So basically they both are super guilty to the point they see hallucinations because this all happened when they were twelve. Sunny then sleeps for 4 years while everyone tries to move on. When sunny comes out of hiding him and Basil go through some issues. Like LOTS of issues. The hallucination that they both share is this figment called SOMETHING which represents the mary’s body hanging out of guilt. Basil saw SOMETHING behind Sunny and tried killing it with garden tools but ended up stabbing Sunny in the eye. They go to the hospital and yeah. There are multiple endings with the hospital and i don’t remember all so if i recall the bad one is sunny commits suicide and good is they tell the truth and the guilt goes away. I did miss a lot of stuff like his headspace’s but it’s because i don’t know to well about those.

hellmari is bad version that resides in black space and real ghost mari is good one that tries to get sunny to move on. my theory is that she's the one (or something) that speaks to sunny when he's fighting his fears. she saved him from drowning. she got him to not be afraid of spiders. she defeated his fear of heights. and when its irl, eg in sunny's house, its her that tells him to persist, to focus and to calm down.
