Breaking news! We now know from a video by Chuck E. Entertainment that the first Monfort location had the extremely rare Prototype Concept Unification Stage! They likely had it in either 1988 or 1989 because the showtape in the background, The Tune Machine, played around those times.
The magic stage was made because at that time CEI was selling their shows to other restaurants so the magic stage was supposed to make showbiz's stage unique than the other restaurants who bought the rockafire.
to answer your question about the first version of phase 4's art, it included the records or whatever art was in the show room pre remodel and framed versions of phase 3 art in white picture frames like the big city poster card, and the chef art on the wall you questioned about
