other thing is eyes are generally more comfortable in slight divergence, particularly if you have mild convergence insufficiency.
      Newer higher end microscopes also have broader field of view through eye pieces (larger diameter) so that allows some flexibility in ipd ..so even if you have set ipd +/- 2 mm you may be able to do surgery.. but its best to have perfect ipd set

      @@SourabhPatwardhanPhacoTips sir, that means myopes and increasing age surgeons will set higher IPD as compared to IPD measured on autoref or manually as these surgeons has convergence weaknesses

      Not necessarily amit. The difference is not huge in normal persons.normal surgeons continue to have good convergence. convergence weakness is disorder and can be treated.

    I am not an Ophthalmologist but have started to use surgical microscope in my practice recently. No matter how much I try to adjust my IPD Im unable to get a single vision. Is it due to inexperience or refractive error or something else??

    I m a first year PG in Ophthalmology
    I am not getting binocular single vision in my wet lab practice. I checked my ipd its 62mm and working on same bt still not getting it... i m a myopic either with glasses or lenses i m not getting it sir.
    Really stressed please help me out sir??

      Change microscope... Most wet lab microscopes are pathetic..

    Sometimes the Instrument trolley gets stuck to the Phaco footswitch preventing full pressing of phaco pedal. That is also one thing to take care of. Specially during Right eye surgery

    Sir when ever I sit on microscope there is always difference between clearity on screen n under microscope..whatever i focused under microscope that's comfortable for me that's not show clear on screen. Ipd set gud .Why this happens not getting understand.pls help

      1) you can try focusing using adapter of camera after you have focused under microscope at higher zoom
      2) you might be accommodating, thats why focus keeps changing

      Helps in adjusting according to height of surgeon. If i was taller i would have kept it more vertical

      You are already a tall surgeon. I'm bothered because such a costly microscope doesn't give perfect ergonomics to all height surgeons... Zeiss tiltable is more than 180 so can actually look up and operate without tilting objective....

      :-) for good ergonomics the neck position should be straight upright. If it is flexed or extended, surgeon will get Musculoskeletal problems in long run. Since i operate for long hours ,i dont want to keep my neck extended or flexed. I maintain primary position. If i had low height table, i might be able to keep objective straight, but unfortunately with wheels its height is a bit more than what i wish.
      Also with height,surgeons may have slightly shorter upper body even if they are tall, so it varies from person to person. The tilt function is for adapting for that. I have Leica m822 as well, it also needs some adjustments like this.
      I am using invertetube here, so you will find that its a bit longer than regular 0-180 , but i have adjusted to that too. 😉
