- Biceps Tendonitis (a.k.a. bicipital tendonitis) is an inflammation or irritation of the long head of the biceps tendon. When it gets irritated, it can be very painful and limit movement in the shoulder. Buy a worksheet with these stretches & exercises at www.askdoctorj...
The biceps tendon is a strong, cord-like structure that connects the biceps muscle to the bones in the shoulder.
To stretch the biceps muscles and tendons, you can stand in a doorway or use a sturdy chair to hold onto. With your palm open facing in front of you, place it on the sturdy chair or doorway and lean forward bringing your shoulder forward and across your body. Hold that for 30 seconds and do it three times.
Another way to stretch them is to use the seat of the chair, and place your palm down on it with your fingers facing away from you. Turn around so your arm is behind you, and again lean forward and push your shoulder in front of you and slightly across your body. Hold for 30 seconds and do three of them.
Now you are going to sit in a chair and use a resistive band. Anchor the band onto something sturdy or have someone hold onto it for you. This exercise will be for internal rotation. Anchor it to a table leg, in a door, or have someone hold it for you. If you want to roll up a small towel and place it between your side and your elbow, this will keep your arm close to your side through out the exercise. Keep your elbow at about a 90 degree angle and your thumb up towards the ceiling. Slowly pull your arm and the band in towards your stomach, and then slowly come back out. You don’t need to come back out far, just to where your forearm is straight out in front of you. Start off with 10 of these, and then work your way up to 20-25. If that becomes easy, then move up with resistive bands.
Now you will do a shoulder external rotation. The band will be on the outside of your body. Start off with your arm at your stomach. Try to keep your elbow by your side through out the exercise. If you want to roll up a small towel and place it between your elbow and your side, this will keep your arm close to your side through out the exercise. Keep your elbow at about a 90 degree angle and your thumbs up towards the ceiling. Also try to keep your wrist in a neutral position. You don’t want to over stress your wrist, and then have a wrist injury. Slowly pull arm out away from your body, and then slowly come back in. Start off with 10 of these, and then work your way up to 20-25. If that becomes easy, then move up with resistive bands.
The last exercise is bicep curls. You can step on the band, and keep your elbow by your side. Pull all the way up and all the way down. Make sure you are doing the full motion to maximize working the muscle. Make sure you are controlling the band; don't let the band control you!

Been dealing with this pain in my right arm for years, now suddenly I've got it in my left as well! Oh damn! Thanks for the help Dr. Jo, you rock~
You're welcome, and thanks!
What do you mean? Did these workouts help you?
@@kwachabodysculptors They did indeed
@@grahamn311, do you mind please sharing how often you did them? I’ve had this problem in my left arm for about a year and a half and haven’t found anything to help. I’m excited to try this, but don’t want to over or under do it.
Tendonitus for almost 6 months now. I stopped lifting for a few but it never really got better and I had cold weather spasms. So now I am just keeping it light and hoping it works itself out. 😛
Did it work itself out? Can you tell me what happened afterwards?
@@lemetamax for the most part. I started doing stretches and warming up more. Eventually it tooks few months but yes.
Great stretch! I didn't expect biceps tendonitis after ice skating but I guess I was flailing my arms around a lot. This video really helped me!
You're welcome! Glad it helped :-)
I did self diagnose 6 monthes ago thinking it was rotator cuff. Finally went to doctor today and got diagnosed as bicep issue instead. Going to PT Monday finally. Don’t self diagnose.
It's definitely very important to get a proper diagnosis first. Good luck.
does bicep tendonitis go away after time or will this keep happening, thanks
Nice video Jo! You are always so down to earth, informative and fun! xx
Thanks!
This tendonitis is super annoying. I can't even lift weights properly anymore and going to work is hell because it's all lifting for 8 hours lol.
Can you please make a series for Stroke recovery.

Hi Dr ,, which exercises should be avoided with bicep tendinitis?? I do weight lifting and I see my tendinitis I getting worse
Hi Dr' Jo. I hit the Subscribe button, thinking that it would take me to where I could make a donation, but it didn't. The Join button takes me to where I can make an ongoing monthly payment, but I would rather make a once off payment any time I visit. Is there a facility to do this please? I would like to ask, do you start the strength exercises (with the band) along with the stretches from the word go, or do you wait till the pain is gone before beginning strengthening? Your videos are a great help, as well as being joyful to watch. So many thanks indeed. Brian
Thank you! You can make a one time donation on my website: www.askdoctorjo.com/ It really just depends on how severe your injury is. I usually don't start with band work right away, but if it's not very painful, sometimes the lightest band can be slowly progressed into the routine in a day or two.
@@AskDoctorJo Many thanks indeed. I found the donation link. B
Thank you for your donation!!
Thank you for this video. I have a question, I'm experiencing some pain in my forearm/elbow in the lower bicep tendon region. Will these same exercises work for that?

Im diagnose with tendonitis with tear?... is it same exercises i need to do?..thank you
Hello Doctor, how many times should I do the exercises AND the stretches per day?
They can be done 2-3 times a day.
I dunno if this is the right video but my symptoms are: My right hand bicep area gets tired easily, makes a pop or crack sound if i raise or move my arm. But it doesn't hurt that much when it makes a pop or crack sound, it just hurta when i try to massage it and i feel a hard type of muscle that stucked in one place in my bicep. I haven't lost my strength tho and hopefully not cause I'm still a teen ager (18) and I've been searching for it if what is this annoying thing I've been dealing with for almost a year now. But i researched some in Google trying to find what it was, and it seems to be a "bicep tendonitis cause i play a lot of computer so i probably got it from computer mouse use. I'm sorry for making it so long but i hope anyone sees my comment. Thank you
Thank you so much doc! Really helpful and saved my time you deserve a like and a new subscriber :)

@@AskDoctorJo no thanks to you!
I broke my wrist about a month ago, had a plate and pins put in 2 weeks ago. In the fall I also strained my upper arm, the tendon I think, what do you recommend in situations where we can’t really move our wrist? Thank you and yes! Long time subscriber, really appreciate your videos!
Thanks! Sorry to hear about your fall. You could probably do some more gentle movements for the shoulder, just to keep it moving until you can do a little bit more once your wrist is a little more mobile. The first three of this video might help: ua-cam.com/video/aGxtFgyTbKQ/v-deo.html
Why the rotator cuff ex. are important for the biceps? Thanks for this video! Greetings from Bulgaria! :)
Many shoulder injuries are caused by weak rotator cuff muscles which in this case will shift the burden to the bicep tendon. Your shoulders will thank you for strengthening the rotator cuff(s).
Is running and sprinting fine during a biceps tendinitis ?
Thank you for the video! How often a week should we do these exercises for best results?
You're welcome! If you can, 1-2 times a day. Good luck.
Great, thanks! Has already been a half year so looking forward for it to be healed :)

I got electrocuted.. And my shoulder is in pain, it's still got a bit of ache even though it's already 5 days..
The symptoms is similar to this, so I'll try this exercise
I hope it helps.
hi, im a new subscriber and will definitely try your exercises.
after a long break (3 months) due to a shoulder injury and now covid i started going to my gym again (been working out for about 8 years or so).
-While performing a pulldown motion on a machine my left under scapula hurts and feels like its jumping over a muscle. i know its from bad posture in front of the computer as i work from home and will try my best to fix it
-While doing a shoulder press my left front deltoid hurts, same goes when i take my shirt off. And it sometimes continue towards the left elbow-golfers elbow
Should i do these stretching exercises everytime i hit the gym? that will slowly be up to 5 times per week
I will probably ask a few additional questions once you reply. thanks in advance :)
Thanks. Therapeutic exercises can be done every day, 1-3 times a day.
You’re awesome
Thanks!
Doctor , there is a pain on my short head of the bicep. It feels like short head of bicep gets pull toward the shoulder .please give some guidance 🙂.
Dr. Jo, I'm currently doing therapy for a slap tear and anterior instability that occurred when I took a hard fall during skiing. My therapist is doing a great job and I'm definitely seeing some improvement after 5 weeks so far. Most of my soreness/pain seems to come from biceps and side of the arm when I'm doing exercises which I know can be an issue with slap tears. Which exercises can I do that help the bicep pain while not aggravating the slap tear or issue of anterior stability? Thanks!
If you feel like you are getting better, I would stick with what your therapist is doing. Since I can't evaluate you, I really wouldn't want to give you anything different. Good luck.
Thank you for another informative video! :)
You're welcome!
at what point should one start these exercises. I have pain doing the first exercise. the pain first appeared 7 days ago and is not going away even after repeated treatment of ultrasound and rest and ice. thanks for the video
It really depends on what is going on. Do you remember any injury? If some of them don't hurt, I think they are fine to do, but if they cause increased pain, I would hold off until you know for sure what is going on. Good luck.
So I had this so terrible in left arm when I broke my shoulder. Surgeon did a biceps tendenesis. Now I have it in my good arm😩. Not sure reason I have it now but my question is how would exercising it decrease inflammation ??? I thought resting it would be better. Dont you hate nurse pts. ?? Lol I’m happy to do them if it saves me that surgery. Last time md said it was so inflamed it was ready to rupture.
Gentle stretches and exercises help decrease inflammation by improving circulation. It also helps keep the muscles/tendons lengthened so they don't heal in a shorted position and/or with scar tissue, which can cause chronic problems or risk for re-injury in the future. If there is any way you can see a physical therapist in person, they should be able to help you figure out a specific program for you and help answer your questions.
Perfect timing, thanks
You're welcome!

My long-head bicep is hot/inflamed, but is it normal that it pops/clicks everytime I lift it? Followed by ridiculous pain + inflammation.
I was told by an orthopedist that I might need a bicep tenodesis...I have had 2 surgeries on my shoulder in the last 6 yrs and this is a new issue from 10 months ago, and I did PT for 4 months + another PT session for 2 months. FML
That's not normal. What were the two surgeries for?
@@AskDoctorJo Thx for reply! :D 1st surgery was for Hill-Sachs lesion. There were complications. 1st dr had to do MUA. Now new complications with scar tissue. 2nd surgery was done by HSS dr to clean scar tissue and remove arthritis that was caused by the 1st surgery (mind you, I was 27 at this time, 31 currently). 2015 was the last surgery. It took 1 yr to get back to a good strength, and 2017-18 were good years. Popping was mild in my shoulder even after the 2nd surgery but it got a progessively worse at the end of 2018.
You're welcome! Sorry to hear. Sounds like that should have been done in the first place. Since the biceps tendon is attached to the labrum, it can cause pain, irritation, and even loss of stability from constant pressure on it. And if you had a dislocation in the past, it was probably already compromised. If you've already tried PT that long, it might be worth considering it. You will just need to decide if it's affecting your quality of life enough for another surgery.
The variation with the chair is the best biceps stretch I have done. Just one question. If I do it with my palm up, will it stretch more of my lower biceps or does it not matter if my palm is up or down?
Asking because I have a distal biceps tendonitis and my upper biceps head is a little impinged because of rounded shoulders.
Thanks for answers! :)
It might stretch a little more at the lower end, but I feel like with the palm down, you are going to get a better overall stretch. Good luck!
Okay, thanks, will try it both ways but will have that in mind.
Keep up your videos, the shoulder ones have helped me a lot!
You're welcome! I'm glad they have helped.
Will these same workouts help when bicep pain is closer to the elbow? When I have my elbow bent at 90 degrees and palm facing up, I can feel a lot of pain in my bicep but it is closer towards the elbow and not near the shoulder.
They should still help.
Doctor my, I have pain in my bisceps part I dont know if its bisceps muscle pain.when I throw ball it pains at biceps please help me doctor what kind off pain it is..
I had shoulder decompression surgery about 5 weeks ago and seem to have developed this bicep tendonitis. I will give these exercises a try and see if they help until i go see my physiotherapist but how often should these be done? Should i try incorporate these into my exercises for the shoulder or would doing both at the same time be too much at once?
Thanks
I think it’s fine to incorporate them into your other routine. Good luck.

Hello dr. Is one not supposed to lower all the way when doing the bicep curl? It looks like you're lowering 75%
You are, it just looks off because of the angle. Good luck.
how about forearm tendonitis?

I have pain in bicep and elbow joint. Only difference is that the pain is not in outside or lateral. But it is inside elbow. right where the bicep muscles is starting. I have seen your tennis/golfer elbow stretches. But it is not helping me. Can you please suggest any other stretch.
Have you tried these yet?
Yes.
Unfortunately, without getting to evaluate you, I really don't know what else would help.
Can I have a plastic surgery to get rid of the biceps
Hi Doctor,
I have mostly pain/discomfort in the front of my shoulder, especially if i do a lateral raise's move.
So could this be my biceps long head that is affected or would it be rather my supraspinatus (but it doesn't matter at all when my physician do resistance's test on it) ?
Best regards
It could be either unfortunately, or even an impingement. Did you have a specific injury?
Love your videos Doc! You communication and technical skills are excellent. But I have to say, your disclaimer alert takes the cake!
I get bicipital tendonitis about 6 hours after a TRX workout and stay for days. These stretches instantly helped.
Looking for your suggestions on should I lay off workouts for a few days? and when to incorporate these into my routine (before or after)?
Thanks!
Thanks :-) It might be worth holding off for a day or two, or at least go lighter on your workouts. If you have time, I would do them before and after, but at least before. Good luck!
@@AskDoctorJo Thank you Doc, much appreciated!

dr jo...i have had 5 subluxations and 2 dislocations in my left shoulder. please make a vid...with exercises to regain strength & ROM.
also I have a subuxated bicep tendon in my right shoulder. does that mean I tore my Subscapularis rotator cuff tendon??
please reply.
I have a video for instability: www.askdoctorjo.com/content/shoulder-instability-exercises It doesn't necessarily mean you have a torn rotator cuff, but with that many injuries, it is likely. Good luck.

I have a question Dr. Jo been. I have been working out in the 2 days and i did some low pulley behind the body curl i did my set i was, i increased the weight then it was ok., third one i put my wrist with the same weight when i was curl the weight after i finished i was feeling pain like inflammation or irritated to middle of the biceps down the biceps. Maybe its a tendinitis or i dont know and i dont know should i keep working too?
Unfortunately, I don't know since I can't evaluate you. If you can, you should go see a physical therapist for treatment.
Ok thanks alot Dr. Jo to answer my question so fast :)
You're welcome. Good luck!
I've had this for decades now, since high school basketball. If I do this every day, roughly how long should it take before I could go back to the gym without my shoulder getting inflamed? A month or two?
And who was the mystery person holding the band?!?!
Have you had imaging done? That seems like a really long time for tendonitis. I would really recommend getting it checked out to find out for sure. Mystery person?? It's magic :-) Good luck.
Yeah, I got an MRI done a few months back. They said it was just inflamed. I didn't meant to say it's been continuous since high school, it just started then. Flares up after a month or two of regular workouts or repetitive overhead work.
Have they tried a cortisone injection?
They said it wouldn't be a good idea because it would only be a temporary fix. Anyway, I've been doing your exercises for the last few weeks (as well as a few others for my winged scapula) and it's working great. Thanks!
You're welcome! I'm glad it's working.
What if you can't get arm behind your back to do excercise or lift it up and down all the way. What is that.

Hi Jo, about 6 months ago I injured my arm which my doctor diagnosed as an AC joint strain. However the pain is still present and on my bicep tendons. Do you think this is a sign of a partial tear to have pain this long and are these exercises still relevant for a tear instead of tendonitis? Thanks!
