    I experienced a similar situation. TKA in July. Diligently worked hard at PT and home exercises but couldn't get beyond 80° flexion. So frustrating! I had a MUA in September. The surgeon was kind enough to take a picture of my knee while I was under anesthesia so that I could see the knee really did bend. It was very swollen after the MUA so it was a gradual progression. Only 90° during my first PT session (I had 3 weeks of daily PT sessions). I was prescribed a muscle relaxant to take before PT because at my one week follow up with the surgeon I was still struggling with tight muscles. I hover at 110-115° flexion now. Still at PT to work on strength but the knee feels so much more functional. I guess I'm sharing to say don't be discouraged. Everyone's experience is different. You'll get there-- it just takes some folks longer than others. It doesn't mean you're a failure. Your body is unique!

      Hi, I had a bilateral knee replacement w/manipulation 3 months later. It has been a year and weekly PT at first and now Pilates 4 to 5 times a week and I am now just getting to on my best days 120 degrees. So, keep working . I am now taking privates lessons for concentration on my muscles.

      I am 7 weeks After TKR and ha e less than 40•. Its excruciating doing PT sessions, I vomit always after. I am about to contact the doctor to ask for an MUA. I’m frustrated, hurt and with sciatica due to lack of strength on legs and using hips. Please someone can advise me? I started PT after 3 weeks post op

    Thank you for this video. I am sitting here in tears as I just returned from my 12 week TKR post op visit with my surgeon and am considering a MUA. PT can force my knee flexion to 110 and I have over worked it at home then underworked it at home but I am not getting it anywhere near 100 and it affects every day activity such as getting in and out of the car and getting dressed. So many friends who have undergone TKR have “done great” and been 100% at six weeks that I’ve just gone into a depression because I have never been a wimp! After viewing this video I still do not know if I am willing to risk this procedure since there are no guarantees

    This is exactly where I am. I am 6 weeks post op and I am not even at 90 degrees. I am not a wimp and I work hard, but it's just not coming. My surgeon wants to do a MUA and I have agreed, but I feel like somehow I failed. I did the knee replacement because I am active and I do hard workouts with guys half my age and I didn't do it to have limited mobility. I will do whatever I have to, but it still feels like I failed. Thanks for your channel and for all this information. It has been very helpful.

    Thank you so much for sharing this. Very, very helpful and reassuring advice. I'm at 6 weeks post-TKA and have been doing PT 2x/week and home workouts 3-4x/day and thought I was making good progress, but my surgeon is advocating for MUA if I don't get from 90 degrees to at least 110 degrees flexion in the next 2 weeks. I wish there had been been better communication from my doctor and home PT up front about more aggressive (stretch) goals in the first few weeks, as I thought I was doing fine. Yet it seems now that I should have been working much, much harder through the pain and inflammation of the first 3-4 weeks.

      It is so great to hear from other people who are facing the same challenges. When I told the patient from this video that I was sharing her story she said "I feel like I am the only one."
      I told her there are so many others who are facing these same frustrations.

    I can't tell you how much this video at 8:00 minutes made me cry. I guess I am a people pleaser, because after my Patella Fracture and going to physical therapy as prescribed and then having physical therapy people act like they were done and it was all my issues, such and it's in your mind etc. I have cried multiple times because I thought I was supposed to do more and I couldn't. I appreciate your video.

    For some folk, MUA would seem the only way. I considered it for extension but the surgeon said he got me to zero in the op. So i kept going on PT but I neglected the flexio to avoid even more swelling. Then, surprise surprise, trying different things (ex bike on no tension 3x a day for 5 min bursts) i just improved. Op was July 2020. Now i have zero extension and 138 flexion. It gets stiff sometimes but each week is easier. Keep at it & listen to Tony M. He kept me sane in the dark moments like no one else.

    Great video. I just had an MUA 7 weeks after my TKR. Went from 62 to 96 now 8 weeks in. Have to do PT 5 days a week for 2 weeks after the MUA, then back down to 3 days a week. Praying all here gets well. It's not easy, but we have to keep our head up 🙏🏾

    Three months after a knee revision surgery, I required an MUA. During recovery of the initial and revision surgeries, even though I was icing on a regular basis, I learned that my body was creating scar tissue faster than the swelling was coming down.
    I was terrified at the thought of doing an MUA, but once it was done, I was so happy to make progress from 90° to 110° within a week of the procedure.
    Once that scar tissue was broken up, I was able to make progress without excessive swelling. I realized how important it was for me to do this procedure. Additionally, I found that when I increased my range of motion, I wasn't as fatigued as I used to be.

      Your comment is the first to give me a little bit of encouragement along with Tony…at nine weeks at only 80° and going in for an MUA next Wednesday… So nervous about the whole process… The pain already is unbearable. I’m sure I’m going to have it after the MUA procedure I just pray I can keep up the work I’m just so afraid of pain… But if you made it, then there is hope, maybe I will too!

    Thank you so much for all your TKR video's. I am at 105, 112 with a push, and my surgeon wants me to do an MUA. I am genuinely happy about my progress (LTKR January, RTKR April) but I'm not comfortable on stairs, still need a rail. The example in this video mirrors my situation and I am hesitant to go through with the MUA in December. I trust my surgeon who says I have a touch of PSTD from the rigors of the surgeries and this won't be as physically challenging as the operations were. So, it's my decision to make, but it's a tough one.
    Tony, your videos were the best pre and post op tools I had. Thank you for donating your time and knowledge.

    Thanks for this video. I am that girl that is busting tail doing all the things and not getting what I was hoping for. My situation is unique in that the TKR was in mid 2018 and range of motion was still 120 a year later. After that I lost range of motion and then Covid got in the way of treatment. I had my manipulation in Dec 2022. Got 120 in surgery and on my own 115 most of the time. There is tightness in front of the knee and clicking behind the knee still. I had my other knee replaced may of 2023 and the result is fantastic. Range of motion about 130 ish (of course I am trying for more) but no pain at 9 weeks out, a bit of stiffness. Today I hit a wall of frustration at therapy. This video was so helpful in putting some perspective out there. Thank you!

    I realize this is an old video, but my sister got an MUA, and the doctor broke her leg. This obviously sat her back to scratch. That possibility is not mentioned on any TKR page. I also got a TKR after falling off my tricycle and smashing my knee.

    Thank you. I meet with the surgeon in a few days to address the second total knee replacement and your comments helped me to feel less crazy. First knee went very well; #2, not so much. It has been so painful that it has impacted my first knee.

    Tony, thank you so much for sharing your wisdom and opinions. Your patients are blessed! You were telling my story here - even the timeframe lines up. I have a great PT team - one especially who “gets” the mental/emotional part of the TKR journey. It has been very frustrating for me, as I went into surgery in very good condition - exercising and eating a healthy, whole food diet. I have realized that because of that, I had set my expectations high for a quick recovery. Oh, those expectations and how they can mess with you! I try to remind myself that what I have today is what I have to work with. Some days are better than others. Your comment about using a calendar, rather than a clock to map progress is so helpful for me. Progress, though slow, is still progress. Keep up the videos - I look forward to what you have to offer!

    This is exactly what is happening to me. Replacement in early June, now it’s the beginning of October- I’m at 100 degrees and have been “ stuck” since week 8. Am experiencing a pinch behind my knee and tightness in the back hamstring. I also feel like I’m doing everything I can. Manipulation has been talked about , this video has given me some questions to ask my surgeon before I proceed forward. Thank you

    I have frozen shoulder. My physiotherapist was Nurse Ratchet from One Flew Over the Cuckoo's Nest. When I got home I couldn't sleep or do the home exercises. Found a physical therapist in the UK and she said they used to do manipulation under anesthesia, but it screwed up too many people, tore tendons and ligaments and even broke arms. She said when she work people too hard they didn't do home exercises. Everything she said that she used to do she doesn't do anymore because it didn't work and that was everything my physical therapist was doing. They wrenched my arm over my head which only caused impingement, damage and more pain. Now I'm doing what she recommends. No overhead exercises until you have full exterior rotation. If you cannot rotate your arm then you cannot lift your arm above your head. Tried it, keep your elbow bent at 90 degrees and try to raise your arm over your head. It can't be done. I'm slowly gaining range of motion by doing only the feel-good exercises. Mostly just stretching. I've been in chronic pain since 1989, and this is the first time I've been pain free.

    I am so glad I have been watching your videos as I feel I have been neglected by my physio . You obviously have a different way of looking after your patients in the USA . I had my first partial LKR in July 2019 and RTKR in January 2020 . I am still struggling with both knees at the moment still in pain on lots of strong painkillers, and having trouble sleeping . My physio doesn’t seem to bother how much flexion I have . I am seeing my surgeon next week to see about my partial KR as having trouble on the non operated side . Thank you for support and wealth of information.

    I love all your videos. I had an MUA after my first TKA 3 years ago and it worked beautifully. I had 110 degrees (assisted) pre- and retain 127 deg (easy) today. Now, with my second TKA 11 weeks ago, I have been stuck for over 6 weeks at 100 (cold) and 115 after 1/2 hour of stretching. My knee seems to have a memory--I can stretch it and stretch it but can't get beyond 115, and then 5 minutes later it snaps back to 100. 115 for a lot of folks would be fine, but I am a competitive cyclist and I still can't get my foot around a pedal stroke without lifting my hip, and I can't walk downstairs without doing the same. I work my butt off and I understand how others feel when it's insinuated that I'm not working hard enough! And by the way, I had the same pinching at the posterior lateral part of the knee that your patient describes. I am going to try MUA for this new TKA and see how it goes. Having been through an MUA on the other side three years ago I have the gift of perspective: this stuff takes attention, persistence, and time! Keep at it and don't give up!! My question is about scar tissue: why is it that "most" people have a relaxing of scar tissue while others like me have scar tissue like a rubber band?

      Would have loved to understand what you said with the use of your 3- letter words. I’m not a doctor,but I am someone trying in get information on how to increase my range of motion to my knee post complete knee replacement by 6 months.

      @@haroldstreit245 Manipulation Under Anesthesia (MUA) is sometimes done to break up scar tissue and improve range of motion under anesthesia. The doctor forcibly bends your knee to release the scar tissue. Total knee arthroplasty (TKA) Is the total joint procedure itself. Since time has gone by, I can give an update:
      I had the MUA done on my new knee (surgery date August 31, 2020) about 10 weeks post operatively. The manipulation was somewhat a success, although I still couldn’t get my foot around a pedal stroke without some impingement from scar tissue. I tried the MUA again 13 months after surgery, and had much better success releasing scar tissue even after more than a year out from surgery. I am very satisfied with my range of motion now! Physical therapy only achieved so much because my scar tissue was elastic and wouldn’t release. Having said this, most people achieve satisfactory results through physical therapy alone. I think I just tend to be one of those that builds a lot of scar tissue.

    Thank you for this video. I had my knee replacement surgery on March of 2020. I had a manipulation last year as well. After one year follow up I still struggle with getting range of motion. I just had a second manipulation on June 2 of this year. I don’t know what more I can do. It’s depressing for me not being able to walk without a limp. I have a sister that has MS that I have always helped but since my initial surgery I have not been able to help my sister at all which breaks my heart. I need help ASAP

    Thanks for sharing this. I to was working every day for hours to get my knee straight. After months of hurting i just decided to rest. After taking a week off and pacing myself i am starting to see some results. I was just pushing to hard and to fast. This was advice from you. Thanks

    This really helped me! I am getting a manipulation next Tuesday! I have gotten up to 90 but they want me to get 120-130 comfortably! At times, my leg is so tight and stiff and sometimes frail feeling! It feels plastic to me! Apparently Iose 2 inches of muscle too from the surgery so walking is torture at times! I am nervous my bones will be broken but I do want to get better soon and be able to walk on my own again with no brace or wheelchair!! It does help to hear others have struggled and it's not just me!! Thanks a lot!!

    I fractured my patella on January 24, 2021. MUA done on April 13, 2021. I am at 88 degrees flexion.
    Going to P.T 3 times per week plus home exercises. I just want to be able to squat & crawl again. Getting back to long steep terrain hiking & backpacking again would be great, but I am pretty sure that those days are over. I cannot squat to set up a tent, & scared to risk a fall. I fell while stepping over my sister's dog gate to go into her kitchen. I landed smack down, direct hot to my patella on her travertine floor. Comminuted bone. I had a trauma surgeon. I cannot believe that this happened to me. My life is forever changed.

      @@janereinhardt4715 Hi Jane, I broke my patella back in May the first time, it stayed together and I didn't need surgery, but then 4 weeks later I tripped and landed on my braced knee flat footed and blew the patella up. Had to have surgery on June 22, my surgeon had me non weight baring and straight extension for 2 months because it wasn't healing the best. Now I am about 4 months from initial break and 3 months from surgery. I can only bend to about 60 degrees on my own and 74 with the help of the physical therapist. I feel so defeated, but have learned that this is a terrible injury and it does take time. We can do this!

I don't remember what I wrote in my original post but I fractured my patella on 1/24/21, had surgery on 1/28/21, then MUA surgery on 4/13/21 because I wasn't able to bend my leg to 90 degrees yet. I now have 98 to 99% range of motion. I can squat, but it hurts. Still very stiff, still swollen. I may have another surgery in December 2021 to remove my screws/pins. I will make my decision in October 2021. Good luck with your recovery!

    14 weeks from TKR and 3 weeks from a MUA and still have only 93 degrees of flexion. Knee is still swollen and painful from the TKR and MUA and therapy twice a week yet can’t get past 93 degrees. Frustrated yes I am but I know we are all different and sometimes that’s how the dice rolls. Hoping for better results as the swelling goes down. Thanks for the informative video.

    Fantastic advice wish we had better post op pt in the uk. we are very much left to our own devices. Having MUA next week 8weeks post TKR. I am an exercise instructor and feel absolultey at my wits end as I was so fit and flexible.

      Lori I relate so much to opening the door wide enough to get out LOL. I won't park in a spot unless I know I can open the door wide enough to get out, and then when someone parks to close to me after that, in my head I think well that's their fault LOL!

      I really get it. The MUA was absolutely worthless. Swelled to where it had been post op. Down from 90 degrees to 65. After 7 months at 85 with pain. Surgeon seems to think I’ll agree to further work, but no way. Worst surgery and decision of my life. I think I’m going to end up like you. Only, since I’m 74 now, in 9 years I most likely will be over pain forever. Can’t wait actually. Life now is half or less of what it should be in my final years. I worked up until this surgery, so no golden retirement for me. Spending my last dime trying to regain mobility as Medicrap is worthless.

      @@Rosary716 so sorry to hear this! I feel so thankful I’ve had more than normal muscle tissue, more than normal fat tissue too lol. But even with the low range of motion I can still get most places pretty comfortably. I just turned 44 and I’m seriously considering learning more about this and trying to help people because I feel like most surgeons and physical therapists have no idea on what needs to be done to rehabilitate. They think we should get back to normal like knee replacement and meniscus repair, but I feel like it is so much different!

    Interesting to hear that more able bodied people have same issue as me. I had my tkr 14 months ago. My first physio gave up once we got to 85 and flexion slowly decreased. I found another one recently and we got to 102 though he now believes that I won't improve anymore as due to polio I have no ham strings in that leg! It moved fine before tkr! Well I also get the pinching behind knee. I did feel one part 'pop' and went from 85 to 95. Maybe the other pinching part is a Bakers Cyst hiding in there! Who knows. Will persevere. Very informative site. Thanks Tony I will keep at the exercising in shorter times but more frequently.

    I’m 15 months postop, guess it’s too late for me. My surgeon won’t do MUA if your over 90, I was over that then, but haven’t got passed 112-115 since. So guess I’ll just keep on trying to improve my range at home and the gym.

    Thank you so much. Your video (and everyone’s comments) has really helped me mentally. I’m in fairly good shape (and quite flexible due to regular yoga and stretching) and had LTKA on 10/14/21. I have been consistently doing my PT and home exercises. At 6 weeks post-op, I’m stuck at about 80 degrees (bend) and 8 degrees (straight). Pre-op I could not straighten my knee fully after injuring it (so I knew I had an uphill battle). I’m having MUA in a couple weeks and am hoping for the best, but also know it will take some time.

    I was at 95 degrees for my first post MUA doctor visit, and now after 3 weeks I'm at 120 degrees knee flexion. I owe it all to my recumbent bike.

    Aug 31st 2023 having MUA. May 26th had second surgery ( 3mnths between). Acl and partial replacement. I just hope that no damage is done. I have 115° after vigourous help from physiotherapists. Basically 110° is where it is usually at. Been 13 weeks and for about a month is stayed at 110-115 , 3x week physio plus me at home. Im worried itll get damaged coz ive now had 3 knee ops and i cannot handle a 4th... heart issues, vein issues etc... i was active, working manual job and playing sport up until old acl failure got too much via osteo. But from being an idiot and smoking for so maby years im having issues that make ne not wanting anymore surgeries on knee.

    I’m in the same situation; just had my 2 nd manipulation and now just 105 flexion. It’s very discouraging and it’s tight I. The back of my knee. I was doing great the first 3 days but now I can hardly walk; maybe I overdid it. I see my Surgeon in a few days.

    Did anyone consider Baker's Cyst. Had Sam situation after bilateral TNR, and impingement behind knee and lateral tendon chaffing delayed full recovery to 160 deg flexion. BC does not have to be palpable to elicit pain and restriction.

      Absolutely true. Baker's Cyst or even just pinching of the soft tissue behind the knee may limit flexion initially.

    Tony had a partial knee in Dec 2019 had constant pain since and then had to have a total in June this year. Have just been told that I need manipulation in January as cannot get beyond 90 degrees. I have been back at the gym 5 times a week since the second week and have PT once a week. I do the bike exercises I can do 10KM on the cross trainer at a medium level under an hour and do weights so I am above average fitness for a male at 62 I am fit and carry on daily life its just moving beyond 90 degrees on the left knee. Going down steps are still one at a time. I do over heal and get keloid scars. Thanks for the video it gives me hope that movement will be coming beyond 90 degrees

    This was the worst part of the knee replacement the manual I was basically talked into. Which after I had to start over again and really didn’t think it helped at all. Just keep on working hard and be patient the rom will come if you do. The funny ( not really) part was my doctor insisted the manual would help my rom and when it didn’t all of the sudden he said “ well nobody will have the total rom after the manipulation.

      Sorry to hear that. Do you know what the ROM was while you were under anesthesia? I have had many patients get to 130-deg under then within 24 hours they can't get past 85 again. BUT I have also had many patients who couldn't get past 90 before MUA and after they were at 115 and improved from there.

    Thank you Tony....Very interesting. A month or so after my TKR in 6/2020 my surgeon told me if my knee did not bend anymore then what it was he was going to need to do a MUA, luckily for me with PT/ home exercises my knee was bending more so I did not have to go through it. Now it measured 116 & in time I am sure it will bend even more.. I love your video's and I am not comparing myself to others with my progress. I just need to continue exercising.

    I had this done 35+ years ago, the best thing I ever have done, medically. I was in a car accident head on on ice, I was on the outside of the spin and got a quite severe seal belt injury. For months I tried everything, nothing helped. My chiropractor did 5 MUA's in a row. I was sore for several weeks, but no back pain at all.

    I had this done October 13th doctor got me to 115 after nerve block wore off I've only gotten to 65 im 8 weeks post op I have excessive scar tissue

      Hi Tammy, you are a great example for the case that it might not be scar tissue. Since you were at 115 with the nerve block even once it wore off you should still be close to 115. The fact that you dropped back to 65 would indicate to me that there are other factors involved.
      The patient from my story is, in my opinion, limited by the pain and mechanical pinching. I don't believe it is a problem with scar tissue for her. But it is often blamed on scar tissue.

    I ruptured my quadricep tendon on april 24th, had surgery on april 27th, leg was hot swollen, red from the onset, spent 6 days i hospital asking to see anyone, for anti biotics as i believed it wasn't right, kept asking for them, i could not get any, i discharged myself, expected to be seen maybe a week later or 10 days, i got an appointment for staple removal and wound check on the 19th of may, i thought that is a long time, the leg was a 7/10 pain, it started discharging fluid around the 10th, i went to my local gp who said that was normal, i asked for anti biotics, the gp said they dont like us interfering, wouldnt even change the bandage, two days later i went back and said this isnt right it should be healing, its red hot agony, i was assured it was fine, got to the hospital on the 19th, straight away, thats infected, ofcourse it was, booked in for debridement which i got on the 21st of may, i thought all went well, discharged on the 26th, and its taken 4 plaster casts, with weekly and fortnightly wound checks for it to finally heal, it was a big incision, i got put in to a brace on the 13th of july, my leg has been in full extension since the 24th of april, ive tried bending the knee but its extremely tight, rock hard above the pateller, and just isn't working out. The surgeons and everyone else are passing the buck, these things happen, people often look for someone to blame when nobody is......hhhmmmmmmm 22 days left with a festering staph infection, nobody is to blame. Can anyone relate to this, because i keep reading about knee replacements, but nothing like my situation, am i ever going to get range of motion after 11 weeks on none, am i going to need manipulation under anaesthesia? Any advice would be helpful at the minute, everything that could go wrong has.

    Thank you I had my TKR on my left leg.I’ve not got broken tibia which is hurting my progress because I’m no weight bearing right now. I have three more weeks of NWB I sure hope I do nit hand manipulation therapy

    Well knowing asking so many others that had this sad lie knee replacement..this 90% success rate has to be a lie...more like 50/50. The pain after the antisteica was over the top depression, crying. Couldn't move leg just laying off the right my right leg, couldn't keep it straight at all. Moving ankle was it. So many want to walk and most of us don't... The hype of 2 months ur good is the 1st lie, range of motion is the 2nd lie, quality of life improvement is the 3rd lie. And to accept you will be less than half or bout the same before surgery is honestly 90% that's the truth. Doctors bury there mistakes, lawyers wear a tie to hide the foreskin... That's more truths than what they spew

      Why did you need a revision? Is that like a new knee replacement? Was there a mechanical problem with the first knee replacement or a structural or a muscular? Since Tony, so eloquently described the variations of where pain and trouble come from!

    I am at 8 weeks and the physical therapist says to do MUA and Dr said a few weeks ago no not until I am 6 months post op. I can only get 80 degrees. I had a blood clot that kept me in the hospital longer and they didnt do much physical therapy with me in the hospital only had me walking very little. I just don't understand why they wont help me and the Dr don't help me. I work hard doing my physical therapy exercises now. I cry and push myself as much as a I can. I just can't get any better.

    I am just past 4 weeks postop TKA & facing mua option as only getting to 40 degree bend with so much hard work and effort - I do not see anyone else with that low degree - doubled rom from 20 to 40 in 4 days and the now am stuck - 40 degrees! follow up appt monday next week - what questions do I need to be asking? could there be something else going on? - also, have had full ability to straighten from day 1, no issue at all there, it is with the bending - help

    Six weeks post OP Im at about 105 flexion and I also have this same pinching and pain when bending it's been frustrating this was a big problem before my Tkr.

    hi doctor..i had a distal femur operation last july 2024, but my first doctor didn't advise me that i should take my therapy as soon as possible aug 2024 whole month i undergo wound care service..my first therapy is sep 25 and on oct 3 would be my 5th session my concern is my knees are still stiff i think its around 50 to 60 degrees is its possible that i can bend more...although its kind a late to go therapy procedures thank you..or i need to go mui thank you

    Hello, can I ask a question? I had a TKR on 25th July 2022, on the 12th Sept 2022 I had an MUA. Here I am on the 12th Oct 2022 still with limited flexion, I think my physio has measured me at 82 deg max. At what point do I say the MUA hasn’t worked, or do I just stay with the physio and hope things eventually improve? My knee also swells up so quickly after limited physio workouts and it takes a day or two to calm down, during which time I cannot exercise. Ami right to let the swelling go down before continuing to do my physio?

    Thanks very much for those great videos I had a tkr one year now and am still experiencing pain and stiffness especially during the early hours of the morning I have difficulty sleeping am considering an Mua because it is becoming very frustrating for me please advise me accordingly

      It is pretty common to hear this. Once someone gets moving things seems to loosen up. You might consider trying therapy again. I see patients after a year to reassess their progress and make recommendations.

    Good evening Tony I have a question I went to physical therapy today I range to 85 doing my best in my work outs ND at home my therapist was checking my knee it looks good the only thing he see is when he was pressing on the side of my surgical knee he says I could be containg fluid he said it was Edema is that bad ..

    I had a total knee replacement on March 30th. I did well for up to two weeks then I started to get stiff and in a lot of pain, my range is getting worse. My Dr. Wants to do a total knee replacement on my left knee but, I don't feel like my right knee is where it needs to be yet. I have become so depressed after this and the pain is wearing me down. My Dr. told me after 6 weeks your knee won't get any better. Is this true? Sorry, for rambling I am just so overwhelmed and tired of the pain. I started this with two knees on bone and a bad back so, I am grateful for any relief.

    Surgery TKR 3/1. Here we are, 10 days after MUA. Surgeon an PT not happy with ROM. I’m 73, was in hospital 2 days, inpatient rehab 12 days. I’m thinking Covid keeping any advocates far from me resulted in sub par care. Long list, briefly I wasn’t given ice for knee, PT nearly non existent in tiny room. Pain was horrible with both procedures. This medical event has pushed me into old age. I tried and tried, I cried, I screamed, I was pushed nearly to passing out. Best ROM 90degrees. Surgeon got it to 110. Whoopee. After, they pushed me to 105 two sessions. Then got swelled to the point I couldn’t get my shorts on. It’s been the swelling that’s held me back. Isn’t there anything else for swelling? I hope swimming will help. If I never get better, I blame those first two weeks. I’ll live with that. I’m not getting hurt anymore. I talked until I’m blue with PT about pushing too hard. I still go, crying through it all. Does anyone get enough ROM on their own? No MUAs, no surgery on other knee. I’m so done bits May 10 and I’m where I was March 3.

      Maryland that is terrible! After I was told I could do therapy, 8 weeks locked in a brace because of a patella fracture, I was so excited to start moving my leg/knee, then after the first 3 PT appointments my knee was screaming at me, I went in the week after and regressed in ROM, they acted like it was my fault and I was like no it is inflamed. That's when I realized they might know much about my situation, which I am no PT or Surgeon, but I believe when you push too hard you create inflammation and more scar tissue. So I have decided I am going to try my own ways, not sure if it will work, but oh well, we will see. LOL

      @@TerraSpino Try reading up on the Mind/Body connection. There’s an app called Curable which deals with it. I’m now nearly 7 mos after and bending to 85 degree on a good day. 4th round of PT. Surgeon wants to cut of course but I’d rather not. I have a workbook “Unlearn Your Pain” and doing acupuncture, cupping, TENS, electronic stimulation, hydrotherapy and soon hypnotherapy. My other knee is bothering me, but I’ll never agree to surgery. Nerve pain is worst pain I know. This PT does not torture me. We have gained degrees of ROM without crying and screaming. Good luck. Knees are the worst. That’s why criminals shoot people in the knees. You must take charge and be your own advocate.

      @@Rosary716Thanks for the info! I totally agree! I will definitely look into unlearn your pain, I agree that much can be mentally helped in regards to pain. I am actually trying to use a pt machine called the X10 therapy. It looks like it may help, and I am really hoping so. I agree about doing everything possible to not have surgery to the knee, just seems like people have more issues than not. Thanks for the info! And let me know if you need anyone to commiserate with, such a crazy situation. I actually joined a patella fracture group on Facebook and it has helped me feel not so depressed about the entire situation, just knowing I am not the only one going through this terrible condition helps.

    I had my Total knee replacement it got infected they had to go back in and do a revision one month later I had manipulation under anesthesia. A month later and I am at 90 degrees I am very frustrated

    This is what I am afraid of ...I am PO day 13, and still am not even close to 90...60 or something, I hope when I get my staples out tomorrow it will improve, I also notice only women have any problems with this surgery in general.

    My LTKA was 6/24/20, and I worked as hard as I could at PT and at home. I had gotten to 118 flexion (assisted), but then it started going backwards at about 8 weeks. It went to 114, then 108 and stayed between 102 and 108 for a couple of weeks, so my physical therapist reached out to my surgeon and we decided I would have MUA at 11 weeks on 9/9/20. The surgeon said before the MUA, he could only get me to 90, but during it, he got me to 125. At PT the next day, I was pretty sore and swollen, so we didn't push very hard, but the 2nd day after MUA, I was able to get to 120 (assisted.) My therapist also added Graston Technique to the mix, and that seemed to help a lot. I still feel a bit tight, so we have added dry needling the last 2 sessions, and I believe that is helping tremendously. I can consistently get to 120 fairly easily now. I have scheduled a couple more PT sessions to get the dry needling work done because I would love to get to at least 125. I appreciate these videos, and I have added a lot of your tips into my home workout routine!

    I had the TKR in Oct 2019 then I had an arthroscopy in Nov 19 2020. Have done PT up until 3 months ago, my knee barely make it to 90, hurts all the time. When I try to bend it it feels like there is a tenis ball in the back of knee that wont let me do it. I am sooo frustrated.😞

    I had my bilateral TKR in Aug 2020. Had MUA done after 21/2 mths post op. My flexion is around 115 but can't achieve the zero extention probably 3 to 5. It's almost coming to 6 mths n I still experiencing stiff knees at my patella. I m currently doing exercises n home but felt frustrated bcos my friends recover between 2 to 3 mths.

    Hi Tony, I had LT TKR 12/11/2021. I had home therapy for 3 weeks, which was torture due to swelling and pain. I started outpatient therapy and I am now in 3rd week( 3x’s/week). I have advanced to knee flexion of 82 degrees , where I started at 55 degrees. I do exercises 3 times/day on my own, which includes bike riding, walking and stretching. I feel like I’m progressing, slowly but it’s working. I’m issue is that my surgeon has given me a time frame to improve within the next 2 weeks of I have to have MUA. I don’t like the pressure to have another surgery, especially since everyone progresses at a different rate. What advise can you provide me in this situation. I like my doctor and have confidence in his skill but I know my body and my limits. I feel that with continuing therapy I will gain flexion degrees.

    I'm nervous. Should I have the MUA? Almost 6 weeks post op. Was at 100 ROM, but now stuck at 92. I'm obsessing over this. Pt twice a week, plus doing it myself at home. Dr. Appt next week.     I’m 3 months post op my leg won’t straighten but my flexion is at 125. When I do the exercises to straighten my leg afterwards I hurt behind and outside area of my knee. I am expected to return to nursing with a bent knee. It won’t stay straight!

    I had fallen off a ladder breaking my femur bone and causing damage around the knee on July 29th. Had to have surgery to put a rod/repairs damage around the knee two days later; since that time I have had a struggle getting my range of motion back with flexing and walking normally. I praise the Lord that I did not, and still do not, have pain when I am not flexing it however I do have pain and a lot of tightness when I flex it. On one occasion flexing it I got physically sick flexing it it was so painful. I am flexing my knee 4 sets of 20 each day but have only gotten to 105 degrees on last test. Doctor is wanting to do MAU if I do not reach 120 degrees on my next visit to him on Nov 24th. I am so frustrated with my progress on flexing and frankly how rude the doctor has been at times to me. He even yelled at me because I told him I missed doing 2 days of an exercise flexing it he told me to do. I hope if he is ever a patient that he has a doctor who is more sympathetic to him than he has been to me.

    Hi Tony. I have written to you before from Puerto Rico. I’m one of those people that do everything and get nowhere. I am 7 months after revision surgery and still seeing a fantastic PT group. We have .achieved some more ROM but not quite what I feel I work for. Is MUA recommended after 7 months? Still feel very stiff and after therapy even more so. I loosen up as the day goes on and I move around. Frustrated, depressed and anxiety ridden. I start to see a psychiatrist today. I need help. Is there a point when is time to stop therapy? MUA? Wait months or years? Please I need something to hold on to!

    Tony thanks for all the information! I have a bakers cyst and wonder how that affects flexión. Surgeon keeps saying it will go away but it hasn’t yet. 11 weeks post op. Extension is fine. When flexing I get a sharp pain in the back of my knee right where the tibial nerve should be gliding past the fibia. My pt feels ”gunk” around the nerve and has been massaging it to try to break it up. After he stretches me and massages the nerve area I can get to 110 degrees but can’t achieve that at home.
    Would a cortisone shot help?

    Did you ever figure out what the "pinching behind the knee" was? I have my 7week followup tomorrow after MAKO TKR and have been struggling like crazy to even get 80-85 degrees of flexion. I have MASSIVE pressure and a feeling of something dull being jabbed into the back of my knee and I swear it feels like circulation or something is being cut off or restricted to the lower part of my leg. Talk of MUA is a thing right now and I'm afraid I'll go to my appointment and never find out what it is that's holding me back.
    Little Complicating Factor: Adhesions/Scar is highly likely as I got pretty bad COVID about three weeks Post-Op and was flat out for about two weeks, not getting therapy OR even able to do much in the way of exercise. These last couple weeks have been hard work, but NO progress. Zip, Zilch, Nada. Help!

    Hi Dr. I had quad tendon surgery 8 weeks ago, had surgery 4 days after I ruptured my quad tendon and started PT 3 weeks after the surgery. I’ve been in PT now for about six weeks but my doctor has had me locked in my brace for a bit to let the tendon heal.
    I’m at about 45 degrees of ROM in my 6 week of PT and my doctor is suggesting a manipulation if I can’t get more range in 4 weeks.
    Should I be concerned? I would think that with a quad tendon repair there’s a reason to really take it slow. Just wanted to your opinion

    Tony, My question for you is this. Prior to getting my knee replacement my flexion is 100. Is it unreasonable for me to hope to improve that number after my knee replacement, or is 100 going to be the max I can get to after my surgery?

      I would be curious to hear what your surgeon says. Normally my response is if you were limited before the surgery you will be limited after surgery. My hunch is YES, you will gain more than 100, but maybe not a full 130.

      I had tkr in early November and wasn’t getting much flexion (about 70 degrees) after 8 weeks. Prior to surgery, I had poor flexion and was walking around with a stiff knee bent in a locked position for about 1 & 1/2 years. My surgeon recently did a mua about a week ago and he was able to get me to 120 degrees bent, but only -5 straight. I can get about 100 degrees after mua (forced, but only in one position - on the slider board). Any other position there is poor range of motion. I. can’t even put a sock on, I need assistance. I am so discouraged and I feel like my life as I once knew it is over. I’m still using a walker 2 months later after initial surgery and recent manipulation. I feel like I’m just spinning my wheels because I’m not improving and I don’t want to go through another manipulation. Is all hope lost for me or is will I improve over the next few months with just pt? I feel tight and stiff all the time and I can’t get a good nights sleep because I can’t get comfortable. Please tell me straight up what my progress is looking like. Thank you.

      @@calleecat your situation sounds like mine. I’m 6 weeks out and can use a cane but really shouldn’t as limp too much so back and forth with walker. Still so much pain. Flexion only 75. You’re not alone and hope you’re better by now.

    I'm going through the same thing . Ive been totally dedicated from day one and have worked hard towards my recovery . I did have 115 -118 at 4 weeks then when swelling started coming down , it then starting feeling like my patella (was a hockey puck) it has so much force against the components that it limits my flexion . I am now hovering around 110-112 at 6 weeks . My surgeon wants to do manipulation if not better in 2 weeks. I am 5'10 172lbs and in toned condition . I thought I would breeze right through this . So frustrated.

    Had the exact same issue with back of knee clicking pain and could not get past 90 degree flex so my new surgeon tried a cortisone injection in the popliteus to see if that was the issue and as soon as l had the injection under guided scanning the back of knee pain went in one second. Turns out its very common. So l had an MUA and resection of the popliteus 6 weeks ago and l am already back to a pain free 120 degree flexion.

    I'm 6 months out and only around 96 pushing hard to get there.... My surgeon just told me to straighten my quad.... And left the room.... Call me if you need me.... I guess I'm done!!!???

    Your body looks at an injury as an injury. Your body look at reparative surgery as more injury. I'm dealing with a frozen shoulder 9 months in and not responding to anything. probably gonna mua in January.
