  • This is my FIRST vlog about my FIRST day as a resident doctor! But it’s not a regular day to start… it’s a 26-hour call shift in the hospital!! See me before and after this crazy day.

    Freshly graduated from medical school in Canada, I’m excited (but also a bit terrified) for this huge transition. In this vlog I chat about my expectations, fears, what it’s really like on call with glamorous scrubs and pagers dating back to the 90’s!

    Watching this now after just seeing your video where you receive your results for your final exam and you are officially a rheumatology specialist!! After 5 years of residency it's so exciting and how cool to look back at this!! Explaining what a pager is and how a call shift works seems so crazy now!!

I am pre med and so many doctors i have talked to have told me not to do it, that I’ll regret it and i should find a different career path. 😢 seeing your videos has made me realize, it’s totally in perspective and you have to be doing it for the right reasons!

      Hospitals, often multistory and concrete, as well as filled with motors and noisy electronics, make it difficult to set up communication systems that are reliable.As Violin points out elsewhere, pagers penetrate deep inside buildings where cell phones fail.
      There are practical reasons for that. Pager systems got their channels decades earlier, so they have lower frequencies that penetrate better. They are also more powerful. One pager channel can cover thousands of users, so they can run much higher power, typically many hundreds of watts. A typical cell phone system needs to reuse spectrum every few miles, so it runs 30 watts or less. As a result, pagers are still necessary for must-get-through messaging when a cell phone might fail. Also, it doesn't take much of an emergency to swamp a cellular system since almost everyone today carries one. Paging is more robust and most people don't carry one.
      Her pager seems to offer little or no text content. Some allow for a brief message much like phone messaging.
      The real heroes of hospital communication are the telephone operators who must know where everyone is all the time.

      I can't go into it in detail here, but doctors and nurses should be skeptical of the moves afoot to bring more technology to bear. More tech is not necessarily good tech. The nasty term for it is metrics. It is the horrors of "Taylorism" (aka "scientific management") in long ago Ford factories brought to medicine.

      Here is what drives it: "Scientific management requires a high level of managerial control over employee work practices and entails a higher ratio of managerial workers to laborers than previous management methods. Such detail-oriented management may cause friction between workers and managers."
      Hospital administrators are making decisions about these systems and doing so in ways that are intended let them monitor staff and demand more productivity irrespective of actual conditions. Yes, they think you're being lazy and lolly-gagging at every opportunity.
      When I worked in a hospital, I got away with a heck of a lot that benefited my patients because administrators couldn't be peering over my shoulder. That era could be coming to an end-or at least will come to an end if doctors and nurses don't take action. Hospitals are likely to follow either of two paths.
      1. Administrators will get to indulge their urge to prioritize and track every task, noting time to completion. If staff don't enter every piddly little thing they do, they are treated as if they didn't do it. And if they do enter those tasks, they waste time entering, prioritizing, and checking them off. I worked with kids with leukemia. How do you assign a priority to bring a glass of water to a dying child? Even to suggest that is to talk bosh. And finally, it would encourage cheating. Want to make sure you complete tasks quickly? Don't enter them until you are about to do them.
      2. The alternative are systems that reduce wasted time and allow staff to work more efficiently. It'd be quite useful to have hospital-specific Siri-like apps that staff could use to set reminders. When we gave blood products to those kids with leukemia, I had to follow a specific protocol to check them for reactions. I could have saved time and reduced stress if I could have simply hit a lapel button and said, "Siri, start blood protocol for Jason in B-307, automatically triggering the appropriate reminders. The same is true of communication. Staff waste a lot of time getting in touch. Instant communications would be a big plus. Most of the time saved would be 30 seconds or less, but taken together that'd make a big difference.

    Liked your video! I've just woken up and It's my first day as foundation doctor in the UK (fresh out of med school!) and I'm on call as my first job! I'm terrified but also so excited of what's to come in my career. I'm making sure that I don't annoy any nurses when I'm working ha ha and like you said calling for my seniors when I have no idea what is going on. I'm so scared of my bleep/pager as well especially if someone is going into cardiac arrest! Wish me luck and I hope the best for your future as well doctor!

    I'm working 6 nights over Christmas this year and feeling pretty down about not being able to see family and all that. But after watching your videos I feel so much better !! You work longer hours and you're always so positive and happy. I need to just power through the next 6 nights!! Keep it up. You're very inspiring and a huge role model 💕💕

    As a doctor you will always be a student! Science is always changing n advancing as our knowledge does which is amazing! You worked really hard in med school n it’s not pretentious or anything you should feel ashamed of! Own it girl!!!! You are a DOCTOR n you did that! You’re a badass ❤️

    In regards to the pager: if it ain't broke, don't fix it...its an old system but it does work! Simple to use and you can use a computer to send a page, which is what we do at our hospital. :) cool video! Congrats on your first day!

    Wow - congratulations! Loved the bit about the pager - although the fact that it was not entirely unfamiliar was a bit disconcerting. Can't believe those are still in use. Good luck!

    I just came across your channel, but I've been a hospital volunteer for 8 years doing pretty much everything a volunteer does (I also have a pharmacy techician license and am BLS certified). The best parts of hospital life are 1. The staff is like a second family and 2. No two days are the same. Whenever appropriate, the staff has fun and it rubs off on the patient and visitors, which makes them feel better.

    So cool to find this video AFTER watching many of your other videos. We all start somewhere. I see your confidence now and it is admirable. I am going back to grad school to pursue a degree in Physical Therapy. And I'm nervous and excited and I want to freak out, but its important to enjoy the journey.     Here in the Philippines EVERYONE had a pager back in the 90s when Smartphones didn't exist. I knew they were used primarily in hospitals by doctors, but we just used them during my college days to communicate with our friends and family. I love your videso, by the way! New subbie here!

    Hello! Your videos are very informative, well shot, and concise. I would like to see, if you have time, a video describing good habits for study for a student at any point in the medical education journey for those pursuing a medical degree. I have only recently felt called to become a doctor, making me...pre-pre-med? While I’m letting the vision season into a goal, I’m looking to apply good habits where ever possible as soon as possible and I’d be interested to see your take on the subject. I used to work for a telco and my first job was actually answering the phone and typing in the message and sending it to someone's pager. Oh I also have Turner's syndrome.     QUESTION???? Just found your channel. Not in the medical field at all but very intrigued with your life. Just curious how do you stay healthy when doctors tell us how important it is to sleep a minimum of 8 hrs everyday. I read about how important it is for your liver and adrenal glands and so forth. Just can’t imagine the long term effects. Hoping you get some good healthy down time between shifts.

      Pretty late reply but from what I’ve seen in her videos she usually gets a couple hours of sleep in the hospital and then some proper rest when she heads back home so all in all it sounds like she gets at least a decent amount of sleep depending on how hectic things are.

    NO!!! Say “I’m Dr .....” and donit with confidence! You worked your ass off for that title and you made it! If anything this early in your career at a new job, saying your title is impressive! It will ease the patient’s mind because they know you’re confident and your superiors will respect you.

    How many years did it take you to become a doctor? I’m really curious to find out as I’m an RPN. The first time someone referred to me as a nurse I was like “what? me? no this feels strange yet awesome” haha. I do want to go for my bachelors at some point.

    Do you pick the colour of your scrubs and stethoscope? I've seen sites like MedElita who sell these med outfits in various colours

      Scrub colors are usually designated for specific levels, so a med student gets one, differe t kinds of nurses get others, so kn and so forth. Usually pediatrics get to pick it out

    Pagers is real old system. They were used commonly up to about 15 years ago or so. I hate to think what she thinks of someone who is 40 years old. They are more dependable then your stupid cell phone that depend on your quickly dead battery ect.

    I feel attacked lol I was born in 1978 (I had the old original Beeper that was square and looked like a garage door opener back in 1993-94.) 😂🤣😂 /j 🫶🏼
    So PS I’ve decided to start watching all your videos from the very beginning because 1.) Find the Canadian NHS interesting & 2.) You are so upbeat and happy and working in hospitals and some of the things you have to treat, & deal with and still be so happy and positive is astounding to me k I LOVE IT !!!

      haha yes! The first week I said "hi I'm Dr. Deshauer" a lot more... I think I was trying to get used to the sound of it! But now I've started saying "hi I'm Siobhan, the resident doctor"... what have you been doing??

    I heard somewhere hospitals still need to use pagers because if there is some sort of crazy disaster with cell signalling towers/networks going down then it is too risky for hospital staff not to be able to contact each other :) and they keep them so clunky for battery life purposes :D in case anybody else is wondering.. love your videos by the way :)

      Hi Jessica! mmm good point... and you can also page doctors at home during a disaster (they work outside the hospital... sometimes I wish they didn't when I'm at home hahaha 😂)

      But dont they need to use a phone to page them & then they'll have to use the phone to call whomever paged them back? So how would that factor into networks/phones not working properly if they use them to page/answer pages anyway 🤔

      @@seamuffin I meant like the alternative to pagers on the go would just be mobile phones! Which would be far too risky in case of network issues.

    Howdy! I’ve been on a ViolinMD kick all day! Seeing this really helps, as I’m a nervous junior in high school who would like to go into medicine in the future (pediatric oncology). Thanks for making this video, as it cleared up some future fears for residents and gave insight into how these first days go. Keep up the great content!

    One of my classmates was a concert pianist. I'm not sure he had ever even used a microscope and yet the senior professors on the admissions committee of a great medical school accepted him. He did just as well as everyone else and is having a successful career. One doesn't necessarily have to come to medicine through the sciences. I think it's more important to be VERY good at what you do.

    I feel like even though pagers are very old "fashion" but still every affective ?? My dad is a chemical engineer and work as a quality control "chief" in a salt company and he still uses one of those sometimes

      Oh interesting! Yes I agree they are definitely reliable - you receive pages in the parts of the hospital where you don't get any cell reception, and they are almost indestructible (no big screen to crack like an iPhone!)
